REQUEST FOR BID ON-CALL FIBER OPTIC CONDUIT NETWORK DROP INSTALLATION SERVICES

CITY OF WEST DES MOINES

ENGINEERING SERVICES 1. GENERAL STATEMENT

1.1 The City of West Des Moines, Iowa, solicits interested firms to submit bids for on-call fiber optic conduit network drop installation services for City of West Des Moines.

1.2 Submittals marked "Bid for On-Call Fiber Optic Conduit Network Drop Installation Services” will be received no later than 2:00 p.m., local time, on Wednesday, September 15, 2021.

1.3 It is proposed under this Request for Bids to provide for furnishing completely the requirements set forth in the Contract Provisions and Specifications.

1.4 The purpose of these Specifications is to require the furnishing of highest quality

material, equipment, and workmanship, in accordance with these Specifications and best accepted practice.

1.5 The Bidder is expected to base their Bid on materials and equipment complying

fully with the Specifications. In the event the Bid is based on material or equipment which does not conform, the Bidder will be held responsible for furnishing materials and equipment which conform fully at no change in the Bid Price.

1.6 Each Bidder in submitting their Bid, acknowledges their willingness to comply

with the terms of these Contract Documents. 2. LAWS AND REGULATIONS

2.1 Attention of all Bidders is called to Federal, State, and Municipal laws,

regulations, and ordinances in reference to labor, materials, equipment, and all other matters pertaining to the relationship between the City and Contractor.

3. BIDDER’S KNOWLEDGE

3.1 Bidders shall familiarize themselves with the Specifications and conditions which will affect the construction. It will be the responsibility of the Bidder to make a personal examination of the job site and the physical conditions which may affect their bidding and performance under the Contract.

4. BIDDER’S QUALIFICATIONS

4.1 Bidders will be required to satisfy the City as to their integrity, experience, equipment, personal, and financial ability to perform the work.

4.2 If successful Bidder is a non-Iowa Corporation, proof shall be submitted to the

City, prior to the execution of the Contract, of authorization by the Secretary of State to do business in Iowa.

5. METHOD OF BIDDING

5.1 Bidders shall submit Lump Sum Bid and Unit Price Bids, as required for the work covered by the Specifications. Prices shall cover complete work and include all costs incidental thereto, unless indicated otherwise.

5.2 In the event of discrepancies between Unit Prices and Unit Price Extensions listed

in the Bidder’s Proposal, Unit Prices shall govern and Unit Price extensions shall be corrected, as necessary, for agreement with Unit Prices.

5.3 Bids will be computed on the basis of the work shown on the Specifications. If

Unit Price Bids are called for, quantities are approximate and only for comparison of Bids. City retains right to change location, quantities, and combination of units as may be required during progress of construction. Compensation due Contractor will be computed on basis for final quantities of completed work.

5.4 Unit Prices for payment items included in the Specifications, but not listed in the

Proposal, will be negotiated, if needed.

5.5 The City will have sole discretion selecting the Contractor. All information submitted in the Request for Bid will be evaluated and considered. The City reserves the right to select the Contractor whom the City believes to be the most competitive, competent, reliable and who will perform best pursuant to the terms and conditions of this Request for Bid. Other factors that will be considered include but are not limited to past comparable experience, references, equipment, and any other relevant consideration that the City believes will affect the Contractor’s ability to perform in accordance with the terms and conditions in the Contract.

6. SUBMISSION OF BIDS

6.1 Bids shall be submitted on the Bid Form included herewith. The envelope shall bear the return address of the Bidder and shall be addressed as follows:

6.2 The Bid shall be signed by a legally authorized representative of the Bidder.

6.3 Bids received after the announced time and date of receipt, by mail or otherwise, will be returned unopened. Nothing herein is intended to exclude any responsible firm or in any way restrain or restrict competition. The City of West Des Moines reserves the right to award in part or in whole or to reject any or all bids.

6.4 If you desire not to quote on this Request for Bid, please forward your

acknowledgment of NO BID SUBMITTED to the above address. 7. WITHDRAWAL OF BIDS

7.1 Bids may be withdrawn any time prior to the scheduled closing time for receipt of Bids, but no Bid may be withdrawn for a period of thirty (30) calendar days thereafter.

8. EVALUATION OF BIDS

8.1 The City may consider such factors as Bid Price, responsibility of Bidder, and similar factors in determining which Bid it deems to be in its best interest.

8.2 The City may reject any or all Bids, waive informalities, or technicalities in any

Bid, and accept that Bid which it deems to be in its best interest. 9. TAXES

9.1 The City of West Des Moines will issue an Iowa Sales Tax Exemption Certificate to all contractors assigned to the project.

10. CONTRACT TERMINATION

10.1 Provisions of law, as contained in Chapter 573A of the Code of Iowa, shall apply to and be a part of this Contract, Chapter 573A provides for termination of Contracts for construction of public improvements when construction or work thereon is stopped because of a national emergency. The provisions of Chapter 573A shall be binding upon all parties hereto including Subcontractors and Sureties upon any Bond given or filed in connection therewith.

11. EXECUTION OF CONTRACT

11.1 The Successful Bidder shall, within ten (10) days of written Notice of Selection, enter into written Contract with the City on forms included with the Specifications for the performance of work awarded to the Successful Bidder.

12. QUESTIONS AND ADDENDA

12.1 If any person contemplating submitting a Bid for the proposed work, material, or equipment is in doubt as to the true meaning of any part of the Specifications or other Contract Documents, the Bidder may request an interpretation thereof. The person submitting the request will be responsible for its prompt delivery.

