SEAMEO TROPMED REGIONAL CENTRE FOR MICROBIOLOGY,

PARASITOLOGY AND ENTOMOLOGY INSTITUTE FOR MEDICAL RESEARCH

KUALA LUMPUR, MALAYSIA

1. Introduction

The Institute for Medical Research (IMR) is one of the network of research

institutions under National Institute of Health (NIH), Ministry of Health Malaysia.

The NIH identifies research priorities areas through conduct of research, leading

to utilization and translation of research findings into new knowledge,

development of better tools for the diagnosis and management of important

health problems, health policy formulation, health management, and health

promotion.

IMR main function is to carry out research to identify, elucidate, control

and prevent diseases and health issues prevalent in the country. Its mission is to

improve health by 1) carrying out quality biomedical research to address national

health priorities, 2) providing specialized diagnostic services and 3) building

national capacity through technology transfer and consultative services. IMR also

provide specialized diagnostic services, training in specialized fields and

consultative /advisory services to health providers, managers and planners in

improving the health of the Malaysian population.

IMR’s vision is to be recognised internationally by the scientific community

as the leading centre for biomedical research. IMR Research programmes are

geared towards these various identified priority areas by NIH and in line with the

health problems of the country.

Health Research Priorities in Malaysia for the10th Malaysia Plan (2011 – 2015),

is to improve the health and quality of life of the people of Malaysia. This, in turn,

would enhance their productivity and competitiveness, thereby contributing the

economic development. Research findings and outputs are used by managers

and administrators of Minsitry of Health Malaysia, other Ministry and government

agencies which are related to health to improve the health status, promote

wellness and to reduce the burden of disease among Malaysians. This is towards

becoming advanced nation’ status by 2020.

IMR, one of the research institute under the NIH, will contribute to this

vision. IMR consisted of seven Research Centres (Allergy and Immunology,

Cancer, Cardiovascular, Diabetes and Nutrition, Environmental Health, Herbal

Medicine and Infectious Diseases Research Centre) focusing on biomedical and

clinical research, and supported by one Diagnostic Centre.

The IMR has 805 posts, of which 638 (79.25%) have been filled. There are

12 individuals in the Senior Management group, 208 individuals in the

Managerial and Professional Group, comprising scientists, doctors, dentists,

veterinarians, librarians, system analysts, a statistician, a pharmacist and

administrative officers. The rest of the staff i.e. 418 persons belong to the

Technical and Support group. The total expenditure of the Institute in 2011 was

about RM 83.8 million.

2. SEAMEO TROPMED Network Malaysia

SEAMEO TROPMED Network Malaysia is one of the international affiliation of

the Institute. The Network serve as the Regional Centre for Microbiology,

Parasitology and Entomology, where one of the function is to facilitate the

strengthening of national and institutional capabilities in research and human

resource development through various training and postgraduate degree

programmes, linkages, personnel exchanges, scientific forums, technical

consultant services, publications and information dissemination. SEAMEO

TROPMED Network Malaysia, conduct two regular post graduate Diploma

courses each year; the Diploma in Medical Microbiology (DMM) and the

Diploma in Applied Parasitology and Entomology (D.A.P&E).

2.1 Academic Programmes

In 2010, both courses undergoes reorganization in terms of schedule and sylybus

such that they can be conducted within a year. In 2011 the DMM course was

conducted from March to July followed by the DAP&E from August to December.

Both were conducted for a duration of 5 months each. The courses were

designed to train Medical Doctors, Scientists and other health personnel in basic

and advance Medical Microbiology, Parasitology and Entomology, as well as

skills in conducting research, diagnostic services and approaches in control

programmes.

2.1.1 DMM

The course aims to provide knowledge and practical training on pathogens of

major significance to public health and to enable participants to apply scientific

approach and critical thinking to diagnose, prevent and control infectious

diseases. The course will facilitate and strengthen institutional and national

capabilities in medical diagnostic laboratory services, research and human

resource development. It is also hoped that this course will encourage networking

between the various participants’ institutions and ours to foster exchange in

expertise and collaboration in research projects. In 2012, the course was

conducted from 13 February to 13 July 2012 with 13 participants coming from

one each from Lao PDR, Vietnam, Myanmar and Cambodia, two from Sri Lanka

and 7 from Malaysia. Each student conducted an assignment on topics related to

the subject of the module, they are as listed in Appendix 1. All the students

passed the examination and Mrs. Nilanthi Perera Medical Laboratory

Technologist from the Microbiology Laboratory, Service Lab Section, National

Institute of Health Sciences, Nagoda, Kalutara, Sri Lanka was awarded the best

student for the year 2012.

