There are 2 types of TB

Latent TB Infection TB Disease

Latent TB infection

A person with latent TB infection :

- Has a POSITIVE skin test

- Has a NORMAL CXR

(granuloma, calcification)

Latent TB infection

- Has breathed in TB germs

- Feels fine, is not sick

- Cannot spread TB germs to others

TB disease

A person with TB disease :

- Has a POSITIVE skin test

- Has a BAD CXR

TB disease

- Feels sick

- Can spread TB germs to others

- TB disease can be cured with medicine!!

Tuberculin skin test

Mantoux test - Purified protein derivative (PPD) 0.1 ml ID- อานผลที่ 48-72 hr- ใหผลบวกไดภายใน 2-10 wk หลังจากไดรับเชื้อ

Mantoux test - Immediate hypersensitivity reaction : 24 hr- Delayed (cellular) hypersensitivity reaction

T-cell

Lymphokines eg. IFN-y, IL- 8

Migration

- Vasodilatation

-Tissue edema

- fibrin, Inflammatory cellInduration & Erythema

Max : 48 – 72 hrs

Positive tuberculin skin test

Induration :≥ 5 mm

- สัมผัสกับผูปวยวัณโรค sputum AFB +ve และไมเคยได BCG

- CXR เขาไดกับวัณโรค

- ภูมิตานทานต่ํา เชน HIV, organ transplant, steroid treatment

≥ 10 mm- อยูในถิน่ที่มีอุบัติการณวัณโรคสูง

- โรคทีท่ําใหภมูิตานทานต่ําเล็กนอย เชน DM, cancer, CRF

- เด็กอายุนอยกวา 4 ป

- เด็กทีม่ีประวัติสัมผัสกับผูใหญที่มีความเสีย่งสูง

≥ 15 mm- เด็กอายุมากกวา 4 ป ที่ไมมีปจจัยเสี่ยง

1. Infection : virus, bacteria, fungus2. Recent lived vaccine : MMR3. Metabolic disease : Chronic renal failure4. Malnutrition5. Lymphoid disease : lymphoma, chronic leukemia6. Drug : corticosteroid, immunosuppressant7. Age : newborn8. Stress : surgery, burn, mental illness

- Segmental or lobar pneumonia

- Atelectasis