12.2 Questions concerning interpretation or intent of the Contract Documents should

12.3 Any oral interpretation given will be valid only if confirmed by written Addendum. Information obtained from an officer, agent, or employee of the City shall not affect the risks or obligations assumed by the Contractor or relieve the Contractor from fulfilling any of the conditions of the Contract.

12.4 The City reserves the right to revise or amend the Contract Documents, prior to

the date set for receipt of Bids. Such revisions and amendments, if any, will be announced by an Addendum or Addenda to the Contract Documents.

12.5 Copies of such addenda as may be issued will be furnished to all holders of

Specifications. 12.6 Bidders are required to acknowledge receipt of all Addenda by listing such

Addenda in Proposal.

13. APPROVAL OF MATERIALS AND EQUIPMENT

13.1 Approval of substitutions of any materials or equipment other than that specified shall be obtained in writing from the City. Otherwise, it will be assumed the Contractor will furnish materials or equipment specified.

ON-CALL FIBER OPTIC CONDUIT NETWORK DROP INSTALLATION SERVICES CONTRACT PROVISIONS

CITY OF WEST DES MOINES

ENGINEERING SERVICES 1. STATEMENT OF PROJECT

The intent and purpose of this Request for Bid is to establish a contract and retain a qualified contractor to provide fiber optic conduit network drop installation services for the City of West Des Moines on an on-call basis. The City reserves the right to establish a primary and secondary contract as a contingency. If the primary Contractor cannot perform the needed services in an agreeable time frame, the secondary Contractor may be called upon. If neither of these Contractors can perform the needed services in an agreeable time frame, the repair or maintenance may be awarded to other Contractors for bids on a case-by-case basis. Contractor is not required to provide bid security or performance bond as part of these contractual services. However, the Contractor shall provide proof of insurance as specified hereinafter.

2. SCOPE OF WORK 2.1 The Contractor shall furnish all labor, equipment, and materials required for the

requested drop conduit installations of the fiber optic conduit network on an "as needed" basis.

3. PERFORMANCE REQUIREMENTS

3.1 The Contractor shall provide the City with the name, address, email address(es) and telephone number(s) for at least two designated contact personnel responsible for insuring response to the City's request for contract services.

3.2 The City’s contact person shall be notified by e-mail of any work performed by

the Contractor.

3.3 The Contractor shall respond within the timeframe shown in Table 1 after notification from City Staff. Response shall include, but not be limited to: scope of work, assignment of responsibilities, traffic control, approximation of work area, general construction sequence, estimated construction timeline, anticipated equipment needs, anticipated materials needs, pavement removals, utility locates, notices to affected businesses or residents, right-of-entry permissions needed, storage areas, etc.

3.4 The Contractor shall maintain all drop conduit installation records including

time/date of installation, address of repair and any other pertinent information. A copy of these records shall be provided at the City’s request.

3.5 The Contractor and their designated personnel shall respond to service requests given by the City in a positive, courteous, and timely manner. The City reserves the right to reject any piece of equipment or personnel from continued or further engagement of services due to incompetence or insubordination or inability of equipment to function properly for the requested services.

3.6 The Contractor shall be responsible for any and all damage to private and public property (including public and private utilities), not identified or documented in advance, while performing the drop installation. Any damages caused by the Contractor shall be repaired to new or better condition as soon as practicable. The City has the option to withhold payment until proper repair or cleanup has been made.

3.7 Any work performed within the public right-of-way shall be performed in

compliance with all applicable regulations. If work needs to occur within, or equipment needs to be located in, a travelled lane the Contractor shall follow City of West Des Moines Public Services Department procedures for a lane closure. All traffic control procedures shall comply with the latest edition of the Manual on Uniform Traffic Control Devices for Streets and Highways. All temporary traffic control provisions shall be furnished, erected, maintained, and removed by the Contractor. Long term parking of private vehicles on right-of-way and/or closed streets is not allowed.

3.8 All waste material shall be hauled to a construction debris recycling center or salvage yard of the Contractor’s choice with the City’s approval or an approved hazardous waste disposal site if required. Disposal costs will be included in the Contract Cost and not paid by the City as a separate item.

3.9 The City will provide the following at no cost to the Contractor:

3.9.1 Maps, blueprints, or drawings for conduit and vaults.

3.9.2 Maps, blueprints or drawings for fiber optic, traffic signal and any other

City-owned utilities adjacent to construction area as requested.

3.9.3 Property owner coordination.

3.9.4 As-built data collection.

3.9.5 Legal and physical access to conduit and vaults included in the project.

3.9.6 Water and access to designated fire hydrants.

3.9.7 A liaison between the City and the Contractor during the service request that will also familiarize the Contractor with the location of City infrastructure.

3.9.8 Quality assurance testing for compaction and concrete/asphalt.

3.9.9 The City will provide inspection and materials testing as needed

3.10 Installations Performed by the Contractor.

3.10.1 The Contractor shall furnish all labor, equipment, materials, tools, rentals, sub-contractual services, insurances, and other incidentals necessary for installation of drop conduits in accordance with the specifications.

3.10.2 All services will be performed by experienced, trained workers. It is the responsibility of the Contractor to be compliant with all applicable OSHA, DOT, and other pertinent regulations.

3.10.3 All work is to be done in strict compliance with instructions issued by the City of West Des Moines.

3.10.4 All work is to be done in strict compliance with the City of West Des Moines Standard Construction Specifications requiring the furnishing of highest quality equipment, material, and workmanship in accordance with best accepted practice.

3.10.5 The Contractor shall abide by all Federal, State, and Municipal laws, regulations, and ordinances in reference to labor, materials, equipment, and all other matters pertaining to the relationship between City and Contractor.

3.10.6 The Contractor shall be responsible for documenting existing site conditions prior to commencing any work.

3.10.7 The Contractor shall be in frequent contact with City Staff regarding schedule, action plan, and needs, including any revisions thereto.

