U.S. News publishes Best Hospitals to help guide patients who need a high level of care because of age or multiple health problems. Objective measures such as patient survival and safety data, adequacy of nurse staffing levels and other data largely determined the rankings in most specialties. The rankings are freely available at http://health.usnews.com/best-hospitals and will appear in the U.S. News “Best Hospitals 2015” guidebook. HIPAA Tip
QUESTION: WHAT CAN I DO TO PROTECT PATIENT PHI DURING MY DAILY ACTIVITIES? AANNSSWWEERR:: The following is a list of tips to safeguard PHI and help prevent a HIPAA violation. Everyone is responsible in the prevention of a HIPAA Patient Privacy violation.
Part II of II 1. When releasing patient Protected Health Information (PHI).
A. Verify the identity of the individual requesting the PHI. B. Ask yourself the question: Is the request for PHI needed for TPO (Treatment, Payment
Operations)? C. If the request for PHI is needed for TPO, no authorization is required from the patient prior
to the release. D. If the request for PHI is NOT needed for TPO, the patient or legal representative MUST
complete and sign HIPAA Form # 17, “Authorization for Use and Disclosure of Protected Health Information” prior to the release of PHI.
2. Do not leave phone messages regarding a patient’s condition or test
results on patient answering machines. You can leave your name and phone number and best time to return call.
3. Computer screens; suspend or log off when walking away from your PC. Computers, workstations, mobile devices, etc. are for staff use only.
4. When sending internal EMAILS containing patient PHI:
Send minimum PHI if necessary; Do NOT send to an email distribution list; Do NOT include patient information in the subject line; and Delete the email after it has been read.
5. External EMAILS containing PHI must be encrypted (secure):
Use your TMMC email account; To force secure encryption in your email, type the word “Secure” as the first word in subject
line and follow guidelines for internal emails.
6. Personal Devices and Social Networks; Face-book, You-Tube, Twitter, etc.: Do NOT take, email, text, share, or post patient photos, patient information or anything
that can identify the patient on any social network or to your personal devices.
