Why do we need to work together?
We need to change to meet new challenges as:
• More people are living longer and, as a
result, have more complex health and care
needs
• Both NHS and social care budgets do not
meet demand
• Too many people are receiving care in the
wrong place at the wrong time which can
mean a poor and more expensive service
Mind the gaps
Main focus on how as a whole system we close the three main gaps together:

The North West London approach• Keep people as healthy as possible, rather than
just reacting when they become ill
• Provide the services, information and support so our residents can stay as healthy as possible
• Regular users of health and social care should get as much support as possible in their own home, and the help and expert advice they need to avoid problems becoming a crisis
• Trips to hospital should be absolutely necessary and planned, returning home quickly and safely, with the right support and follow-up in place.

Self care: PAM (patient activation measure) and a
bid to DPP (diabetes prevention programme)

• Brent is in the worst quartile nationally in terms
of people with a long-term condition feeling
supported to manage their condition, and there
is a wide variation in GP performance
• Brent is in the worst quartile nationally for patient
experience of GP services.

• 35% of all emergency admissions are for people
aged 65 and over; once admitted this group stays
in hospital longer, using 55% of all bed days.
• Brent has one of the highest percentages of deaths
taking place in hospital in the country.
Developing joined up frailty services.
• One borough responsible for all NWL hospital
discharges at NWP
• Integrated commissioning residential and nursing
care
• Integrated Rehabilitation and Reablement Services

• People with serious and long term mental health
needs have a life expectancy 20 years less than
the average.
Implementing a recovery pathway (integrating
health, social care, housing and employment) from
secondary services so people can live independent
lives
Promoting, protecting & improving our children and
young people’s mental health and wellbeing
• Joining up primary care, employment and
housing for people with common mental
illnesses in the WLA employment trailblazer
• Make Brent a Dementia friendly borough
• CAMHS local transformation plan

The area around Park Royal is changing with
growth in housing and extra care facilities which
is generating demand for health and care
services. The CMH site is under utilised and
could provide an ideal location.
Developing a health and well being centre around
a redeveloped CMH, and extra care and nursing
facilities to improve services and quality of life
whilst responding to the growing demand and
changing demography of the borough.One Public Estate bid
CCG strategic estates strategy – 3 community
hubs; Wembley, Willesden and CMH
