Padlocked Emergency Exit: 1910.36 (d): An exit door must be unlocked. 1910.36 (d)(1): Employees must be able to open an exit route door from the inside at all times without keys, tools, or special knowledge. A device such as a panic bar that locks only from the outside is permitted on exit discharge doors.

1910.178(n)(4): The driver shall be required to slow down and sound

the horn at cross aisles and other locations where vision is obstructed. If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing.

1910.178(q)(1): Any power-operated industrial truck not in safe operating condition shall be removed from service. All repairs shall be made by authorized personnel.

1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.

1910.178(q)(1): Any power-operated industrial truck not in safe operating condition shall be removed from service. All repairs shall be made by authorized personnel.

Machine Guarding – Band Saw

1910.213(h)(1): …The sides of the lower exposed portion of the blade shall be guarded to the full diameter of the blade by a device that will automatically adjust itself to the thickness of the stock and remain in contact with stock being cut to give maximum protection possible for the operation being performed.

1926.405(b)(2) All pull boxes, junction boxes, and fittings shall be provided with covers.

1926.405(b)(2): Flexible cords and cable shall not be used where run through doorways, windows, or similar openings except as permitted in paragraph (a)(2)(ii)(1)

1910.305(a)(2)(x): Flexible cords and cables shall be protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points.

Abrasive Wheel Guarding

1910.215(a)(1): Machine guarding. Abrasive wheels shall be used only on machines provided with safety guards as defined in the following paragraphs of this section, except: wheels used for internal work (1910.215(a)(1)(i)) Mounted wheels, used in portable operations, 2 inches and smaller in diameter

(1910.215(a)(1)(ii)) Types 16, 17, 18, 18R, and 19 cones, plugs, and threaded hole pot balls where the

work offers protection. (1910.215(a)(1)(iii))

1910.215(a)(1): Guard design. The safety guard shall cover the spindle end, nut, and flange projections.

1910.215(b)(9): Tongue adjustment at top >1/4 inch

1910.215(a)(4): Work rest adjustment >1/8 inch

1910.23(c)(1): open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing

1910.23(d)(1): Every flight of stairs having four or more risers shall be equipped with standard stair railings or standard handrails

1910.23(d)(1)(ii): less than 44 inches wide having one side open, at least one stair railing on open side. (if both sides were

enclosed: one stair rail on right, descending side – 1910.23(d)(1)(i))

1910.37(a)(3): Exit routes must be free and unobstructed.

1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1-1965

Gas cylinders must be secured at all times to prevent tipping.

Gas cylinders can not be stored in public hallways or other unprotected areas.

Cylinders must be segregated in hazard classes while in storage. Oxidizers (oxygen) must be separated from flammable gases, and empty cylinders must be isolated from filled cylinders.

The proper storage for oxygen cylinders requires that a minimum of 20 feet is maintained between flammable gas cylinders and oxygen cylinders or the storage area be separated, at a minimum, by a firewall five (5) feet high with a fire rating of 30 minutes.

Propane Tanks: secured and no longer next to oxygen cylinders.

Portable electric equipment - 1910.334(a) 1910.334(a)(2)(ii): If there is a defect or

evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made.

1910.303(g)(2)(i): … live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact 1910.305(g)(1)(iv)[A]: Flexible

cords/cables used in lieu of fixed wiring

1910.303(g)(1): Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.

1910.1200(f)(5)(i): the employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with the identity of the hazardous chemical(s) contained therein

1910.219(d)(1): pulleys seven (7) feet or less from the floor or working platform shall be guarded.

Sec. 5 (a) Each employer –

(1) shall furnish to each of his employees

employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm to his employees

General Duty Clause

5(a)(1): Do not burn wood in a room where wooden pallets are manufactured!

5(a)(1): No working from the bucket of an excavator!
