Image ConsentPlease print all details

Occasionally, we take photographs of the children at our Club. We may use these images in printed publications that we produce, in displays and on our website. We may also make video or webcam recordings for archival and promotional use.

We may also send images to the news media, or our Club may be visited by the media who will take their own photographs or film footage (for example, of a visiting dignitary or other high profile event). Players will often appear in these images. The news media may use the images in printed publications (including local or national newspapers), on televised news programmes or on their website. They then store them in their archive. They may also syndicate the photos to other media for possible use, either in printed publications, on websites, or both. When we submit photographs and information to the media, we have no control on when, where, if or how they will be used.

To comply with the Data Protection Act 1998, we need your permission before we can photograph or make any recordings of your child.

Conditions of use:

1.This form is valid for the period of time your child holds membership of this Club. Images of your child will not be used after this time except on archived websites. Please write to the Club if you wish to withdraw consent at any time.

2. The images we take will be of activities that show the Club and children in a positive light.

3. Embarrassing or distressing images will not be used. The images will not be associated with negative or sensitive issues.

4. We may use group or team photographs.

5. We will only use images of players who are suitably dressed.

6.We will make every effort to ensure that we do not allow images to be taken of any children for whom we do not have permission or who are ‘at risk’ or disallowed from having their photographs taken for legal or social reasons.

7.We will take all reasonable measures to ensure the images are used solely for the purposes for which they are intended. However we cannot guarantee this and take no responsibility for the way images are used by other websites or publishers or for any consequences arising from publication.

Please note that websites can be viewed throughout the world and not just in the United Kingdom where UK law applies. In giving your consent you understand that images may be used in printed and electronic form.

Respect Codes Of ConductFootball is the national game. All those involved with the game at every level and whether as a player, match official, coach, owner or administrator, have a responsibility, above and beyond compliance with the law, to act according to the highest standards of integrity, and to ensure that the reputation of the game is, and remains, high. This Code applies to all those involved in football under the auspices of The Football Association.

Community: Football, at all levels, is a vital part of a community. Football will take into account community feeling when making decisions.

Equality: Football is opposed to discrimination of any form and will promote measures to prevent it, in whatever form, from being expressed.

Participants: Football recognises the sense of ownership felt by those who participate at all levels of the game. This includes those who play, those who coach or help in any way, and those who officiate, as well as administrators and supporters. Football is committed to appropriate consultation.

Young People: Football acknowledges that public confidence demands the highest standards of financial and administrative behaviour, and will not tolerate corruption or improper practices.

Trust and Respect: Football will uphold a relationship of trust and respect between all involved in the game, whether they are individuals, clubs or other organisations.

Violence: Football rejects the use of violence of any nature by anyone involved in the game.

Fairness: Football is committed to fairness in its dealings with all involved in the game.

Integrity and Fair Play: Football is committed to playing to win consistent with Fair Play.

RespectRespect is The FA’s response to a clear message from throughout the game, that the health of football depends upon high standards of behaviour on and off the pitch.

Respect is a behavioural code for Football.Respect is about recognising that the integrity of the game is more important than the result of the match.Respect is For ALL and plays a key part in uniting the game.Respect is about creating an understanding of what is acceptable and unacceptable behaviour in Football.Respect is about those involved taking responsibility for the consequences of their own actions.Respect is about supporting match officials to do their job.Respect is not a slogan. It is a collective responsibility of those involved in football to create a safe, fun and inclusive environment in which the game can take place.

Young Players - Respect Code Of Conduct

When playing football, I will:Always play to the best of my ability and for the benefit of my team.Play fairly – I won’t cheat, dive, complain or waste time.Respect my team-mates, the other team, the referee or my coach/team manager.Play by the rules, as directed by the referee.Be gracious in victory and defeat – I will shake hands with the other team and referee at the end of the game.Listen and respond to what my coach/team manager tells me.Understand that a coach/team manager has to do what is best for the team and not one individual player.Talk to someone I trust or the club welfare officer if I’m unhappy about anything at my club.

I understand that if I do not follow the Code, any/all of the following actions may be taken by my Club, County FA or The FA. I may:

Be required to apologise to my team-mates, the other team, referee or team manager.Receive a formal warning from the coach/team manager or the club committee.Be dropped or substituted.Be suspended from training.Be required to leave the club In addition:My club, County FA or The FA may make my parent or carer aware of any infringements of the Code of Conduct.The FA/County FA could impose a fine and suspension against my club.

Spectators - Respect Code Of Conduct

We all bear a collective responsibility to set a good example and help provide a positive environment in which children can learn and enjoy the game. Play your part and observe The FA’s Respect Code of Conduct for spectators at all times I will:

Remember that children play for FUN.Applaud effort and good play as well as success.Respect the Referee’s decisions even when you don’t agree with them.Appreciate good play from whatever team it comes from.Remain behind the touchline and within the Designated Spectators’ Area (where provided).Let the coach do their job and not confuse the players by telling them what to do.Encourage the players to respect the opposition, referee and match officials.Support positively. When players make a mistake offer them encouragement not criticism - never engage in, or tolerate, offensive, insulting, or abusive language or behaviour.

I understand that if I do not follow the Code, any/all of the following actions may be taken by my club, County FA or The FA. I may be:

Issued with a verbal warning from a club or league official.Required to meet with the club, league or CFA Welfare Officer.Required to meet with the club committee.Obliged to undertake an FA education course.Obliged to leave the match venue by the club.Requested by the club not to attend future games.Suspended or have my club membership removed.Required to leave the club along with any dependents.

In addition:The FA/County FA could impose a fine and/or suspension on the club.

Coaches, Team Managers And Club Officials – Respect Code Of Conduct

Match Officials - Respect Code Of Conduct

We all have a responsibility to promote high standards of behaviour in the game. The behaviour of the match officials has an impact, directly and indirectly, on the conduct of everyone involved in the game – both on the pitch and on the side-lines. Play your part and observe The FA’s Respect Code of Conduct of match officials at all time. I will:

Be honest and completely impartial at all times.Apply the Laws of the Game and competition rules fairly and consistently.Manage the game in a positive, calm and confident manner.Deal with all instances of violence, aggression, unsporting behaviour, foul play and other misconduct.Never tolerate offensive, insulting or abusive language or behaviour from players and officials.Support my match official colleagues at all times.Set a positive personal example by promoting good behaviour and showing respect to everyone involved in the game.Communicate with the players and encourage fair play.Respond in a clear, calm and confident manner to any appropriate request for clarification by the team captains.Prepare physically and mentally for every match.Complete and submit accurate and concise reports within the time limit required for games in which I officiate.

Adult Players - Respect Code Of Conduct

Social Media And Online Code Of Conduct

Fry Club JFC understands the importance of new technology for children and young people’s development. However we recognise that relevant safeguards need to be put in place by sports clubs to ensure children and young people remain safe and responsible whilst online or using social media.We ask that all parents/guardians spend a few minutes to read through and discuss this statement with their child/children and then sign and return this form to the Fry Club JFC Welfare Officer.

I will be responsible for my behaviour when using the internet and social media, including the content I access and how I conduct myself.

I will not deliberately create, browse or access material that could be considered offensive or illegal.

I will not use social media or the internet to send anyone material that could be considered threatening, offensive, upsetting, bullying or illegal.

I understand that all my use of internet and social media is potentially visible to everyone in the online world and that any issues involving my behaviour which may be perceived as being detrimental to the Club or its reputation may be addressed by my coach or other officials at the Club.

I understand that these rules are designed to keep me safe and if they are not followed my parents/guardians may be contacted.
