Task Description Students are to complete a Work Health and Safety Quiz and short answer questions to demonstrate their understanding of Work Health and Safety concepts. Students are to then form small groups (4-5) to create a Work Health and Safety committee. The aim of the committee is to complete a mini audit/inspection of a sporting facility, storeroom or classroom in the school. Students will develop a Work Health and Safety Action Plan and Portfolio of work tasks. Students will then carry out the mini inspection as well as conduct maintenance on the area. Students will report their findings to a Supervisor/Manager and provide future recommendations.

John is a Richmount High School Rugby League coach. Before each game John must check all equipment and
get the field set up for the game, ensuring he meets all Rugby League safety requirements. On the 24th July 2018
John starts to complete his pre-game checks at 12.30pm as the game is meant to commence at 2.00pm. He is in
the football storeroom of the school when he realises that his kicking tee has been placed high on a shelf that is

Part B: Work Health and Safety Action Plan and Portfolio
Instructions to students:
• Work in a safe and appropriate manner at all times
• Follow all verbal and written instructions provided by your teacher and communicate effectively with others
• You must complete this task in a small group of 4-5
• Must be word-processed and handed to teacher in a Portfolio
• Students will be required to complete a group work Portfolio that includes an Action Plan, detailed work tasks, timeline, audit checklist and work log of completed work
• Students will have 2-3 weeks to complete this section
Task 1: Planning Work Tasks
In this section students are to form a group of 4-5 and develop a comprehensive work plan Portfolio that demonstrates the
group’s ability to organise work schedules and complete work tasks. This portfolio will be directly linked to the successful
maintenance of a sporting facility/area within your school.
Students are to hold a number of mock WH&S committee meetings with their group when completing their planning tasks.
These should be conducted before planning, after the inspection of area when undertaking action plan, before the work
maintenance begins, once a week during maintenance and on completion of all tasks. These meetings need to be recorded on
their planning schedule (Gantt chart) and have someone take minutes to include in their work Portfolio. (Attachment C)
The group Portfolio should include:
(i) Students are to develop a detailed list of work tasks that will be required to complete the Action
Plan/inspection and maintenance of a sporting facility/area within the school.
(ii) Plot these work tasks and their components onto a GANTT chart (see Attachment A for sample Gantt chart).
These can be downloaded onto I-pads or computers if available. Free Gantt programs are available online for
PC’s or students can create a simplified Gantt chart using Microsoft Word or Excel. These charts must clearly
show the group’s ability to prioritise work tasks required.
(iii) Complete the table outlining the factors that could possibly affect your performance throughout the
assessment and explain how the group plans to overcome these issues. (Attachment B)
(iv) Include the documented minutes from your groups’ committee meetings as part of your work Portfolio.

Task 2: Action Plan and Inspection
Your task is to complete an Action Plan on a sporting facility/area within the school that is used for Sport and Recreation. You
will need to register your facility/area with your teacher before it is undertaken to ensure that only one group is assigned to each
area.
You will need to complete an inspection of the area to determine what tasks need to be undertaken in the facility/area in order
to meet WH&S expectations and to maintain the facility in a safe manner.
The Action Plan must be word-processed and handed in prior to the commencement of the practical task.
Groups can use the template in order to complete their Action Plan. (Attachment D)
Students are to prepare an action plan addressing the following areas:
1. Equipment checklist for the required tasks. What specific resources are available to complete the tasks?
2. Explanation of the specific tasks that need to be completed.
3. Allocation of roles and responsibilities within the group. What will be done and by whom?
4. WHS analysis: Identification of potential manual-handling risks and management of tasks prior to the start of
the production according to organisational policies and procedures.
5. Safety Considerations when undertaking the maintenance of the facility/area.
6. Timeline. When will each task be completed?
Task 3: Work Log of Completed Tasks
Work Log and Photographic evidence
Using photographic images and a work log book (Attachment E), create a diary of the progress of the maintenance task and
any modifications (for unexpected alterations to the original plan). This is only brief documentation of the work in progress and
will be used to support the project evaluation.
The Work log should include:
1. Before and after photos of the sporting facility/area undertaken
2. A lesson-by-lesson log/diary of what the group is completing. What tasks are being undertaken each lesson?
3. Any modifications needing to be made to original Action Plan.

• Maintain a diary of completed work tasks (Attachment E)
• Take photographic evidence throughout to demonstrate work progress. This needs to be sent through with the Work Portfolio.
Using the group Action Plan developed students are to undertake practical maintenance of the specified sporting facility/area
within their school.
This practical maintenance will include:
• Undertaking any work tasks identified in their Action Plan
• Completing all tasks, within their control, to meet WH&S expectations of the area
• Taking before and after photos as well as progress photos of the tasks completed
• Maintaining the facility/area according to WH&S policy and maintaining the safety of all members of the group
• The group must meet once a week to discuss their progress and adjust their work according to any feedback
obtained during their meetings. This is to be documented in their log book/diary. Minutes from this meeting are to be
documented and submitted with Work Portfolio (Attachment C)

Maintenance/Incident report
Upon completion of the maintenance of the sporting facility/area students are to complete the attached Maintenance/Repair
Report (Attachment F). This report should be addressed to the WH&S coordinator or maintenance supervisor within the
school and should address any areas identified within the Action Plan that could not be completed due to being out of their
scope of responsibility.
The report should indicate:
1. Any hazards/risks/safety issues identified that were unable to be fixed/maintained within your group tasks
2. Recommendations on what needs to be undertaken in order to meet expected WH&S guidelines.
3. Any incidents or accidents that occurred when undertaking your work tasks.
4. Make suggestions for improving any Work Health and Safety practices within their facility/area undertaken.
Evaluation
Upon completion of the work tasks each individual in the group is to complete a self and group evaluation on their work
undertaken. Students are to complete the questions provided (Attachment G) to review their performance both as an
individual and as a group. The group is to conduct a final Committee meeting at the completion of the task to give feedback
to group members regarding their performances.
Evaluation is to be word processed and submitted at the completion of all work tasks.
Evaluation should include the following:
1. Commenting on your individual work performance when completing your Work Portfolio as well as the
Maintenance of facility/area
2. Commenting on your groups work performance when completing the Work Portfolio as well as the
Maintenance of the facility /area. Use feedback provided in your final Committee meeting.
3. Recommendations for future work. Areas for improvement both individually and as a group.