3.10.8 Obtaining utility locates prior to work.

3.10.9 Protection of the work. Protection of all property from injury or loss resulting from operations.

3.10.10Replace or repair objects sustaining any such damage, injury, or loss to satisfaction of City.

3.10.11Cooperate with City and representative utilities in locating underground utility lines and structures. Incorrect, inaccurate, or inadequate information concerning location of utilities or structures shall not relieve Contractor of responsibility for damage thereto caused by operations.

3.10.12Keeping cleanup current. 4. CONTRACTOR’S PERSONNEL

3.4 Contractor's employees appearing to be under the influence of alcohol or drugs

shall not be permitted to perform required work.

3.5 No loud or boisterous conduct will be permitted.

3.6 Work area where service is being conducted will be neat and clean and in an orderly fashion when the work is complete.

3.7 All contract personnel will have a governmental issued identification. Accepted form shall be a valid state issued driver’s license, or State issued Identification.

5. LENGTH OF AGREEMENT

5.1 The initial term of this contract shall be effective for the period of one year from the executed contract. The City reserves the right at its sole option to renew this contract on an annual basis per the same terms and conditions as set forth in the resulting contract. Any renewal contract agreement shall be signed by the Contractor prior to the expiration of the initial term and any subsequent term. Contract award and renewals shall be limited to a total of five (5) years from the original executed contract date. This contract shall have a 3% cost of services increase for every year the contract is in effect for the length of the contract.

5.2 This Agreement may be terminated for any cause by the City during the Agreement period subject to a thirty (30) day written notice being delivered by registered mail to the Contractor at the address referenced on the bid form.

6. PAYMENT

6.1 Invoices must contain the contract number, job location, and detail on services

provided, including parts. All work being billed shall be referenced on invoices according to the tasks listed on the attached bid sheet. Invoices shall be sent to:

7. INSURANCE

7.1 Contractor, performing as an independent Contractor hereunder, shall be fully

responsible for obtaining and maintaining, through the term of the Agreement and two years beyond, insurance with terms and limits of coverage equal to or in excess of those set forth in the specifications governing the services provided, but in no event, are such terms and limits to be less than those set forth below. The limits of liability under insurance policies required by this Agreement shall in no-way limit the Contractor’s actual liability.

7.2 Before providing any services, the Contractor shall furnish insurance certificates to the City Clerk, showing adequate insurance to be in force. Any subcontractors utilized shall be subject to the same insurance requirements below. All insurance certificates shall indicate that the City shall receive a thirty (30) day notice prior to any cancellation or termination of policy.

The City shall be named as an additional insured on a primary and non-contributory basis; include waiver of subrogation in favor of the City. The limit may be satisfied by providing a primary policy or in combination with an excess liability policy. Bodily Injury & Property Damage $1,000,000 (Each Accident)

Pollution Liability Insurance This includes coverage for potential risk to the environment or losses caused by pollution conditions that may arise from the operations of the Contractor. The City shall be named as an additional insured on a primary and non-contributory basis; include waiver of subrogation in favor of the City. Each Occurrence $1,000,000 Policy Aggregate Limit $1,000,000 Umbrella or Excess Liability Contractor shall also maintain umbrella liability insurance on an occurrence basis in excess of the underlying liability insurance described above, except for pollution liability insurance, which is at least broad as all underlying policies; including, but not limited to Additional Insured, Waiver of Subrogation in favor of the City, and Primary & Non-Contributing. Each Occurrence $5,000,000 Aggregate Limit $5,000,000

8. HOLD HARMLESS AND INDEMNIFICATION 8.1 Contractor agrees to indemnify, hold harmless, and defend the City, its officials,

employees, agents, contractors, and assigns, from any and all claims, demands, causes of action, liability, loss, damage, or injury, both to person and property, arising out of, related to, or connected to this Agreement and the services provided under this Agreement. This indemnification applies to and includes, but is not limited to, the payment of all penalties, fines, judgements, awards, decrees, attorney’s fees, expert witness fees, investigation fees, settlements, related costs or expenses, interest, and any reimbursements incurred by or assessed to the City, its officials, employees, agents, contractors and assigns. This Hold Harmless and Indemnification Agreement is effective upon execution and shall be of indefinite duration and shall survive the termination of this Agreement. The person signing this document also represents that he/she possesses the requisite authority to enter into this Agreement on behalf of the Contractor.

9. FURTHER INFORMATION 9.1 The Contractor certifies that all their employees who may perform safety sensitive

functions for the City are included in a substance abuse program that meets the requirements of Federal Department of Transportation Drug and Alcohol Testing regulations, (Code of Federal Regulations, 49 CFR., Part 382 and Part 40).

9.2 The bidder certifies that, in accordance with the “Hazard Communication Rule”, 29 CFR 1910.120 (the “Right-to-Know” Law) and the State of Iowa “Hazardous Chemical Risk Right-to-Know” rule, employees exposed to materials on the worksite will be trained for the materials in use by the successful Contractor as part of the contract. Material Safety Data Sheets (MSDS) for City materials will be supplied to the successful Contractor upon request.

9.3 Contractor will comply with all applicable provisions of the Americans with

Disabilities Act and applicable Federal regulations under the Act. 9.4 “We hereby certify that these goods were produced in Compliance with all

applicable requirements of Sections 6,7, and 12 of the Fair Labor Standards Act, as amended, and of regulations and orders of the United States Department of Labor issued under Section 14 thereof.”

9.5 The City of West Des Moines does not discriminate against any person on the

basis of age, color, creed, disability, gender identity, national origin, race, religion, sex, sexual orientation, or veteran status. Contractor will not discriminate against any employees or applicants for employment, or passengers served through this contract on the basis of age, color, creed, disability, gender identity, national origin, race, religion, sex, sexual orientation, or veteran status, except where age and sex are essential bona fide occupational requirements, or where disability is a bona fide occupational disqualification.

