nursing services, 12 organized HD) CDPHE completed state assessment

• Fountain Valley Community Services started in 1997

• Committed Citizens• Catholic Health Initiative had funded a

Community Indicators Project• In January 2005, EPCDHE joined

Fountain Valley Healthy Community Initiative and introduced MAPP and PACE/EH

Next Steps for EPCDHE

• Expand community ownership of process Form task forces to address top

• Replicate the Fountain model in other communities in El Paso County

• Need a champion with the ability to execute the plan.

• Work with the willing.

• Enlist partners to accomplish goals.

• Document the process.

• Avoid “paralysis by analysis.”

• Measure and display results early, often and regularly.
