Transplantationisaneffectiveandwell-establishedtreatmentthatcansignificantlybenefitAustraliansfacingillness,disabilityorprematuredeath.NotingthatthedemandfororgansfortransplantationcontinuestoexceedsupplyinAustralia,asintherestoftheworld,wearecommittedtooptimisingeverydonationopportunity.Cliniciansinthedonationandtransplantationsectors,governments,andmembersofthecommunityhavearoletoplayincontributingtoanorganandtissuedonationserviceforAustraliansthatmeetsourexpectations.
On2July2008theAustralianGovernment,recognisingtheimportanceofbridgingthegapbetweenthedemandfortransplantationandtheavailabilityoforgansandtissues,announcedanationalreformprogrammetoimplementaworld’sbestpracticeapproachtoorganandtissuedonationfortransplantation,whichwasendorsedbytheCouncilofAustralianGovernmentson3July2008.
InJanuary2009theOrganandTissueAuthority(OTA)wasestablishedtomanagetheimplementationofthenationalreformprogramme,andoverthatyeartheDonateLifeNetwork,anationalnetworkofDonateLifeAgenciesandhospital-basedmedicalandnursingspecialistsinorganandtissuedonation,wasestablishedtoprovideacoordinatedapproachtoorganandtissuedonationfortransplantation.
TherehasbeenstronggrowthindonationandtransplantoutcomessincetheestablishmentoftheOTAandtheDonateLifeNetwork.In2013,ournationalorgandonationoutcomewas58%higherthan2009andthenumberoftransplantrecipientswas39%higher.
Despitetheimprovementsindonationandtransplantationoutcomes,therearestillnotenoughdonatedorganstomeetthecurrentneedandthefutureneedwhichisanticipatedtogrowwiththeageingofAustralia’spopulationandtheincreasedincidenceofchronicdiseases.In2013,therewereapproximately1,500Australianswaitingforanorgan
transplantatanypointintime.ObviouslythereisstillmuchtodotocontinuetobuildonandsustaintheincreaseinorganandtissuedonationtoensurethatmoreAustraliansareabletobenefitfromtransplantation.
Todate,strategicprioritiestofocusimplementationofthenationalreformprogrammehavebeendevelopedonanannualbasis.Astheimplementationofthenationalreformprogrammematures,itistimelytodevelopastrategicplanoveralongerperiod.
ThisstrategicplanhasbeendevelopedwithinputfromtheAdvisoryCouncilandtheJurisdictionalAdvisoryGroup.Itaimstooutlineourvisionandprioritiesfor2014-18toensureasharedunderstandingofourpurposeandstrategiestoachieveournationalgoalof25organdonorspermillionpopulation(dpmp)by2018.
Thestrategiesandactionsoutlinedintheplanwillbereviewedandupdatedannuallytoensuretheplanremainscurrentandalignswithavailableresourcesandtheupdatedplanwillinformthedevelopmentofstrategicprioritiesforeachfuturefinancialyear.
Inthe2014federalBudget,theAustralianGovernmentannouncedplanstomergethefunctionsoftheNationalBloodAuthority(NBA)withtheOTA.Thisworkisbeingprogressedinthecontextofawhole-of-governmentapproachtoensurenewarrangementsareeffectivelyimplemented.
Wearecommittedtoachievingtheobjectives,andimplementingthestrategiesandactions,outlinedinthisstrategicplan.

1. Toincreasethecapabilityandcapacitywithinthehealthsystemtomaximisedonationrates,and
2. TobuildcommunityawarenessandstakeholderengagementacrossAustraliatopromoteorganandtissuedonation.