10. ADDITIONAL TERMS

10.1 Neither the Contractor nor City shall assign its rights or obligations under this Agreement without the prior written consent of the other party.

10.2 This Agreement, including any attached schedules and exhibits, constitutes the entire

agreement and understanding between the parties hereto and is the final expression of their agreement and no evidence of oral or other written promises shall be binding. This Agreement may not be supplemented, amended, modified, or otherwise altered except by written instrument executed by both of the parties hereto and no course of dealing or trade usage between the parties shall be effective to supplement, amend, modify, or alter this Agreement.

10.3 The failure to enforce or to require the performance at any time of any of the provisions

of this Agreement herein shall in no way be construed to be a waiver of such provisions, and shall not affect either the validity of this Agreement or any part hereof or the right of either party thereafter to enforce each and every provision in accordance with the terms of this Agreement.

SPECIFICATIONS ON-CALL FIBER OPTIC CONDUIT DROP INSTALLATION SERVICES

CITY OF WEST DES MOINES

ENGINEERING SERVICES

1. GENERAL

1.1 This part of the specifications includes the requirements for installation of Contractor-provided materials and equipment necessary to complete, in place and operational, a fiber optic drop conduit system.

1.2 Provide any items, materials or equipment not specifically addressed in the contract documents but required to provide a complete and functional installation. The level of quality shall be consistent with other specified items. All miscellaneous electrical materials and equipment shall be listed for its specific application.

1.3 The project involves close interaction with the public. The Contractor is expected to conduct all communications and operations in a professional and courteous manner:

1.4 The City will provide coordination with the property owner. Contractor shall provide the owner with the proposed date of the installation of drop conduit.

1.5 If property owner requests an on-site meeting, Contractor shall meet with property owner and verify path. Contractor shall make any changes within reason if requested. At that time the contractor shall inform property owner of proposed installation date.

1.5.1 If there has been no response from property owner within the 72-hour

review deadline. The contractor shall schedule the drop conduit installation within 48 hours after this period.

1.5.2 Contractor shall complete all restoration prior to leaving a work site.

1.5.3 If restoration cannot be completed due to weather, or preferred seeding timing, Contractor shall notify property owner of the updated estimated completion dates.

1.6 Contractor shall provide a daily status report of all assigned work orders. Report

will be tabulated in a format agreeable to City.

1.6.1 City has an on-line system designed to record work order status changes and pull tape sequential records which can be provided at no cost to the Contractor to satisfy this requirement.

2. CONSTRUCTION SURVEY

2.1 The Contractor shall flag designated drop conduit alignment based on final utility locates and information provided by property owner of installed underground facilities: 2.1.1 Contractor shall locate the nearest property pin to ensure the proposed

installation does not encroach on an adjacent private property.

2.1.2 Contractor shall ensure the running line is no closer than one (1) foot from the property line to avoid encroachments on adjacent properties.

2.1.3 Conduit alignment direction changes of more than 30 degrees and transitions from plowing to bore or bore to plowing.

2.1.4 At all locations requiring changes in the specified minimal depths to avoid existing or future facilities

2.1.5 A minimum of 20 feet apart along bored conduit

2.1.6 At other locations as noted within the contract documents

2.2 All flags marking bore locations shall be labeled with required minimum conduit depth below existing grade.

2.3 Contractor shall make best effort to position the conduit riser no closer than four (4) feet from the front edge of the building and as close to the power meter location on the building where possible, but no closer than twelve (12) inches or further than forty-eight (48) inches from the power meter. There are cases where the drop conduit was designed to terminate on an opposite side of the structure. The drop conduit must terminate at the designed vault location.

3. TRAFFIC CONTROL

3.1 Erect and maintain barricades and lights and/or provide watchmen for protection

and warning of pedestrians and vehicles. All barricades, lights and/or watchmen at expense of Contractor. Maintenance shall include sign washing as required by the City.

3.2 Location and arrangement: Conform to Ordinances, Laws and MUTCD; meet approval of City.

3.3 Responsibility: Maintain, control, and protect traffic within construction area including side street crossings, temporary run-arounds, use of one or more lanes and related operations and detours outside construction area.

3.4 Closing or partially closing streets: Close or restrict traffic in streets only with permission of City; provide for maximum safety and convenience of public; open street when work is complete or as directed by City. Access to private property must be maintained during construction.

3.5 Emergency traffic: Provide continuous access for police, fire, and emergency vehicles.

3.6 Take every precaution to protect pedestrian and vehicle traffic.

3.7 Lane closure notices must be submitted to West Des Moines Public Services for approval a minimum of 48 hours in advance of closure.

4. WATER USAGE

4.1 West Des Moines Water Works will provide the Contractor a nominal amount of

water on the Project free of charge for watering sod only, with the following requirements:

4.1.1 The water is to be metered and the Contractor must obtain the meter from

the West Des Moines Water Works and a refundable meter deposit is required.

4.1.2 The Contractor will abide by rules and regulations of bulk water usage as prepared by West Des Moines Water Works.

4.1.3 The Contractor will only use hydrants that have been previously approved by the West Des Moines Water Works for bulk water usage.

4.1.4 The Contractor shall not use water from private residential or business hose bibs, without the written consent of the private owner. Any such agreements must be filed with the Engineering prior to water usage.

4.1.5 The water provided on the Project free of charge will consist of 870 gallons of water per square (100 square feet) of sod installed on the Project. Additional water usage shall be the sole responsibility of the Contractor. Additional water shall be paid for at the current West Des Moines Water Works rate.

