Planning
• Planning is the phase of the DENTAL HYGIENE PROCESS in which priorities are set, client goals are established and interventions & outcome measures are determined
Planning CARE PLAN
• Its purpose is to develop the plan of care that will target the resolution of an oral health problem amenable to dental hygiene care, the prevention of a problem, or the promotion of oral health
CARE PLAN
• Therefore, the phrase CARE PLANCARE PLAN, rather than TREATMENT PLAN, is consciously used to denote the broad range of preventive, educational, therapeutic, & support services within the scope of dental hygiene practice
CARE PLAN
• Given the human need for responsibility for oral health, the dental hygienist actively involves clients, according to their ability & motivation, in the process & obtains the informed consent of the client (or the client’s guardian) before initiation of care
• The tangible outcome of the planning phase is the dental hygiene care plan
To formulate a care plan, the dental hygienist must:
• Possess a sound background in dental hygiene theory
• Be thoroughly familiar with the parameters or standards of dental hygiene care
• Possess the ability to collect, analyze, & interpret client data
• Develop dental hygiene diagnoses• Formulate client goals
• Select supportive dental hygiene intervention
• Synthesize the collected information into a written plan
• Communicate oral health need to clients effectively
• View the dental hygiene care plan within the context of the total treatment plan developed by the dentist
Dental Hygiene Care Plan• Is a written BLUEPRINT that directs the
dental hygienist & client as they work together to meet the client’s oral health goals
• The plan increases the likelihood that the oral healthcare team will work COLLABORATIVELY to deliver client-focused, goal-oriented, individualized care to clients
• The plan facilitates the monitoring of client progress, ensures continuity of care, serves as a vehicle for communication among healthcare professionals & increases the likelihood of quality care
CARE PLAN
• The plan specifies the dental hygiene diagnoses, client goals, dental hygiene interventions, & evaluative strategies to measure the proposed & expected outcomes
CARE PLAN
• Once the CARE PLAN is written & shared with the client, the CARE PLAN becomes a legal contact between the dental hygienist and the client
• Once the diagnosis is made, client-centered goals are established by the dental hygienist in conjunction with the client
CARE PLAN
• From the established client goals, interventions & expected outcomes can be drafted
• With client input, priorities for each goal are established and then appointment planning can begin
• Once priorities are established, the dental hygienist estimates the time necessary to complete each intervention

• Evaluation is a critical component to the successful outcome of dental care

• Evaluation allows the clinician’s to measure the short-term achievements of client-centered goals as well as to anticipate the client’s long-term prognosis in maintaining the goals achieved
• Evaluation includes the monitoring or reassessing the client’s self-care, changes in self-care behaviours, and/or parameters of oral health & disease at continued care

• Although evaluation is indicated as the 5th phase of the dental hygiene process, an evaluation strategy is determined in conjunction with the planning phase and is evident during the implementation of care

• BE SURE THE CLIENT HAS PARTICIPATED IN THE DEVELOPMENT OF CLIENT-CENTERED GOALS & IS IN COMMITTED TO THE GOALS
• EXPLAIN TO THE CLIENT HOW THE GOALS WILL BE USED & HOW CLINICAL OUTCOMES OF CARE WILL BE RELATED TO THE ORIGINAL GOALS
• REINFORCE THAT THE DENTAL HYGIENIST & CLIENT ARE WORKING IN PARTNERSHIP, AS CO-THERAPISTS, TO ACHIEVE THE CLIENT-CENTERED GOALS
