A Simulation Exercise to test some

aspects of preparedness for an influenza

pandemic, involving the AUTONOMOUS

REGION OF GALICIA - SPAIN and the

NORTHERN REGION OF PORTUGAL,

took place on April 27 - May 7 2007.

Simulation Exercise

• To test the execution of national, regional, and institutional pandemic plans for influenza pandemic alert phases 3 to 5

• To improve the compatibility and interoperability of the two neighbouring regions’ pandemic plans• To determine the availability and suitability of containment measures• To provide experience and practice to those who may be involved in a response

The simulation activities covered a scenario occurring for 7 days:

Scenario of the Exercise

A scenario was designed by the Dirección Xeral de Saúde Pública’s Department of Epidemiology..

• It started with an H5N1 highly pathogenic avian influenza cluster in wild birds

• In a nature reserve in Galicia

• Visited by people living in the north of Portugal

• It ended with evidence of human-to-human transmission of H5N1 virus.

• Introduced the scenario to all participants of the health institutions involved in the response to each specific situation

• Sent back a response procedure to the Department of Epidemiology in Galicia, Spain

Scenario of the Exercise

1st day – Dead wild birds were found in a nature reserve in Galicia that was visited by Portuguese tourists

2nd day – Avian influenza virus A/H5 was isolated in wild birds

3rd day – A Portuguese tourist that visited the nature reserve in Spain was admitted to Viana do Castelo’s Hospital as a suspected case of avian influenza A/H5N1

4th day – Suspicion of a new case of A/H5N1 infection in a student that spent a previous day in the nature reserve

Scenario of the Exercise

5th day – Suspicion of two new cases of A/H5N1 infection in a student and in a saleswoman from Porto

6th day – Laboratorial confirmation of the first case of A/H5N1 virus infection in humans (saleswoman). Respiratory symptoms appeared in a contact of this case, without animal exposure

7th day – Laboratorial confirmation of the second case of A/H5N1 virus infection in a close contact of the first case (human-to-human transmission)

Lessons learned

The exercise provided an excellent opportunity to discover strengths and areas for improvement

• A stockpile of suitable personal protective equipment (PPE) should be maintained in Health Centres

• Health centres personnel should be trained in the use of PPE, in infection control measures to minimize the risk of disease transmission and in identification of possible cases of avian influenza

• Alert levels should be defined in the Pandemic Influenza Contingency Plan of the Northern Region

Lessons learned

• National technical orientations regarding antiviral prophylaxis, should be reviewed

• National orientations on contact tracing should be reviewed

• A communication plan for the Northern Region of Portugal should be developed

• A place for quarantine, in alert pandemic phase 4, should be previously planned

Conclusion

The scenario was viewed by the participants as being very realistic. It addressed all the various aspects of the pandemic alert phases 3 to 5 management, such as:

• antiviral supply and prophilaxy

• provision of antivirals to the areas first affected by the new strain of influenza

• internal and external communication

• transport and travel, and

• the cancelling of mass gatherings

• contact tracing