4.1.6 The Contractor must notify West Des Moines Water Works prior to using the assigned water meter on any other project.

5. EQUIPMENT AND MATERIALS 5.1 Equipment and materials shall be of new stock. New materials and equipment shall be

the product of reputable manufacturers and shall meet City approval.

5.2 Six (6) copies of catalog cuts and manufacturer's specifications shall be furnished for all standard "off-the-shelf" items.

5.3 City review of shop drawings and catalog cuts shall not relieve the Contractor of any responsibility under the Contract documents.

5.4 All electrical equipment shall conform to the standards of the National Electrical

Manufacturers Association (NEMA), and all material and work shall conform to the requirements of the National Electrical Code (NEC), the Standards of the Telecommunications Industry Association (TIA), the Standards of the Electronic Industries Alliance (EIA), the Society of Cable Telecommunications Engineers (SCTE), Underwriters Laboratories and local ordinances. Wherever reference is made in these specifications or in the standard provisions to the code, the safety orders, the general order, or the standards mentioned above, the reference shall be construed to mean the code, order, or standard that is in effect at the date of advertising of these Specifications.

6. CONDUIT MATERIALS

6.1 Provide certification from the manufacturers of all conduit supplied by the Contractor stating said material complies with these specifications.

6.2 The conduit shall be constructed of High Density Polyethylene (HDPE).

6.3 High Density Polyethylene (HDPE) conduit shall be smooth wall and SLATE in color.

6.4 Comply with ASTM F 2160 (conduit) and ASTM D 3350 (HDPE material), minimum SDR 13.5.

6.5 Provide sequential foot markings printed on HDPE.

6.6 All Conduit shall be provided as a continuous reel or straight pieces to minimize splicing.

6.7 Conduit shall contain a pull tape which contains sequential numbers at 1foot increments throughout its length.

6.7.1 Pull tape shall be 3/8” in width and contain sequential numbers at 1foot

increments throughout its length.

6.7.2 Pull tape shall have a tensile strength of 600 ft-lbs. 6.8 Conduits shall be 1.0” diameter SLATE in color without stripe.

6.9 All conduit connections must be watertight and able to withstand a minimum PSI

threshold of 125psi to allow for the blowing fiber through the conduit.

6.10 Fittings shall be designed for use in direct bury applications, adhesive type connections shall not be allowed.

6.11 Duct plug must contain an eyelet to contain pull tape.

7. CONDUIT INSTALLATION

7.1 Installation of the conduit shall be in close conformity with the lines, grades and dimensions shown in the contract documents.

7.2 Conduit shall not be installed closer than one (1) foot of the property line to avoid encroachment on adjacent properties.

7.3 The City must approve any deviations from the agreed upon location of conduit

placement. Contractor shall flag final proposed conduit alignment prior to construction as it relates to existing utilities or other possible conflicts in the field and request any alignment or vertical adjustments that may be required. If the City finds the installed conduit has resulted in an unsatisfactory alignment, the installation shall be considered unacceptable work and shall be removed and replaced at the expense of the Contractor.

7.4 It is the design intent to plow, or use other minimally invasive means as approved by City, HDPE conduit from a conduit stub which has been previously installed by others on the property side of the sidewalk to building structure where it will sweep up 24” minimum above grade. The stub will be within the right-of-way if a sidewalk is not present. No more than one splice at or near the right-of-way line used to connect to the pre-installed stub from vault shall be permitted.

7.5 Conduit shall be installed from the stub designated for the address as shown on the plans to the nearest side of the house.

7.6 In event there are unforeseen obstruction to the planned route, the contractor may propose an alternate termination location on the building, acceptable to property owner and to be approved by the City.

7.7 Change in direction of conduit shall be accomplished by bending such that the conduit will not be damaged or deformed, adhering to manufacturers’ minimum bend radius specifications.

7.8 Contractor shall expose the previously installed drop stub and tracer wire.

Excavate the area to a depth necessary to cut off the riser portion of the stub to allow connection to the installed drop conduit. 7.8.1 Existing pull tape in the stub shall be removed and a new pull tape placed

from the duct plug in the handhole to the termination plug at the building. 7.8.2 Sequentials from pull tape shall be recorded as part of the drop conduit

installation as-built. The total length of the drop conduit shall be represented by the difference between the sequential numbers on the vault side plug and the building side termination.

. 7.9 The minimum bending radius of HDPE conduit shall be the larger of twenty (20)

times the outside diameter or the HDPE manufacturer’s recommendations for minimum bending radius.

7.10 Driveway crossings shall use horizontal directional drilling (HDD) methods to place the conduit without cutting, removing, or disturbing existing ground/pavement. Conduit within fifteen (15) feet of a vault may be open trenched to facilitate entry into the structure. Pits for boring shall not be closer than two (2) feet to the back of curb unless otherwise directed by the City. 7.10.1 All water for boring operations shall be supplied by the Contractor and

follow the water usage specifications detailed in this document.

7.10.2 Slurry disposal shall be the responsibility of the contractor and shall be by a means and method acceptable to the City. Slurry disposal shall meet the Iowa DNR and USEPA requirements. Do not dispose of slurry in storm or sanitary sewers.

7.11 Concrete, asphalt, or other solid surface obstructions surrounding buildings, such

as driveway, patios and walkways which abut the structure will require removal and replacement of surface material to facilitate installation of the conduit and the building structure riser. Work shall conform to City and SUDAS specifications. All removal and replacements must be approved by the City in advance of performing the work.