Our world’s best practice approachAustralia’snationalreformprogrammewasdevelopedinthecontextofclearinternationalcommitmentsandstatementsbytheWorldHealthOrganizationrelatingtotheobligationofgovernmentstobemoreaccountableandresponsibleforimplementingsafe,ethicalandeffectiveorganandtissuedonationandtransplantationsystemsatthenationallevel.
Evidencefromcomparablecountriesdemonstratesthatacoordinatednationalapproachandsystem,focusedonclinicalpracticereform,improvesorgandonationandtransplantationrates.ThekeyreformelementsadoptedinAustraliaandbyleadingcountriesinorganandtissuedonationare:

TheAustraliannationalreformprogrammehasbeeninformedbybestpracticeincountriesthatareworldleadersinorganandtissuedonation.

Organ and tissue donation - a precious opportunityOnlyaround1-2%ofpeoplewhodieinhospitals,dieinthespecificcircumstancesrequiredtobeapotentialorgandonor.Forthisreason,itisimportanttooptimisetheidentificationofpotentialdonorsandtheconversionofpotentialdonorstoactualdonors.
Unlikeorgandonation,manymorepeoplecanbecomeeyeandtissuedonorsastissuescanbedonatedupto24hoursafterdeathregardlessofhowandwheredeathoccurred.
Throughoutthisplan,unlessotherwisespecified,‘tissue’referstoalltissuetypes,includingeyetissue.
The2014-18strategicplanincludesoutcomedataforthe2013calendaryear.Asthisplanisupdatedforfuturefinancialyears,theoutcomedatawillbeupdated.
In2013theAustralianpopulationwas23,130,900withanestimated149,500deathsoccurring.
Ofthese,approximately75,400deathsoccurredinhospitalswitharound725potentialorgandonorsidentified.
Requeststofamiliesfordonationweremadeinaround700cases,withapproximately430familiesconsentingtodonation.
Injustunder40caseswherefamilyconsentwasgiven,donationdidnotproceedforavarietyofclinicalreasons.
Theresulting391deceasedorgandonorsenabled1,177organstobetransplanted,transformingthelivesof1,122transplantrecipients-thehighestdonationandtransplantationoutcomesincenationalrecordsbeganin1989.

AustraliahasachievedstronggrowthindonationandtransplantoutcomessincetheestablishmentoftheOTAandtheDonateLifeNetworkin2009.The2013donationoutcomewas10%higherthanin2012(391donorscomparedto354in2012).The2013outcomerepresenteda58%increaseinthenumberofdonorsin2009,theyeartheDonateLifeNetworkwasestablished.
Asaresulttheorgantransplantrecipientoutcomewas7%higherthanin2012(1,122transplantrecipientscomparedto1,053in2012).The2013outcomerepresenteda39%increaseinthenumberoftransplantrecipients(808)in2009,theyeartheDonateLifeNetworkwasestablished.
InthistimetherehavebeenmajorchangesinAustralia’sorganandtissuedonationinfrastructure,mostnotablywiththeembeddingofdedicatedmedicalandnursingdonationspecialiststaffinorganandtissuedonationinkeyhospitalsacrossAustralia,
whoworkcollaborativelywithstafffromeightDonateLifeAgenciesandtheirclinicalcolleagues.InitiativessuchastheintroductionoftheProfessionalEducationPackage(PEP)andtheDonateLifeAuditofpotentialdonoropportunitieshavefurtherdevelopedtheskillsofthehospitalbaseddonationspecialistdoctorsandnurses,whiletheintroductionofOrganDonationHospitalSupportFundinghasprovidedacontributiontowardsthecostsassociatedwithorgandonationactivity.
Furthermeasuressuchastheintroductionofanationaldonorfamilysupportservice,theimplementationoftheAustralianPairedKidneyExchangeprogrammeanddevelopmentoftheConsensusStatementonEligibilityCriteriaandAllocationProtocolsforOrganTransplantationfromDeceasedDonorshaveallprovidedgreatersupport,accessandsectoraccountabilitytotheAustralianpublic.

Whiletheneedfororgansandtissuewillcontinuetoexceedavailability,wecandomoretoincreaseourdonationandtransplantationoutcomes.ThisisachallengeforthewholeofAustraliansociety.

