Lessons learned
Health Facility Reports and Standards of Care are useful tools for transition planning:
• Promoted programmatic data-driven discussion
• Buy-in for transition of CQI activities to MOH
• Identified special needs on planning capacity building at districts and provincial level
Next steps
• Standardize graphics and tables (slide set) to include in all health facilities reports;
• Capacity building in data analysis;
• Develop a M&E framework at site and provincial level to monitor the transition process (outputs and outcomes indicators and targets)