7.12 Pavement crossings up to 6 feet in width may use pneumatic or other mechanical means to complete the crossing.

7.13 All crossings shall be at a depth of 12 inches and below the base of any bedding material whichever is greater.

7.14 Conduit buried using open chute plowing shall be placed a minimum of 12 inches deep unless otherwise directed by the City or identified in the plans. Open trench methods of placing conduit will be permitted where tight constraints exist and as approved by the City. The Contractor shall maintain depth control on the placement of the conduit. The backfill material in open trenches and excavations for structures shall be deposited in layers not to exceed 6 inches in depth and each layer shall be thoroughly compacted before the next layer is placed. Backfill material shall be free of cinders, broken concrete, or other hard or abrasive materials. All surplus material shall be removed from the property and public right-of-way.

7.15 Backfill 7.15.1 Backfill with suitable excavated material or borrow materials. Use no

large stones, large clods, organic matter, rubbish, or frozen material.

7.15.2 If excavated materials have excessive soil moisture and cannot be compacted to specified density, dry material by discing or mixing with dry soils or remove and replace with suitable borrow materials.

7.15.3 If sufficient suitable excavated material is not available, remove and dispose of unsuitable excavated material. Furnish suitable borrow materials to complete trench backfill.

7.15.4 Provide temporary support for existing water, gas, telephone, power, and other utility services that cross the trench until backfilling of the trench has been completed; do not use support methods which transfer weight or forces to other utilities.

7.15.5 All cables, conduit, and pipe exposed during construction will be secured in such a manner that no deflection or sagging occurs during or after construction. All backfill shall be natural earth materials excluding rocks, foreign materials, or other abrasive materials that may be injurious to the cables.

7.15.6 Compact backfill under existing utility crossing to ninety-five per cent (95%) maximum density; moisture content not less than two percent (2%) below optimum or more than three percent (3%) above optimum moisture content.

7.15.7 Whenever excavation is made across parkways, driveways or sodded areas, the sod, topsoil, crushed stone, or gravel shall be replaced or restored to its original condition and the whole area involved shall be left in a neat and presentable condition. Use of tarping in areas of open evacuation shall be used to minimize additional restoral of excavated areas Concrete sidewalks, pavements, base courses, and bituminous surfaces shall be replaced with new materials within 48 hours of removal of original material. Surface restoration shall comply with Section 16 of the Technical Specifications.

7.16 Installation Clearances.

7.16.1 Ground should not be disturbed within an area of at least three (3) feet

distance from any City Survey Benchmark. Do not disturb City Survey Benchmarks.

7.16.2 Maintain the minimum depth throughout the length of all conduit installations.

7.16.3 Depth of all bores shall be a minimum of 12 inches unless otherwise specified in the plans.

7.16.4 Maintain 1.5 feet of horizontal clearance (minimum) from existing utility lines and privately owned facilities. Contractor to notify City when 1.5 feet of horizontal clearance is unable to be achieved.

7.17 Protect all open excavations. All open holes shall be backfilled within 48-hours.

7.18 The Contractor shall have a licensed plumber and electrician on call 24/7/365 for

the duration of the project to quickly respond to any damage to water mains or water service lines. 7.18.1 Contractor’s agreement with the licensed service providers shall require a

2-hour response time.

7.18.2 Contractor shall provide to City a list of contracted firms, their contact information, and qualifications.

7.19 Contractor is required to follow all Federal, State, and local requirements,

including reporting related to all public utility and privately owned infrastructure damage or service disruptions resulting from the construction of the conduit network. 7.19.1 All damage or service disruptions are to be reported to the City

immediately and site secured.

7.19.2 During an installation should damage occur to water, gas or electric lines, the Contractor must take immediate measures to secure the site to ensure the safety of on-site crew members and safeguard general public and vehicular traffic.

7.20 Drop Conduit shall terminate at a riser secured to the side of the structure a minimum of 24 inches above grade. 7.20.1 Conduit is to be fastened to the building at intervals no more than 10

inches apart starting at grade. Suitable fastener devices are to be used based on the material composition of the building attachment location.

7.20.2 Conduit shall be capped with a plug device which is designed to contain the pull tape.

7.20.3 A copper ground strap shall be installed within 6 inches from the end of the duct. This device is used as the termination point of the tracer wire. Device shall be a Belden SC13B-1 6in Copper Ground Strap or equivalent.

7.20.4 A weatherproof, outdoor rated metal tag is to be fastened to the ground strap. The tag will state conduit ownership as “WDM Conduit”.

8. ACCEPTANCE OF CONDUIT SYSTEM

8.1 The contractor shall notify the City 48 hours in advance of the date when

installations will be ready for acceptance. A representative of the Owner shall be present for all conduit proofing and electrical continuity testing of the tracer wire system. All acceptance testing shall be accomplished to allow installation of cable (by others) in conjunction with the remainder of the construction.

8.2 All testing shall be conducted in conformance with manufacturer’s

recommendations. Prior to commencing testing, the contractor shall prepare a written plan for review by the City listing the equipment planned to be used during testing and the sequence of testing locations. Upon authorization of the City, jet (blow) or pull a mandrel through each duct taking care not to scuff or damage the conduit. The mandrel shall measure at least 90% of the cross-sectional area of the duct being tested. Any location where the mandrel fails to pass through the conduit shall be repaired or replaced. The repaired or replaced section(s) shall be re-tested by jetting (blowing) or pulling a mandrel through the entire length of conduit from structure to structure. All additional work required to meet the acceptance testing, including but not limited to excavation, conduit repair or replacement, surface restoration, re-testing, etc. shall be the contractor’s responsibility and will not be paid as extra work.

8.2.1 Mandrel Sizing. Mandrel shall have the outside diameter (O.D.) a

minimum of 90 percent of the inside diameter (I.D.) of the duct. The inside diameter of duct, for purpose of determining outside diameter of BB shall be the nominal inside diameter of the ASTM F2160, SDR-13.5, HDPE conduit standards. Statistical or other “tolerance packages” shall not be considered in mandrel sizing.

