OveRviewHeart failure (HF) affects as many as 6 million adults
in the united States, and the prevalence is anticipated to increase
to more than 8 million by 2020. mortality rates associated with HF
have been on the rise for the first time in more than a decade. as
management guidelines and treatment options continue to evolve, it
is increasingly crucial that clinicians who treat patients with HF
understand the latest approaches for optimizing care. this cme
activity will apply knowledge of the latest guidelines, clinical
evidence, and quality measures to the treatment of patients with
HF, including special considerations when managing HF in african
american adults. Strategies to optimize care coordination
throughout the health care journey of patients with HF will be
discussed. the interactive case-based format is intended to help
clinicians integrate scientific advances into real-world practice
settings.