Despitetheprogressachievedtodate,thedemandfortransplantationinAustraliacontinuestoexceedthesupplyoforgans.Thewaitforatransplantcanextendtoyearsandcomesatasignificantcosttothoseneedingatransplant,theirfamilyandcommunity,andtothehealthsystem.Itisforthesereasonsthatcontinuedsustainedgrowthisrequiredoverthenextfouryearsto2018.
TheabilityofCommonwealth,stateandterritorygovernments,workingwiththecommunityandclinicianstomaximiseorganandtissuedonationfortransplantation,willrequireongoingfocusonkeyareassuchasincreasingthenumberofidentifiedpotentialdonors,improvingthenumberofsuccessfuldonationoutcomesandensuringthatappropriatesystemsareinplacetosupportorganandtissuedonationandtransplantation.InordertotrytomeetthedemanditisessentialthatweworkwithAustraliangovernments,thecommunityand,thedonationandtransplantsectorstodevelopstrategiesthatwillincreaseavailabilityofdeceaseddonororgansandmaximiseorgantransplantation.
TheAustraliancommunityalsohasanongoingroletoplayinincreasingorganandtissuedonationfortransplantation.Unlessindividualsandtheirfamiliesconsenttodonation,transplantratescannotimprove.Australiansneedtoconsiderorganandtissuedonationasthenormalandexpectedthingtodo,bothforthemselvesandtheirlovedones.Workwillcontinuewiththecommunitytomaintainactiveengagementandbuildonthehighlevelofsupportfororganandtissuedonation;toencourageallAustralianstoregistertheirdonationdecisionontheAustralianOrganDonorRegisterandmostimportantlytodiscusstheirdonationdecisionwithfamilymembers.
Whiletheneedfororgansfromdeceaseddonorsoutweighstheiravailability,anappropriatebalancemustbefoundtoaddresstheneedsofindividualswhowouldbenefitfromreceivingatransplant.
Unfortunately,whatevertheallocationprocess,therewillbepeoplewhocouldbenefitfromanorgantransplantbutwillnotreceiveoneduetothelimitedavailabilityofdeceaseddonororgans.
Inanattempttoexpandthenumberoforgansavailablefortransplantation,extendedcriteriafororgansuitabilityhavebeendevelopedforsomebutnotallorgans.Successwiththisapproachisbeingachievedinternationally.Extendedcriteriarelatetothecharacteristicsofthedonor(e.g.comorbidities,infectiousdiseases,age)orthedonororgan(e.g.longerischaemictime)thatcanbeassociatedwithpooreroutcomesaftertransplantationinsomerecipients.However,theprospectofthepotentialrecipientsurvivingconsiderablylongerthanmayoccurwithoutatransplant,andtheresultingcommunitybenefit,shouldbeweighedagainstthemedicalfactorsandpotentiallyhigherriskinvolved.Inthesecases,theapplicationofanalternateallocationsystemtomatchanextendedcriteriaorgantosuitablerecipientsshouldbeexplored.
Furthergrowthinorganandtissuedonationfortransplantationwillrequireustoworkevenmorecloselywithourclinicalandtransplantcolleaguesandthebroadercommunity.WemustensurethattheverybestuseismadeofthisscarceresourceinordertoprovideasmanyAustraliansaspossiblewithaccesstotransplantation.Thiswillnecessitatefurtherstrengtheningofthealreadystrongprofessionalrelationshipsthatexistbetweenthedonationandtransplantcommunitiestoensurethatmoreorgansareuseableandsurgeonsarebettersupportedtotransplantorganssafelyintothemostappropriaterecipient.Itwillrequireongoingdevelopmentofrelevantsystems,policiesandprotocolstokeeppacewiththechangingclinicalenvironmenttomaintainasafe,ethicalandeffectiveorgandonationfortransplantationsysteminAustralia.