8.2.2 Mandrel Design. Mandrel shall be a flexible mandrel comprised of neoprene or similar material.

8.3 As-built documentation showing the horizontal and vertical location of the

installed conduit and structures shall be completed and provided to City after the acceptance of conduit system testing have been completed and all repairs have been made.

8.4 Test results, including any re-testing done as a part of conduit proofing and

electrical continuity testing shall be documented on an appropriate report form and submitted to the City for inclusion in project “as-builts”.

8.5 The contractor shall proof test, in the presence of the City, all installed 1” conduit

via mandrel a minimum of 90% of the inner diameter of the conduit being tested.

9. TRACER WIRE MATERIAL

9.1 Tracer wire shall be required along the full length of the conduit pathway. Tracer wire shall be installed external to the 1inch drop conduit and secured to the conduit at 10-foot intervals.

9.2 Tracer wire for all installations shall be hard drawn copper-clad steel, utilizing an AISI 1055 high carbon steel core with a minimum break load of 1,150 lbs. for use in horizontal directional drilling (HDD) operations. The tracer wire shall be insulated with an orange colored 45 mil high-density, high molecular weight polyethylene (HDPE) insulation pursuant to ASTM D1248.

9.3 Tracer wire must be at 21% conductivity for locate purposes and locatable at 512 Hz (low frequency).

9.4 Tracer wire splice kits shall be limited to one splice per run and it is to be at the point of interconnection to the tracer wire previously placed with the drop conduit stub. Tracer wire splice kits shall be designed to be waterproof and rated for direct bury and submersible applications. Splice kits shall be incidental to the Tracer Wire bid item.

10. TRACER WIRE INSTALLATION

10.1 For tracer wire installed on the outside of conduit as part of the directional drilling or open trenching method of buried conduit installation, install tracer wires continuously along the lower quadrant of conduit.

10.2 Tracer wire is to be spliced to the drop stub’s pre- installed tracer.

10.3 Tracer wire will be zip tied to drop conduit and contained with any conduit wall fasteners where it rises up the side of the building. 10.3.1 Zip ties to support tracer wire at the building riser shall be UV rated and

designed for outdoor use.

10.4 Zip ties are placed every 6 inches along the exposed conduit riser where it cannot

be contained by any conduit wall fasteners and terminate at a termination lug on the cooper ground strap (Belden SC13B-1 6in Copper Ground Strap or equivalent)

11. TRACER WIRE ACCEPTANCE OF SYSTEM

11.1 The contractor shall notify the City as installations are completed and ready for acceptance testing. A representative of the City shall be present for all conduit proofing and electrical continuity testing of the tracer wire system. Prior to commencing testing, the contractor shall prepare a written plan for review by the City listing the equipment planned to be used during testing and the sequence of testing locations. All acceptance testing shall be accomplished to allow installation of cable (by others) in conjunction with the remainder of the construction. 11.1.1 Contractor shall provide the City with 48-hour notice of Contractor’s

intent to conduct tracer wire acceptance testing.

11.2 Upon authorization of the City, test the electrical continuity of the tracer wire system and to demonstrate the locate system is working properly throughout the entire locate system up to the vault located in the public right-of-way. All tracer wire testing shall be performed using testing equipment that will verify that the tracer wire, and subsequent tracer wire route, are locatable at a frequency of 512 Hz (low frequency). Any failure or malfunction of the tracer wire system during the electrical continuity test shall be corrected at the Contractor's expense and the system retested after corrective action has taken place.

12. AS-BUILT DOCUMENTATION

12.1 The City shall supply as-built documentation showing the horizontal and vertical locations of all conduit and all changes in conduit paths greater than 30 degrees constructed with this project.

12.2 Begin and end sequential footages shall be recorded as identified on the pull tape for all conduits from the serving handhole to the building structure riser. Contractor shall provide redlined conduit routing on all work orders.

13. WITNESS MONUMENTS

13.1 Contractor to ensure that the legend “WDM-CONDUIT shall be displayed using weatherproof metal tags fastened at the top of the conduit at the building side of all drop conduit installations.

14. REPLACING DAMAGED IMPROVEMENTS

14.1 Improvements such as sidewalks, curbs, driveways, roadway pavement, irrigation systems, electric fences, or any other improvements removed, broken, or damaged by the Contractor shall be replaced or reconstructed with the same kind of materials found on the work or with materials of equal quality at the Contractor’s expense. The new work shall meet the requirements of City standards and specifications. If additional pavement removal is required other than a standard box out, full panel removal and replacement will be required. No partial panel removal will be allowed. This cost shall be incidental to the cost of applicable related bid items. All removal and replacement limits shall be approved by the City.

14.2 Surface restoration, including sidewalk, driveway, and street surface replacement shall be completed in accordance with the City of West Des Moines Standard Construction Specifications for Subdivisions.

14.3 The Contractor shall preserve in place existing monumentation including property pins. The contractor shall be responsible for the repair or replacement of all removed, broken, or damaged monumentation in accordance with the Iowa code.

15. RESTORATION

15.1 Temporary or permanent restoration of impacted areas, as described herein, shall be required upon the cessation of all construction activities planned or otherwise, within 14 calendar days.

15.4 Concrete sidewalks, pavements, base courses, and bituminous surfaces shall be replaced with new materials within 48 hours of removal of original material. 15.4.1 Repairs shall be completed in accordance with the 2021 edition of the

“Iowa Statewide Urban Design and Specification (SUDAS)” section 7010, 7030, and 7040 as well as the West Des Moines Standard Drawings No. 7.2, 7.4, 7.5, 7.13, and 7.14, shown in Section 16 of Technical Specifications for reference

15.5 Topsoil, crushed stone, and gravel shall be replaced immediately and shall be free

of all construction debris.

