Your Affiant has probable cause to believe that on October 30, 2018, Michael Collins, a FDC, OIG Inspector assigned to Wakulla Correctional Institution, did violate Florida State Statute 837.012, Perjury when Not in an Official Proceeding, when he made a false statement, which he believed was untrue, under oath, not in an official proceeding, regarding any material matter to wit:
