CMC, we know maintaining our equipment and facilities is essential to ensure a safe and efficient workplace and business.

Completing an annual physical is your opportunity to ensure your own health and safety. And that’s why we require

employees and spouses on our BCBSTX medical plan to get an annual physical.

Avoid Paying More for Medical InsuranceIf you are enrolled in CMC’s BCBSTX medical plan, getting an annual physical can save you $600 – $1,200.

To pay the standard rates in 2020, you and your covered spouse enrolled in the BCBSTX Medical Plan must get an annual physical between

November 1, 2018, and October 31, 2019, or you will each pay a $50 surcharge per month in 2020.

Schedule Your Appointment TodayIf you haven’t seen your doctor yet, call today — it can take up to six weeks to get

an appointment. Remember, CMC pays 100% of in-network annual physicals! Visit

BCBSTX.com or call 1-877-262-7977 to confirm your doctor is in-network.

In-network annual physicals are covered once in a calendar year rather than a

rolling 12 months. So if, for example, you got your annual physical in September

2018, you can get another one in June 2019.

When you make your appointment, make sure that you schedule your physical

with an in-network doctor so it’s covered at 100% by CMC. Once you get an

in-network physical, your doctor files a claim with BCBSTX, and BCBSTX notifies

CMC that you met the requirement. If your doctor’s office uses a code tied to “Preventive Health,” your visit will count as an

annual physical.

If your doctor sends you to another location to get blood work or other tests, coding is key. You need to verify the facility and services

performed are in-network and are coded as part of the annual physical. If during your visit you and your doctor also talk about a condition or

concern you have that is not part of your annual physical (for example, a sinus infection), that part of the visit may not be covered at 100%.

Your in-network doctor will file your claim with BCBSTX for you.

Confirmation and Reminder PostcardsIn April, CMC will begin mailing “High Five” postcards to employees and spouses

who got their physical and reminder postcards to those who haven’t. It can take

up to two months for your claim to process and CMC to receive notification you

met the requirement.

If it has been more than two months since you got your physical but you haven’t

received a confirmation postcard, it may be time to investigate how your visit was processed. See the Quick Tips section for more information.

4 Don’t delay! Schedule your appointment today. It can take up to six weeks to get in to see your doctor.

4 Contact BCBSTX online at BCBSTX.com or by calling 1-877-262-7977 to confirm your doctor is in-network or to locate a new one. Note: CMC covers your in-network physical at 100%. Out-of-network annual physicals are not covered but they count toward the requirement.

WHEN SCHEDULING YOUR VISIT

4 If it’s been less than a year since your last in-network annual physical, you can still make an appointment — you are covered for one in-network annual physical per calendar year (not a rolling 12 months).

CHECK YOUR EXPLANATION OF BENEFITS (EOB) AND KEEP A COPY OF THE ITEMIZED STATEMENT FOR YOUR RECORDS

4 Tracking is claims-driven and coding is key. Check your EOB to make sure your claim was processed correctly to avoid the surcharge. It can take from two to four weeks for a visit to be processed and posted on BCBSTX.com.

4 Go to BCBSTX.com to check your EOB. On your EOB look for the words “Physical History” as the description of services. This is how BCBSTX refers to annual physical exams and seeing it on your EOB will mean your visit was processed and coded correctly.
