1.2 Responsibilities of the RCCP The Respiratory Coordinated Care Program (RCCP) is a specialised, multidisciplinary community based program designed to manage people within the St George and Sutherland LGA’s with chronic respiratory disease. The service aims to provide specialist nursing, physiotherapy and medical care to this patient cohort in their home environment with a view to achieving improved outcomes through timely access to specialty specific clinicians; through reduced presentations and admissions to hospital and via the delivery of health care in a supportive and familiar environment

2.2 Rationale/business case

The RCCP is currently a St George Hospital hosted service providing a satellite service to Sutherland Hospital and is operationally managed by the Medicine 1 Service Line Nurse Manager. Medical governance for RCCP is managed independently at both St George and Sutherland Hospitals through the respective Respiratory Departments. Nursing and physiotherapy staff currently work across both sites in line with demand under the direction of a Nurse Unit Manager employed by St George Hospital. Administration support is provided for the service by a position located at St George Hospital, but funded by Sutherland Hospital This restructure proposal is to align the nursing, physiotherapy and administration staff to a specific site to allow for improved management of the program in line with site specific demand, indicators and service vision.

The benefits of the restructure include:

Site specific nursing management structure

Improved medical governance working in collaboration with site based Nursing Management structure

Single (site) team for projects & service development

Clarity of medical staff practices and protocols at each site

Transparency and governance over site based projects and product trials

Improved service model specific for each site

Nursing and financial management of Sutherland Hospital nursing staff by Sutherland Hospital nursing management structure

Improved supervision of Sutherland Hospital RCCP nursing staff by local nursing management

Removal of nursing management layer, based at St George Hospital, within nursing structure

Opportunity for nursing staff at Sutherland Hospital to professionally develop in RCCP service through secondments

Model of care improvement & expansion at each site in line with vision and demand:- o Improvements in discharging process o Redistribution of resources & design of model of care for peak activity o Adaptation of time limited service o Introduction of technology & models of care

Alleviation of confusion of the hosted service model

3. Proposed Changes The is one proposed change: Change of Reporting Lines

The reporting line for 2.8 Registered Nurse’s, 0.53 Level 1 Physiotherapist and 1 Administration Officer Level 2 will change. These staff will report to the Nurse Manager – Southcare & Rehabilitation via the Physiotherapy Unit Head / SHALT – Southcare at Sutherland Hospital The remaining 3.77 Registered Nurses and 1.71 physiotherapists (1 FTE level 3, 0.71 FTE level 1) will continue to report to the Nurse Unit Manager RCCP at St George Hospital. There are no proposed changes to position descriptions, classifications or grades of the positions. The changed reporting line will be reflected in the position descriptions. 4. The recruitment and matching process No recruitment matching process is necessary as the restructure will result in a change of reporting line for 4.33 staff. The work location of the current staff will be discussed on an individual basis and personal preferences will be considered. Any staff members not located for all their rostered hours at their preferred work location will be offered available hours at their preferred location. It is anticipated that the majority of staff’s preferences will be able to be accommodated.