2.1.2 DAP&E

The Course is designed to train doctors, scientists and other health personnel for

research, diagnostic services, control programmes and teaching institutions in

Applied Parasitology and Entomology. The course is conducted yearly. In 2011,

the 42th Course, was conducted from 1st August to 30th December 2011,

attended by 16 candidates from 7 countries, one each from Cambodia, Myanmar,

Lao PDR and Vietnam, four from Solomon Island, two from Vanuatu and six

from Malaysia. The SEAMEO-TROPMED sponsored 7 candidates, 1 each from

Myanmar, Lao PDR, Vietnam, Cambodia and 3 from Malaysia. The GLOBAL

FUND sponsored 4 candidates from Solomon Islands and the Australian Aid

sponsored 2 students from Vanuatu. IMR sponsored 3 students from the

Institute. As a fulfillment of the course, each student conducted a research

projects, and the list of the research projects are as listed in Appendix 2. The

final examination was conducted from the 22 to 29 December 2011. During the

examination we were assisted by two external examiners, Dr Dorn

Watthanakulpanich, Department of Helminthology, Faculty of Tropical Medicine,

Mahidol University, Thailand and Associate Professor Dr Zairi Jaal, Vector

Control Research Centre, Universiti Sains Malaysia, Penang. Their expert

contribution and opinion were invaluable. The examination concluded with fifteen

students passing the examination and was awarded the Diploma in Applied

Parasitology and Entomology. Two of the fifteen students passed with Distinction,

there were Dr. Aye Thinda Win and Ms Nurhainis Ogu Salim. One student was

unsuccessful and was awarded a “Certificate of Attendance”. The Top Student

Award was awarded to Dr. Aye Thinda Win and she received a prize from the

Director of SEAMEO TROPMED Network, Malaysia. The Best Thesis was

awarded to Mr. Trevor Desmond Punufimana from Vanuatu. The title of the

thesis was “Overlapping of Different Flies From Different Animal Facilities. He

received the Deans Best Thesis Award.

3. IMR Research Activities In 2011, staff members of the Institute were engaged in 50 research projects

comprising several disciplines including Infectious Tropical Diseases such as

dengue, malaria and Chikungunya, Environmental Health, Drug Discovery,

Stem Cell, Tuberculosis research, and others. Majority of research projects are

sponsored by the Ministry of Health Malaysia, and some by other International

and National funding agency. The lsit of research projects conducted in 2011 and

2012 are listed in Appendix 3 and Appendix 4. The Institute published 98

scientific papers in 2011 (Appendix 5) and 43 publications until June 2012

(Appendix 6).

3.1 Allergy and Immunology Research Centre

The research carried out is a continuation of that done in 2010. The Allergy Unit’s

research focused on identifying and characterising seafood allergens as well as

establishing the basophil activation test. The Autoimmune Disease Unit studied

the role of autoantibodies in foetal loss, celiac disease and paraneoplastic

syndrome. The unit also commenced research to determine the role of Human

Leukocyte Antigen and Anti-Aquaporin 4 antibodies in patients with Multiple

Sclerosis. The unit coordinates the National Quality assurance Programme for

Antinuclear Antibody Testing (ANA). The Immunogenetics Unit carried out

research in the association of disease with major histocompatibility complex. The

Primary Immunodeficiency (PID) Unit continued to study and characterise

patients and families affected by immunodeficiencies, particularly those with B

cell defects, neutrophil dysfunction and Wiskott-Aldrich syndrome. The PID Unit

also continued the study on the role of cytokines and other immune parameters

in the pathogenesis of dengue.

3.2 Cancer Research Centre

The Haematology Unit of the Cancer Research Centre continued to carry out

research in haematological malignancies and stem cell. The availability of single

nucleotide polymorphism arrays has been applied in hematologic malignancies to

detect deletions, amplifications, loss of heterozygosity (LOH) and uniparental

disomy at high resolution. The unit have identified genomic imbalances in adult

acute lymphoblastic leukaemia (ALL) by genome wide single nucleotide

polymorphism analysis. The unit have successfully identified 13 chromosomal

aberrations, 24 Copy-neutral LOH (CN-LOH) regions and 23 SNPs in adult ALL.

By using array CGH technique, the unit also successfully identified common

chromosomal aberrations in multiple myeloma patients. Interestingly,

chromosomal aberrations were detected in 100% samples studied. About 90% of

these samples were having normal karyotype with conventional cytogenetic.

Array CGH can be used as an alternative tool for the identification of copy

number variations in myeloma patients in place with conventional cytogenetic.

