Report of Dog Show or Obedience Trial (Licensed or Member Clubs Only) Upon completion of a Dog Show or Obedience Trial, the Superintendent or Event/ Trial Secretary must fill out this report. This report along with the event records described in the Rules Applying to Dog Shows, Chapter 17, Section 2, and the appropriate fees must be received by Event Operations within seven days after the event. Please answer all questions; if additional space is needed please attach 8½”x 11” paper only. Each event, including combined Specialties, Back-to-Back All Breed, and Limited Breed Events require a separate report for each event. (There is no recording fee or event service fee for junior showmanship, sweepstakes, futurities, brace/team, multi-dog classes, obedience/ rally non-regular classes, or special attractions.) Event Secretary Individuals may act as Event Secretary for up to 8 events per year with no fee due. The fee for each event after the eighth is $10 per event. Superintendents pay for each event.
