Care planning is an essential part of healthcare, but is often misunderstood or regarded as a waste of time. People with a chronic medical condion* may be able to access Medicare benefits to cover allied health services to help manage your condion. Care planning provides a ‘road map’ of sorts, to guide all who are involved with your care. The first step in care plan- ning is to gain an accurate and comprehensive assessment that includes consideraon of your unique abilies, physical, social and emoonal needs, and cultural preferences. Re- viewing your care plan is the best way to assess if it is work- ing or needs to amended—this generally happens 3-6 monthly. Eligibility Anyone with a chronic or terminal condion can have a GP Management plan. * A chronic condion has been, or is likely to be present for at least six months Other Requirements A person may be referred for allied health services and Med- icare rebates may apply if they are being managed by their ‘usual’ doctor and have: A GP management plan (GPMP) also referred to as a ‘care plan’ & a team care arrangement (TCA) - a plan developed by your doctor & nurse with you and at least two other care providers in- volved in your ongoing care. It is a good idea to talk to our Skin Doctors about your level of risk and for advice on early detecon. It’s important to know your skin and what is normal for you, so that you noce any changes. Skin cancers rarely hurt and are more frequently seen than felt. Develop a regular habit of check- ing your skin for new spots and changes to exisng freckles or moles.