This is especially important if the information is important for patient’s prognostic

prediction and treatment decision making. The identification of genomic

imbalance has contributed toward the knowledge base of the human genome

and lead to a better understanding of the molecular pathogenesis of the

haematological cancers.

In stem cell research, the unit studied the multipotency of limbal stromal cells and

how these cells differ from mesenchymal and embryonic stem cells in terms of

differentiation potentials and gene expression profiles.

The Molecular Pathology Unit of the Cancer Research Centre continued to carry

out studies on cancer and genetics, with a particular emphasis on

nasopharyngeal carcinoma (NPC), a major cancer in Malaysia.

Oral Medicine study on retrospective study of patients seen at the oral medicine

clinic, Kuala Lumpur Hospitaly was conducted by the Stomatology research

group.

3.3 Cardiovascular, Diabetes & Nutrition Research Centre (CDNRC)

The Nutrition Unit research activities focus on the area of functional food and

analytical food chemistry. The current project is on the updates of Malaysian

Food Composition Database. The research focuses on analysing new raw and

processed foods available in food list. Local Malaysian seaweeds are being

studied where selected Malaysian seaweeds were analysed for its nutritional

values and identifying functional food properties in seaweeds for in vitro lipase

and amylase enzyme inhibitory activities.

3.4 Environmental Health Research Centre (EHRC)

Environmental Health Research Centre continued to conduct research with focus

on quality of air, water and assessment of environmental contaminants in food.

However research activities had mainly centred on quality of indoor air in hospital

premises supplied by Air Handling Units (AHU). This study was using parameters

based on ICOP and ASRAE guidelines and standards. The centre provided

expertise to other organization in water sampling activities using ultrafiltration

methods as well as testing the water sampled for the detection of sarcocystis.

The environmental research group received funding from WHO to conduct

research on the development of a risk assessment tool for managing the health

impact of climate change. This project is in line with Malaysia’s commitment to

addressing the issues of climate change, specifically in protecting health from the

impact of global warming due to climate change. At the regional level, Malaysia is

committed and contributing to climate change initiative. The project seeks to

identify populations most vulnerable to these impacts, develop a functional

climate and disease mathematical model for better understanding the relationship

between climate change and for forecasting of health outcomes, to assist

relevant stakeholders to take steps to assure that these health risks can be

managed now and in the future.

3.5 Herbal Medicine Research Centre

Research on drug discovery for neglected tropical diseases (Filariasis, malaria

and Dengue) are being conducted with compounds obtained from collaborative

projects with Drug Companies locally and Internationally. Herbal medicine

research focus on natural product discovery from medicinal plant with traditional

claims in development of standardized extracts and isolation of compounds for

common diseases in Malaysia, including the anti-proliferative activities of the

plant extracts to breast cancer and Colorectal Cancer cell lines, anti hypertensive

and anti oxidant activity.

Herbal Medicine research group are continuing with the compilation of traditional

and scientific uses of local medicinal plants for documentation purposes. The

Global Information Hub on Integrated Medicine ‘GlobinMed’ project is ongoing

and to present up-to-date and evidence-based information on T&CM (Traditional

and Complementary Medicine), by doing it in-house and also working closely with

both local and international partners.

The Malaysian Herbal Monograph being one of the EPP (Entry Point Project) of

the NKEA (National Key Economic Area) project, embarked on research on

preclinical and clinical research for medicinal plant products to provide scientific

data for evidence of claims and for regulatory requirements for

commercialization.

3.6 Infectious Diseases Research Centre (IDRC)

Research in the Acarology Unit continued to focus on 3 main areas: the detection

of pathogens in ticks, establishment of a PCR based blood meal identification

assay (BMIA), and control of acarine pests of public health importance.

The Bacteriology Unit, developed PCR for Bordetella pertussis and Brucella

species. An increase in the number of request and positive cases was noted for

Bordetella pertussis for the year 2011. The emergence of multidrug-resistance

gene NDM-1 among Enterobacteriaceae isolated from patients around the globe

has led to the developing of an in-house PCR technique to detect this gene

among local isolates. The detection of this gene is crucial for infection control

measures and the Bacteriology Unit is the referral laboratory for the confirmation

of strains carrying this multi-drug resistant gene.

The Medical Entomology Unit continues with the problem-solving research in

medical entomology. Research on dengue vectors especially Aedes aegypti is

still the main priority for this Unit. The project on transgenic mosquito continued

to move forward this year. Other studies on dengue vectors were related to the

mosquitocidal effects of various seaweed extracts on the growth and

development of Ae. aegypti and Ae. albopictus. With regard to dengue vector

surveillance, studies using a modified ovitrap enhanced with hay infusion were

found to be effective as an entomological tool for Aedes surveillance. Studies on

the effectiveness of thermal fogging for dengue control in closed premises were

carried out.

