1 Relationship Builder Introduction The Relationship Builder is a powerful tool that can ensure the most current assessment data is updated to the care plan. Relationship Builder allows creation of rules that specify the conditions in which specific data elements in an assessment, when assigned a particular value, will trigger a window in the care plan module where the user can view the information to help with care planning decisions. By simply making the selection right on the window, the care plan is automatically updated. Rules can be built for any type of assessments. For each rule condition, it can include a set of outcomes and interventions. For example, if the assessment indicates that the client is a high risk for nutrition, the rule will trigger the window in care plan to show not only the value to indicate that the client is high risk, but it will also show a list of possible high risk best practice interventions for the user to consider when planning care for the client. A rule for low risk on the other hand will present its own set of outcomes and interventions. Having information readily available to the users will help enforce compliance with established policies for clinical care, practice standards, and best practice guidelines. MED e-care has created sets of rules for some of the most commonly used assessments such as MDS-RAI, Resident Assessment Protocol (RAP), and other risks assessments i.e. skin, falls, and nutrition. The organization can decide on which set of rules they would like to be deployed to their live environment. The existing rules can easily be deactivated, deleted, and modified.

The Relationship Builder is a powerful tool that can ensure the most current assessment data is updated to the care plan. Relationship Builder allows creation of rules that specify the conditions in which specific data elements in an assessment, when assigned a particular value, will trigger a window in the care plan module where the user can view the information to help with care planning decisions. How to build the care plan

using rules

Procedure:

1Click on e-plan tab in the green bar.

2Click on Care Plan tab in the blue bar.

3Search for a client.

4When the client’s current care plan opens a window called Build to Care Plan opens if there are rules that have been triggered.

5Select from the left side of the window the sections of the care plan to work on. Example:

Eating.

6Review the information associated with the section selected.

7The Available to Build area outlines the data elements from assessments. There may be different data elements coming from different assessments. Therefore, in this section of the Build to Care Plan window, the assessment name, the date the assessment was completed, and assessment code for the data element are included with each data element and the value for that data element when the client was assessed.

8Review the assessment data against the data in the Current Care Plan as shown on the right side of the screen.

9Select the data elements to be pushed to the care plan.

10Click on the blue button called Build.

11Click on Remove Selected when choosing not to consider a data element.

12Click on Close to close the window. The window will close but the window will reappear with

the same information if the information has not been addressed, which means either the data

elements have been built into the care plan or have been removed using ‘Remove Selected’.

How to create rules

Procedure:

1Click on Settings tab in the green bar.

2Click on Relationship Builder tab in the blue bar.

3Go to Choose an Option. 4Click on Facility Rules. 5Click on Create Rule.

6Create a Rule Name. 7Select the Assessment Tool.

When looking at an assessment, note the following areas.

8In the rule, select a Section. 9Then select a Group within the Section. 10Then select a Field within the Group.

The blue are is referred to as the Group.

The white area is referred to as the Field.

The green area is referred to as the Section.

11Select the value(s) for the data field to be considered in the rule. 12 Decide how the value(s) will be considered in the rule. Select the appropriate logic operator to define the condition under which the value(s) will trigger the rule. In the example below, to know if the client has ‘a fall resulting in a fracture’, the rule condition would be “field #3 is = (equal) to 1”. When this field is coded as 1 in the assessment, this rule will trigger. Refer to the Notes section for the different types of logic operators.

13Decide the type of care plan where the information will be available. To do this, select the Care Plan type by clicking on the Green Plus icon. This action will prompt a message box to select the type of care plan.

15Decide whether the information is considered to be a problem, an outcome, or an intervention.

20Select the section of the Care Plan to associate the information. In order to do this, select the Domain, then the Class, and then the Diagnosis.

How to group conditions to

create a rule

Procedure:

1Click on the Green Plus icon to add another condition for the rule.

2Select the conditions for each data element to be considered in the rule.

Conditions Definition

And This is used when there is more than one data element involved in the rule. The conditions for all data elements must be satisfied for the rule to be triggered.

Or This is used when there is more than one data element involved in the rule. This outlines the different conditions that could trigger the rule not requiring that all conditions must be satisfied.

Grouped And This is used when there is more than one set of conditions. All sets of conditions must be satisfied for the rule to be triggered.

How to delete a rule

Procedure:

1Click on Settings tab in the green bar.

2Click on Relationship Builder tab in the blue bar.

3Select the Assessment Tool and it will list all the rules associated with that tool on the left hand

side under Rule Name. Select the rule to be deleted or Type the name of the rule and click

Search.

4Click on the Delete icon. This action will prompt a message box.

Notes for Relationship Builder

Rule Name: The rule name must be unique. The system will not allow duplicates. It is recommended to create a name that easily identifies the data element associated with the rule.

User Role A security role can be assigned to allow the user to view the Build To Care Plan window.

Procedure:

4Select ‘Relationship Builder’ as the Category and Object Type. 5Check the Full Control box to give the user full access to Relationship Builder. Then click the Save button. Unclick the box if the user does not need to see the Build To Care Plan window.

Build To Care Plan Window There is an option in Settings for the window to pop-up only when the assessment is completed and signed. This is the default option.

Logical Operators

The proper selection of the logical operator is critical in ensuring the rule is triggered properly. The logical operator will tell the system when to trigger the rule based on what value was selected as follows:

The rule will trigger if the coding in the assessment is more than or exactly the same as this value.

between between The rule will trigger if the coding in the assessment is inclusively between the 2 values specified in the rule. For example, between 1 and 3 means the rule will be triggered when the coding is 1, 2, or 3.

in in The rule will trigger if the coding in the assessment is according to the specified values in the rule. For example, if the rule says “in 1, 2 and 8” means the rule will be triggered if the coding includes all the three values 1, 2, and 8.
