Dear Campers and Parents, Please chose which session(s) your child will attend, you may send the completed registration form with the required $50 non-refundable payment for each session. We will not hold a space for your child unless we receive the deposit. Please return your balance due with your completed forms by June 5. Balances and completed forms must be received prior to the first day of camp, as campers will not be allowed to start camp without complet- ing this process. Checks should be made payable to Playful Minds. Also we are now accepting Visa or MasterCard. Playful Minds highly recommends families to sign-up for Tuition Express. DROP OFF Arrival to camp is 8:30 AM. Please park and walk your child to his/her councilor PICK UP Dismissal from camp is 4:00 PM. Please make sure your camper is signed out daily. Please be aware that there is a charge for late pick up. PRE CAMP Pre Camp will run daily from 6:30 AM to 8:30 AM please meet at the picnic tables and remember to sign in and leave pay- ment with the counselor POST CAMP Post Camp will run daily from 4:00 PM to 5:30 PM. 1. Campers are expected to be respectful to all Playful Minds counselors, administrators, peers and all other

Playful Minds staff at all times.

2. Campers are expected to be respectful to the property of Playful Minds and the property of others.

3. Campers are expected to wear clothes, SOCKS AND SNEAKERS at all times. You will need to provide

water shoes for out door play.

4. Campers are expected to always use pleases and thank you.

5. Campers are expected to clean up after themselves and throw out their trash.

6. Campers are expected to recycle whenever possible.

7. Campers are expected to use appropriate language at all times and are expected to not bully any other children.

8. Campers are expected to ask a counselor to go anywhere.

9. Campers are expected to take responsibility for their actions and tell the truth, even if it means admitting

to wrong doing.

10. Campers are expected to take responsibility for their own property. They should keep track of their towel, bathing suit and all other property they bring with them to camp.

11. Campers are expected to leave all toys, video games and other electronics at home. They are not allowed

at camp.

12. Campers are expected to not draw or use pretend weapons, including guns, at any time.

13. Campers are expected to participate in activities without exhibiting rough or aggressive behavior.

14. Campers are expected to keep their hands, feet and the rest of their bodies to themselves. Any fighting will be brought directly to the director.

Please thoroughly read these regulations. Any violation to these regulations may result in an Incident Report, Parent and Director Meeting or possible suspension or expulsion from camp.

Campers will not use obscene or vulgar language.Campers must not leave Playful Minds premises without permission.Campers will not speak or act disrespectfully to any peer or adult. Campers will not threaten, harass or physically harm another person. Campers will not posses or distribute indecent literature.Campers will not intentionally destroy or vandalize any Playful Minds property.