In malaria vector research, retrospective assessment was conducted on two

long-lasting insecticidal nets (OLYSET® and PERMANET®) after two and four

years of household use in Sarawak and Laos respectively. The effectiveness of

these nets was tested against laboratory bred Anopheles maculatus. Both nets

can be effectively used to prevent mosquito bites even after field used for two

years in Sarawak and four years in Laos. Bionomics of the malaria vector An.

maculatus in the field was studied and map of larval breeding habitat distribution

and malaria transmission risk areas were developed using a combination of field

data, satellite image analysis and GIS technique. Studies pertaining to insecticide

susceptibility status of An. cracens, vector of Plasmodium knowlesi in Peninsular

Malaysia were also conducted.

In forensic entomology, study using molecular approach to identify forensically

important fly species was successfully completed. A first record of Piophila

foveolata larvae (skipping larvae) found on human corpses was reported. DNA

profiling for identification of forensically important flies showed that all stages of

insects including eggs, egg skin, larvae, pupae, pupa case, adults, and damaged

or inappropriately preserved entomological specimens can be employed to

identify the species. DNA-based characterisation of selected important forensic

Sarcophagid fly in Malaysia was also investigated to facilitate species

identification.

The Virology Unit focused on research pertaining to locally important medical

viruses, aiming to determine the epidemiology of the viruses and the

development of new technology for rapid diagnosis of these viruses.

The Virology Unit performed satisfactorily for WHO, RCPA (Australia) and NRL

(Australia) Quality Assurance programme for various serological, viral isolation

and molecular tests. The National Poliovirus Laboratory received accreditation for

Viral Isolation and Identification testing for the 13th successive year and was

accredited and entrusted to perform the Vaccine Intratypic Differentiation (ITD)

testing and testing for Vaccine-Derived Poliovirus by WHO. Also in 2011, the

National Japanese Encephalitis laboratory received full accreditation by WHO.

The programme of HIV sero-surveillance continues with the Virology Unit serving

as the National AIDS Reference Laboratory (NARL) for the study of cases with

difficult or unresolved serology. The Unit continued to provide consultancy

services for babies born to HIV positive women. The Unit also carried out

surveillance programme on circulating dengue virus serotype, Nipah, JE,

influenza and hand, foot and mouth diseases for Ministry of Health. The dominant

circulating dengue serotype for 2011 was Dengue-1 and Dengue-2 and as for

influenza, A/Perth/16/2009-like (H3N2) and B/Brisbane/60/2008-like strains were

found to be the predominant strains for influenza A and B respectively. This

differs from 2010, whereby A/ California/7/2009-like (H1N1) strain was the

predominant strain for Influenza A.

3.7 Medical Resource Research Centre (MRRC)

The Medical Research Resource Centre (MRRC) consists was responsible in

providing supports in particular on training for officers especially the new medical

and research officers in research methodologies, statistical software programmes

applied to biomedical research such as SPSS and STATA. The centre was also

responsible to conduct several motivation workshops and training on Microsoft

Office which included MS Word, Excel, power point and Access and designing

website for the general staff of IMR.The centre was also involved in several

research projects. They include the effectiveness of pneumococcal vaccine

among Malaysian pilgrims performing hajj in Saudi Arabia 2011, the health status

of Orang Asli in Peninsular Malaysia and incidence of human plasmodium

knowlesi malaria, host-parasite interactions and associated risk factors in

Malaysia.

3.8 Specialised Diagnostic Centre focused on research and development

towards expanding molecular diagnostics testing with the objective of primarily

developing more new molecular genetic testing for the confirmation of selected

genetic diseases including IEM. The unit conducted R&D projects for 12 pediatric

disorders and the outcome of these are 19 new molecular diagnostic tests for

mutational analysis for 17 new genes and two for deletion/duplication of genes.

4. Diagnostic services.

Being the referral laboratory for the Ministry of Health, the IMR continues to

provide and improve clinical laboratory tests. IMR provides specialised and

referral diagnostic tests, and tests that are not perform in other laboratories.

The Bacteriology Unit continues to provide diagnostic services to government

and private hospitals in Malaysia. The Unit is the referral laboratory for the

diagnosis of leptospirosis, pertussis, brucellosis, verification of antibiotic

resistance, serotyping and DNA fingerprinting of food and waterborne diseases. It

plays an active role in the investigation of nosocomial infections.

IMR is Referral Centre for:

The Biochemistry Unit of the Diagnostic Research Centre, has set up a

comprehensive screening of urine for inborn error of metabolism using

electrospray ionization tandem mass spectrometry.