15.6 Restoration areas requiring seed or sod shall follow procedures as required and in accordance with the 2021 edition of “Iowa Statewide Urban Design and Specification (SUDAS)” Sections 9010 and 9020. The Contractor shall adhere to all SUDAS requirements including preparation, fertilizing, installation, maintenance, clean up, and acceptance.

15.7 Restoration areas requiring seed or sod shall adhere to the following additional requirements as mandated by the City of West Des Moines:

15.7.1 Permanent seeding and sodding shall be permitted between April 15th

through June 15th and between August 10th through October 15th only.

15.7.2 Restoration for all disturbed areas not within the permitted seeding and sodding times shall be considered temporary and shall utilize hydromulch for stabilization.

15.7.3 Area preparation shall require a minimum of three inches of new topsoil

for all disturbed areas.

15.7.4 Restoration for any single area exceeding 100 square feet shall require sod.

15.7.5 Disturbed areas that require sod shall be prepared with straight edge lines

to abut new sod; new sod shall be installed flush to undisturbed areas.

15.7.6 Contractor shall be responsible for watering of new sod in accordance with supplier recommendations and the following:

15.7.7 New sod shall be watered within one hour of placement.

14.7.7.1 Sod shall be watered a minimum of twice daily for a duration of

twenty minutes or as required to adequately maintain moisture levels. Daily watering shall be maintained for a minimum of 14 days or until such time as required for establishment of proper rooting.

14.7.7.2 Once rooting is established the Contractor shall be responsible for daily maintenance and watering for a minimum of 10 calendar days.

15.8 Restoration for areas not exceeding 100 square feet shall require seeding; all

seeding shall utilize United Seed Inc. Rescue + Tall Fescue/Bluegrass Mix.

15.9 Contractor shall be responsible for maintenance and watering as recommended by seed supplier.

15.10 Hydromulch shall be used in all cases as permanent and temporary stabilization.

15.11 Seeding and mulching shall be considered as separate operations.

15.12 All operations, construction, and material as required for restoration as outlined herein shall be considered incidental to project and shall not be paid for as separate bid items.

16. METHOD OF MEASUREMENT AND BASIS OF PAYMENT

16.1 MOBILIZATION AND ACCEPTANCE TESTING: Paid on a unit price basis for

each drop conduit installation. The unit price shall be full compensation for all scheduling, property owner coordination and meetings, subsurface locate surveys, internal administration, reporting, acceptance testing of installed conduit and tracer wire, independent of the construction method used. Full payment will be made upon successful acceptance of the installation of the conduit and tracer network.

16.2 PLOWED INSTALLATION OF CONDUIT, FURNISH & INSTALL, 1” SLATE & TRACER WIRE: Paid on a linear foot price basis for all footage of the installation. Measurement for all conduits shall be per linear foot based on the pull tape recorded length provided by the contractor. The unit price shall be full compensation for furnishing and installing the 1” SLATE conduit, plowing, trenching, crossing of sidewalks and other hard surface of up to 6 feet in width, excavation for stub connections, or direct entry to vault, for bore pits, trenching/excavation/potholing at all locations where new conduit crosses an existing public or private utility (including electric lines, sewers, or gas mains irrigation, fences, and all other private improvements) conduit mounting on buildings, backfilling, surface restoration, excavation protection, removals, protection of existing utilities and incidental items necessary to meet the requirements of the contract documents. Full payment will be made upon successful acceptance of the installation of the conduit and tracer network.

16.3 BORED INSTALLATION OF CONDUIT, FURNISH & INSTALL, 1” SLATE & TRACER WIRE: Paid on a linear foot basis of the total bored length of the installation. The linear footage price shall be full compensation for furnishing and installing the 1” SLATE conduit, plowing, trenching, crossing of sidewalks, driveways and other hard surfaces, excavation for stub connections, or direct entry to vault, for bore pits, trenching/excavation/potholing at all locations where new conduit crosses an existing public or private utility (including electric lines, sewers, or gas mains irrigation, fences, and all other private improvements) conduit mounting on buildings, backfilling, surface restoration, excavation protection, removals, protection of existing utilities and incidental items necessary to meet the requirements of the contract documents. Full payment will be made upon successful acceptance of the installation of the conduit and tracer network.

16.4 DRIVEWAY BORE: Paid on a unit basis for each bored driveway greater than 6’ long for a plowed installation. The unit price shall be full compensation, but not limited to, for furnishing and installing the 1” SLATE conduit and tracer wire, below the surface of a driveway to parking area, including backfilling, surface restoration, excavation protection, removals, protection of existing utilities and incidental items necessary to meet the requirements of the contract documents. Full payment will be made upon successful acceptance of the installation of the conduit and tracer network.

16.5 HARD SURFACE REMOVAL AND REPLACEMENT: Paid on a square yard basis. This shall be full compensation for, but not limited to, the removal and replacement of all hard surfaces, not limited to concrete, brick, block, or bituminous materials and subbase materials, cost of all labor, removal of spoils and permits required from the City of said work. Full payment will be made upon successful acceptance of the installation of the conduit and tracer network and all as-built and GPS data.

16.6 VAULT SITE EXCAVATION AND RESTORATION: Paid based on each location where the drop conduit stub does not exist and the drop conduit and tracer is terminated at the vault It shall be full compensation for, but is not limited to, equipment, materials and labor for fill, seed, or sod, hydromulch required to meet the restoration requirements in the specification document. Restoration must conform to specifications detailed in section 16 of the Technical Specifications. Full payment will be made upon successful acceptance of the installation of the conduit and tracer network and all as-built and GPS data.