OngoingawarenessofdevelopmentsinmedicaltechnologiesisintegraltounderstandingandutilisingthebestoptionsavailabletotheAustraliancommunity.Therearescientificdevelopmentsandresearchonthehorizonthatwillchallengecurrentacceptedclinicalandethicalpracticeindonationandtransplantation.Weneedtobepreparedtoaddressclinicalandethicalissuesthatmayariseinrelationtoanyevolvingtechnologyandtechniquesinorgantransplantationand,theongoingtransitionfrominnovationofresearchtoestablishedpractice.
Developmentsinthefieldoforgantransplantationsuchasmachinereperfusionoforganshavethepotentialtotransformandincreasetheopportunitiesforpatientsneedingatransplant.Repairandregenerationoforganswillhaveasubstantialimpactonthenumberofsuitableorgandonors.Bothmechanicalandbiological(stemcell)organreplacementtherapiesmaychangetheprofileofpeopleneedinganorgantransplant.Eyeandtissuebanksarestrongcandidatestobecomelaunchinghubsfornovelbiotechnology,enhancedtissuegraftsandbiotherapeutics.Workwiththeeyeandtissuebankingsectorwillcontinuetofosterastrongnationalnetwork,andconsistencyandsustainabilityofthesector.
WewillworkevenmorecloselywithourdonationandtransplantationsectorsandthebroadercommunitywiththeaimtoprovideAustralianswiththeopportunitytomakeinformeddecisionsabouttheirdonationandtransplantationoptions,inordertoprovidemoreAustralianswiththeappropriatetransplantoutcomeforthem.
TheOTAwillworkwiththeDepartmentofHealthandtheNBAonplanstomergefunctionstostreamlinecorporateandadministrativeactivitieswithinthecontextofawhole-of-governmentapproachtootherentitymergerstoensurenewarrangementsareeffectivelyimplemented.

1. Implementing the second stage of the DonateLife Clinical Governance FrameworkThesecondstageoftheDonateLifeClinicalGovernanceFrameworkwilldeliveratargetedhospitalpracticeimprovementprogramforthesubsetofDonateLifeNetwork(DLN)hospitalswithdemonstratedperformancebelownationalaverageoutcomesinrequestandconsentrates.ItwillprioritiseinterventionusingoutcomeandprocessperformancedatafromtheDonateLifeAuditandtheClinicalGovernanceFramework.

Delivering specialist education to health professionals involved in conversations with families about the opportunity for donationEducationwillcontinuetobeprovidedinallstatesandterritoriesthroughtheFamilyDonationConversation(FDC)workshopsoftheProfessionalEducationPackage.FDCworkshopsprovidehealthprofessionalswiththenecessaryknowledgeandskillstocommunicatewithgrievingfamiliesandsupportthemtomakeafullyinformedandenduringdecisionaboutdonation.
TheeffectivenessoftheFDCtraining,includingitsimpactonrequestingpractices,familyexperiencesandconsentratesfordonation,willcontinuetobeevaluated.

Conducting community awareness and education activities on organ and tissue donation, in partnership with sector and community organisationsThepublicwillcontinuetobeeducatedabouttheneedforfamilydiscussionandknowledgeoforganandtissuedonationdecisionsandbeprovidedinformationtoassisttheminmakingandregisteringdonationdecisionsontheAustralianOrganDonorRegister.
Inpartnershipwithsectororganisationsandthebroadercommunity,awarenessraisingactivitieswillcomprise:DonateLifeWeek;targetedcommunityeducationcampaigns;medialiaison;and,onlineandsocialmediaengagement.Theseactivitieswillincludetargetedoutreachwithcommunitiesanddemographicgroupsthatarelessengagedwithorganandtissuedonation,includingculturallyandlinguisticallydiverseaudiences.
5. Merge functions of the OTA and NBATheOTAwillworkwiththeDepartmentofHealthandtheNBAonplanstomergefunctionstostreamlinecorporateandadministrativeactivitieswithinthecontextofawhole-of-governmentapproachtootherentitymergerstoensurenewarrangementsareeffectivelyimplemented.
During2014-15,theOrganandTissueAuthority(OTA)willcontinuetoremainfocusedonthedeliveryofthenationalreformprogrammeobjectivesandoutcomes,andtoworkwithallourstakeholdersinachievingcontinuedandsustainedincreasesinorganandtissuedonation.Inparticular,theOTAwillbefocussingonfivekeypriorityareas:

