There are four faculty levels and three administrative positions surveyed for each ophthalmology specialty:

The following data reflecting the physicians’ identification and work effort is requested:

Physician Identification (ID): Your organization’s physician tracking ID. This is requested so that you can track the information your organization provides by individual physician in case SullivanCotter needs to follow up with any questions.

Faculty Position Level: Record the faculty position level for each physician: 1= Instructor, 2= Assistant Professor, 3= Associate Professor or 4= Professor. This column should be recorded for every physician.

Administrative Position Level, if applicable: Record the administrative position level, if it is applicable: 5= Research Director, 6= Residency Program Director or 7= Chair. This column will not apply to all physicians.

Primary Specialty Code: Record the specialty code that reflects the physician’s, PhD’s or optometrist’s primary practice area. The codes are shown on page 6 and on the data collection form. This column must be recorded for every physician.

Secondary Specialty Code: If the physician spends at least 20% of his or her time in a secondary specialty, record that specialty code in this column. The codes are shown on page 6 and on the data collection form. This column will only be recorded for those physicians who spend at least 20% of their time in a secondary area. Not all physicians will have a secondary specialty code.

Full-Time Equivalent (FTE) Status: Data reflecting the FTE status of the physician is required for each physician. This information is critical so that all data can be adjusted to reflect a full-time equated physician. If the physician works less than full-time for the organization, then report his/her FTE status based on the expected work effort. For example, if a physician is expected to work half-time for the organization and spends the other half of his or her time in private practice, record a .50 FTE. All full-time physicians would be recorded as 1.0.

Administrative Time: Record the number of one-half day sessions the physician spends per week on administrative work for which they are paid. (This does not include administrative activities that are the routine part of any physician’s job for which they are not paid).

Clinical Time: Record the number of one-half day sessions the physician spends per week on clinical work. For example, a 1.0 FTE physician who spends all of his/her time on clinical activities would have a maximum of 10 one-half day sessions per week; a 1.0 FTE physician who spends half of his/her time on research activities and half of his/her time on clinical activities would have a maximum of five one-half day sessions per week for clinical time.

Research Time: Record the number of one-half day sessions the physician spends per week on research activities. For example, a 1.0 FTE physician who spends all of his/her time on research activities would have a maximum of 10 one-half day sessions per week; a 1.0 FTE physician who spends half of his/her time on research activities and half of his/her time on clinical activities would have a maximum of five one-half day sessions per week for research time.

Teaching Time: Record the number of one-half day sessions the physician spends per week on formal teaching activities. For the purposes of this survey, teaching time is intended to capture formal teaching activities such as preparing for and conducting lectures; it does not include time spent working with residents while performing clinical activities. For example, a 1.0 FTE physician who spends all of his/her time on teaching activities would have a maximum of 10 one-half day sessions per week; a 1.0 FTE physician who spends half of his/her time on teaching activities and half of his/her time on clinical activities would have a maximum of five one-half day sessions per week for teaching time.

The combined administrative, clinical, research and teaching time should not amount to more than 10 one-half day sessions for a full-time physician. Physicians with an FTE status less than 1.0 will have fewer than 10 one-half day sessions. For example a physician with an FTE of .80 will have a maximum of eight one-half day sessions.

The following data reflecting the physician’s total cash compensation is requested:

Fixed Salary: The compensation that is set at the beginning of the fiscal year. Report in thousands (e.g., $200,000 = $200.0).

Departmental Supplemental/Variable Income: Income that is not set at the beginning of the fiscal year and is tied to the performance of the department. This income is typically controlled by the organization from which the income is derived (e.g., the medial school or medical center). Report in thousands (e.g., $10,000 = $10.0).

Bonus/Incentive Compensation: Compensation paid to the physician which varies based on the physician’s productivity and/or performance. Report in thousands (e.g., $10,000 = $10.0).

Other Income: Other cash compensation paid to the physician that is not part of earnings within the faculty practice plan however; it is compensation received as part of the physician’s employment with the medical school or medical center. For example, this might include compensation paid for performing services for a public entity such as a county or veterans administration hospital. This does not include cash compensation earned in the physician’s private practice. Report in thousands (e.g., $10,000 = $10.0).

Gross Patient Charges (GPCs). Include the GPCs generated for all direct professional services provided by the incumbent. Do not include GPCs for ancillary services and physician extenders. If your organization tracks GPCs with ancillary services and physician extenders included, either subtract those figures or do not report the data. If your organization does not track GPCs by individual physician, leave this section blank. Report in thousands (e.g., $800,000 = $800.0).

Collections. Include the collections generated for all direct professional services provided by the incumbent. Do not include collections for ancillary services and physician extenders. If your organization tracks collections with ancillary services and physician extenders included, either subtract those figures or do not report the data. If your organization does not track collections by individual physician, leave this section blank. Report in thousands (e.g., $400,000 = $400.0).

Work Relative Value Units (wRVUs): Record the annual wRVUs performed by the incumbent. Included in this calculation are: wRVUs using the Resource Based Relative Value Unit Scale published by the Centers for Medicare and Medicaid Services for all payors. The wRVUs should be based on work personally performed by the incumbent and not weighted by a conversion factor. Do not include wRVUs attributed to: physician extenders; the technical component for laboratory; radiology or other procedures not personally performed by the physician; the practice expense component; or the malpractice expense component. If your organization does not track wRVUs by individual physician, leave this section blank.
