Objectives
Our health center will provide students with a place to go for
psychological, physiological and emotional needs. The center will encourage students to come in
and improve their learning experience by talking about the
things that are holding them back which may be many things in the environment or with their health.
History
Nestled in the center of the small town of
Silver Grove, population just over 1200, sits a small
school of around 300 children preschool-
12th grade.
History
Since the parents of these 300 students do not have
readily available access to public transportation or
healthcare providers the school district has opened a
School-based Health Center in order to help parents
maintain their child’s health.
The Health Center provides students with a variety of
primary, preventative, and mental healthcare
services.
Statement Of Need
The Center has currently lost some of the funding it has received in the past.
The Center is a place where children can go to discuss what is going on in their lives and have a chance to take control of their health.
The Center’s employees are able to know each student individually so the students place their trust in these individuals in order to cope with everyday problems.
The Center provides happier children who are able to concentrate on their schoolwork and futures.
The Center needs help to provide more information to parents to full to encourage them to be more involved in their processes.
Helping Through Awareness
The Center is used to break down barriers
between students, the school and parents.
The Center helps is helping students who:
Do not have access to a ride to a primary care
physician.
Who do not know where else to go for help.
Helping Through Awareness
The Center provides parents with:
A piece of mind that they do not need to take
off work for doctor appointments.
A place to go for information about how to
keep up their child’s health.
Happiness that their children have a place to
go to receive information on how to keep
themselves healthy and happy.
Helping Through Awareness
The Health Center provides the following services:
Well exams
Sports physicals
Immunizations
Evaluation and treatment of minor illnesses/ injuries
Counseling services
Assistance with chronic disease management
Referrals to doctors, dentists, therapists, and social services agencies
Current Funding
The Health Center was funded by a grant from the
Health Foundation of Greater Cincinnati and
continues by grants and private or business
donations.
Budget
The Center has no overhead being part of
a school. Therefore, all proceeds go to the
direct needs of the students and the
center.
The Center will use proceeds to stay open
and continue to help the students in the
community.

Vaccinations
Supplies
Information
Updating resources
Build awareness in the school
The money will be used to help around 300 children in the community
of Silver Grove
