Instructions for self enrollment:
1. Go to our website www.northwesthumanservices.org
2. Click on the Patient Portal link
3. Agree to the terms and conditions and click on ‘continue’

4. If you have a Patient Portal Account, please login with your Username and Password
5. If you don’t have an account yet, please fill out the form below and Click ‘Next’
All fields with a red asterisk are required (*)
6. Enter your Health Insurance Information and your Health Insurance Claim Mailing Address
(This information is optional. Just click Next if this doesn’t apply to you)

7. Create a Username and Password
8. Create a Security Question
9. Create Password Recovery Credentials
10. Type the Captcha characters in the field provided
11. Click on “Complete Enrollment” to complete the registration
This completes the Patient Portal registration. You will receive an email with instructions on how to log in, once your request has been approved

To log in to your Patient Portal:
Go to www.nextmd.com or use the Patient Portal link posted on our website
Note: You will need the Username and Password, and the answer to the security question you created
during the registration process

Welcome to the Patient Portal!
Use the tabs at the top of the screen to navigate the patient portal
Please note that the Personal Health Record available from the Patient Portal is limited. For more detailed health information, a signed release
of information must be on file with our medical records department.
Click on the “MY CHART” tab to view your health information
MY CHART – Scroll down to view all the information on file or click on any of the topics listed on the left
to view the information related to the topic
Example: Click on ‘Medications’. The medications will display on the screen

Go to the “MAIL” tab to send us an email
Simply fill out all the fields and click on “Submit”
Go to the “SCHEDULE” tab to view your appointment history or to request an appointment.
To schedule an appointment, please fill out all the fields and click on “Submit”

Use the PATIENT EDUCATION tab to view Patient Education, use the Symptom Checker, view your
Medication or use the Health Decision Tools
Use the SETTINGS tab to edit your Account Settings or edit your ‘My Information’ (Name, Address, E-mail
Address and Phone number),
*Click on ‘Statement Notifications’ to view your statements
*Click on the ‘Payments’ tab to make payment