1. Implementing the second phase of the DonateLife Clinical Governance Framework including a Targeted Hospital Improvement ProgrammeTheOTA,inpartnershipwithStateandTerritoryGovernmentsandtheDonateLifeNetwork,willimplementPhase2oftheClinicalGovernanceFrameworkandClinicalPracticeImprovementProgram(CPIP)includingaTargetedHospitalImprovementProgramme.Thisphasewillincludebuildingcross-borderpeerrelationshipsbetweenlikehospitalstostrengthenlocalorganandtissuedonationpracticethroughexposuretoalternatesystemsandapproacheswithinthenationalDonateLifeNetwork.Itwillprovidedevelopmentopportunities;sharedlearningprogrammes;andtargetedfocusonhospitalswiththegreatestpotentialforgrowthindonationoutcomesaspartoftheTargetedHospitalImprovementProgramme,toensuretheadoptionofbestorgandonationpracticefordonoridentificationandfamilyconsentconversationsinallDonateLifehospitals.Thisimplementationwillbemonitoredusingenhancedperformancemeasuresandreportingprocesses.

Continuing to deliver specialist education to health professionals involved in conversations with families about the opportunity for donationEducationopportunitiesforhealthprofessionalsinvolvedinfamilydonationconversationswillcontinuetobedevelopedanddelivered.Thisworkwillcontributetoincreasingthedonationconsentrate.AdvancedFamilyDonationConversation(FDC)workshopswillbedevelopedtoprovidetreatingcliniciansanddonationspecialistswithincreasedknowledgeandskillstosupportfamiliesinmakingdonationdecisions.Inaddition,anEyeandTissueFDCworkshoptailoredtotelephone-basedfamilyconsentforeyeandtissuedonationoutsideofthehospitalsettingwillbedevelopedanddelivered.
3. Developing a national vigilance and surveillance framework for organ donation and transplantationEndorsementoftheNationalVigilanceandSurveillanceFrameworktofurthersupportsafetyandqualityinorgandonationfortransplantationwillbesoughtfromstatesandterritoriesandkeystakeholders.TheFrameworkwillsupporttheinvestigation,resolutionandlearningfromadverseoutcomesrelatedtoorgandonationfortransplantation.Inaddition,aVigilanceandSurveillanceExpertAdvisoryCommitteewillbeestablishedtoprovideadviceonappropriateevaluation,reportingandremedialactionrequiredfornotifiedadverseeventsrelatedtodeceasedorgandonationfortransplantation.
4. Commencing development of the Australian Organ Matching SystemDevelopmentoftheAustralianOrganMatchingSystem(AOMS)willcommencetoreplacethecurrentorganmatchingsystem.TheAOMSwillbedevelopedwiththefunctionalitytoadaptinanagileandtimelymannertoimplementfutureclinicalinnovationsinthematchingandallocationofdonororgans.Thesystemwillallowforoptimalmatchingoftransplantrecipientsthroughtheapplicationofbest-practicealgorithmstofurthermaximisetheequityofaccessandclinicaloutcomesoftransplantsinAustralia.
5. Conducting community awareness and education activities on organ and tissue donationTheproportionoflegalconsentregistrationsontheAustralianOrganDonorRegister(AODR)willbeincreasedbyintroducingthefunctionalityforelectronicsignaturestoregisterlegalconsent.Thiswillremovethecurrentbarriersintheprocess,aswellasprovidinggreatercertaintyofanindividual’sdonationdecisionforcliniciansandfamiliesatthefamilydonationconversation.TheelectronicregistrationprocesswillbesupportedbytheOTAthroughtheimplementationofanationalonlinedonorregistrationcampaignandrevisingtheProfessionalEducationProgramtoincorporateAODRchanges.
