During the review period, any modifications or revisions to objectives and competencies should be documented on this form. Supporting performance-related documentation should be retained by the department. At the year-end performance review meeting, both the supervisor and employee sign the form. Needs Improvement (NI) – Performance does not meet minimum expectations and needs immediate improvement, although certain components of performance may be acceptable or improving. Individual demonstrates inability to meet overall requirements on a consistent basis. May require constant direction and regular follow-up.