4.1 IMR is Referral Centre for:

1. WHO Collaborating Centre for the Ecology, Taxonomy and Control of Vectors

of Malaria, Filariasis and Dengue for another 4 years.

2. WHO National Influenza Centre,

3. WHO National Laboratory for Poliovirus. In 2011, the laboratory received

accreditation for Viral Isolation and Identification testing for the 13th

successive year and was accredited and entrusted to perform the Vaccine

Intratypic Differentiation (ITD) testing and testing for Vaccine-Derived

Poliovirus by WHO.

4. WHO National Japanese Encephalitis Laboratory, It received full accreditation

by WHO in 2011,

5. WHO, RCPA (Australia) and NRL (Australia) Quality Assurance program for

various serological, viral isolation and molecular tests.

6. National AIDS Reference Laboratory (NARL) where programme of HIV sero-

surveillance continues for the study of cases with difficult or unresolved

serology.

7. Surveillance program on circulating dengue virus serotype, Nipah, JE,

influenza and hand, foot and mouth diseases for Ministry of Health Malaysia.

The dominant circulating dengue serotype for 2011 was Dengue-1 and

Dengue-2 and as for influenza, A/Perth/16/2009-like (H3N2) and

B/Brisbane/60/2008-like strains were found to be the predominant strains for

influenza A and B respectively. This differs from 2010, whereby A/

A.

diagnostic for genetic diseases including Inborn Errors of metabolism (IEM).

5. Consultative services

IMR’s staff provides advisory and consultative services to the Ministry of Health

(MOH), other government departments, as well as international organisations.

Most units of the Institute also serve as referral centres to MOH laboratories

throughout the country. During the year, 86 staff members provided consultative

services at the national level, while 18 staff members provided such services at

the regional /international level.

6.Technical Training Programmes, Staff Development and Conferences

6.1 In 2011, 139 staff members of the Institute attended 54 conferences/scientific

meetings, whilst 643 staff members attended 372 short courses/workshops to

improve their knowledge and skills. There were 17 staff members who had

training overseas under the in service training scheme and 97 local training

courses were carried out. 34 members of the Institute are currently undergoing

post-graduate studies; 14 officers are studying for their Master degrees while 20

are pursuing their PhDs.

6.2 The Institute also conducted 32 training workshops, 17 seminars and 17

courses during the year.

6.3 The regular training courses include the SEAMEO-TROPMED postgraduate

courses namely, the Diploma in Applied Parasitology and Entomology and the

Diploma in Medical Microbiology courses.

6.4 The Medical Research Resource Centre in collaboration with SEAMEO

TROPMED conducted the 9th SEAMEO-IMR Course on Advanced Epidemiology

which was held at the Institute for Health Management in Bangsar from 26

September to 7 October 2011. A total of 30 participants attended this course.

They were researchers, physicians, statisticians and other health personnel from

various countries in South East Asia namely Thailand, The Philippines,

Myammar, Vietnam, Cambodia, Lao-PDR and Malaysia. The course was

conducted successfully.

6.5 The MRRC was responsible in providing support in particular on training for

officers especially the new medical and research officers in research

methodologies, statistical software programmes applied to biomedical research

such as EpiInfo, SPSS and STATA and few other sub-specialised statistical

softwares. The centre was also responsible to conduct motivation workshops and

training on Microsoft Office which included MS Word, Excel, power point and

Access and designing website for the general staff of IMR. The centre

successfully conducted a total of 16 courses throughout 2010.

6.6 The Environmental Health Research Centre (EHRC) conducts the annual

Environmental Health Forum with the theme of ‘Air Quality and Impact on Health’

attracted 196 participants to the IMR. Participants were from the Ministry of

Health, local universities and other ministry departments such as the

Occupational Safety and Health Department, Department of Environment,

Department of Chemistry and NIOSH. Speakers were from UKM Bangi,

Department of Environment and Institute Respiratory Medicine, Hospital Kuala

Lumpur.

6.7 EHRC has continued its role in supporting IMR as a member of the Working

Group on Innovation and Research under the National Committee on Green

Technology and Climate Change.

6.8 EHRC has also continued successfully to secure and maintain the MS ISO

14001:2004 Standard: Environmental Management System (EMS) certification

(since 2005).

6.9 The EHRC with Institute for Health Management (IHM) organised a meeting

with WHO experts for the development of a training module for Climate Change

and Health for health personnel. Three climate change experts from WPRO, Dr

Yasushi Honda, Dr Masahiro Hashizume, and Dr Ho Kim provided technical

assistants in the development of the training module specific for health

personnel.
