Eugene Street • Greensboro, NC 27406 www.guilfordccn.org Eliminating Health Disparities and Improving Overall Health and Well-Being in Guilford County GCCN is a partnership between Guilford County’s safety net providers focused on the following: Coordinated System of Care Shared Resources & Information Standardized Case Management Standardized Eligibility and Enrollment GCCN Mission & Vision The mission/vision of the GCCN is to improve access to care, reduce and eliminate redundant systems of care, develop systems to allow for the analysis of outcomes (clinical and financial), improve care coordination and the creation of a shared information system that will allow clinicians and their support staff to care manage the target population. The GCCN, in collaboration with the Partnership for Community Care’s Uninsured Program, provides comprehensive, quality healthcare and related services through a network of volunteer physicians, pharmacies and other healthcare and related agencies for the adult, low-income, uninsured residents of Guilford County. What the GCCN Needs FROM YOU Our Volunteer Providers Primary care providers to serve as a patient’s medical home. Specialty care providers to serve as referral points for primary care providers whose patients have specialty care needs. To accommodate a set number of uninsured patients in your practice annually. Benefits To Providers The GCCN will care manage all patients enrolled in the volunteer provider practices to ensure: Low no-show rate. High patient compliance with treatment plans and access to free or reduced cost medications. Improved communication between primary provider and specialty provider. Controlled management and enrollment of the number of uninsured patients requesting unassigned medical care by private community providers. Accurate reporting of the number of patients seen and the total amount of donated services. Access to interpreters. Disease management education. Guilford Dental Access Program The Guilford Dental Access Program’s mission is to provide accessible, affordable and quality dental services to Guilford County residents with incomes between 0% and 200% of the federal poverty level. Guilford Dental Access Program is a program organized by Guilford Adult Health, Inc. Services are limited to extractions, pain management and minor restorative care. In order to qualify for services, patients will be referred by primary care providers who already provide medical care to county residents between 0% and 200% of the federal poverty level. There are two convenient locations in Greensboro and High Point. Volunteer providers who provide care to GCCN patients in their offices benefit from these and many more services: Lower no-show rate. Increased patient compliance with treatment plans. Improved communication between primary and specialty providers. More accurate reporting of patient numbers and amount of donated services by volunteer providers. Patients receive disease management education. Access to interpreters.

Who Does the GCCN Serve?
The GCCN serves any patient with an income between 0%–200% of the federal poverty level. Some member agencies have higher income limits. The groups which comprise the GCCN serve a patient population that exceeds 80,000 adults and children.
