Altogether Better have Worked with more than 18,000 volunteer
community health champions who have supported more than 100,000
people in their communities to live better And the numbers continue
to grow.. 4. Community Health Champions are people who voluntarily
bring their life experience and their ability to relate to people
to promote health in different communities. 5. COMMUNITY HEALTH

means little if no one is LISTENING We want to be in RELATIONSHIP

relationship between citizens and services What have we learnt

conversations that patients have with clinicians and other health
care workers A productive way to improve the whole health service
is to transform the way the NHS does business by supporting really
