Opioids in Illinois Overview Since 2008, more than 11,000 people have died in Illinois as a result of opioid misuse; in 2016 alone, nearly 1,900 people died due to overdoses. As a response to rising opioid use and death across Illinois, the State of Illinois issued the State of Illinois Opioid Action Plan in September of 2017. This action plan aims to reduce opioid deaths by 33% in three years through pillars of (1) Prevention, (2) Treatment and Recovery, and (3) Response. Women are not spared from the impact of the opioid crisis in Illinois. The number of women of reproductive age (15-44 years old) who died of opioid-related poisonings increased from 122 in 2008 to 282 in 2016. More specifically, between 2013 and 2016, the rate of heroin-related deaths among women of reproductive age doubled, and the rate of deaths related to synthetic opioids (e.g., fentanyl) increased by nearly 6-fold. Opioid-related deaths to women while pregnant or in the first year postpartum increased by 4-fold between 2008 and 2016. There has been a subsequent impact on the health of infants related to the increase in opioid use among women. Between 2011 and 2016, there was a 52% increase in rates of neonatal abstinence syndrome (drug withdrawal syndrome seen in neonates) for Illinois infants. While opioid misuse impacts people across racial, ethnic, and socioeconomic groups, there is a specific need to address this issue in pregnant women and ensure appropriate substance use treatment.
About this Resource Treatment Manual
This Opioid Use Treatment Resource manual was developed in an effort to create awareness about available treatment resources for pregnant Illinois women who are insured by Medicaid and to illuminate geographical areas with gaps in services. The maps included in this manual depict the availability of services, including Medication Assisted Treatment (MAT) which includes both methadone and buprenorphine. The Illinois’ Department of Human Services Division of Substance Use Prevention and Recovery (IDHS/SUPR) considers pregnant women to be a priority population. IDHS/SUPR requires that providers maintain current policies and procedures that reflect the special needs of patients who are pregnant. Regulations require prenatal care and other gender specific services be provided either by the opioid treatment provider (OTP) or by referral to appropriate health care providers. IDHS/SUPR requires programs to accept pregnant patients without regard to their ability to pay or the size of the program’s existing population. This is a standard of practice in the field. IDHS/SUPR funds special programs for pregnant women and women with dependent children. These programs will treat families as a unit and admit children with their mothers as appropriate.

All resources accept Medicaid as a form of payment for services; when known, contracted accepted managed care organizations are indicated at the resource level. Substance Use Treatment Service Definitions
Methadone: Daily opioid agonist medication that can only be dispensed by a registered opioid treatment program; recommended by the American College of Obstetricians and Gynecologists as safe for use in pregnancy Buprenorphine: Daily opioid agonist medication that can only be prescribed by a medical provider who has completed training and obtained a waiver from the Drug Enforcement Administration (DEA); recommended by the American College of Obstetricians and Gynecologists as safe for use in pregnancy Outpatient: Level I. Non-residential substance abuse treatment consisting of face-to-face clinical services for adults or adolescents; frequency and intensity of such treatment shall depend on patient need but shall be a planned regimen of regularly scheduled sessions that average less than nine hours per week Intensive Outpatient/Partial Hospitalization: Level II. Non-residential substance abuse treatment consisting of face to face clinical services for adults or adolescents; frequency and

intensity of such treatment shall depend of patient need but shall be a planned regimen of scheduled sessions for a minimum of nine hours per week Inpatient Sub acute/Residential: Level III. Residential substance abuse treatment consisting of clinical services for adults or adolescents; frequency and intensity of such treatment shall depend of patient need but, shall, except in residential extended care, include a planned regimen of clinical services for a minimum of 25 hours per week; inpatient care, with the exception of residential extended care, shall require staff that are on duty and awake 24 hours a day, seven days per week, and that low intensity treatment services be offered at least five hours per week Medically Managed Intensive Inpatient: Level IV. Inpatient sub-acute residential substance abuse treatment for patients whose acute bio/medical/emotional/behavioral problems are severe enough to require primary medical and nursing care services; such services are for adults or adolescents and require 24 hours medically directed evaluation, care, and treatment and that a physician see the patient daily Recovery Home: Alcohol and drug free housing authorized by an intervention license issued by the IDHS whose rules, peer-led groups, staff activities and/or other structured operations are directed toward maintenance of sobriety for persons in early recovery from substance abuse, who recently have completed substance abuse treatment services, or who may still be receiving such treatment services at another licensed facility Residential Extended Care: Residential clinical services for adults or adolescents provided by professional staff in a 24 hour structured and supervised treatment environment; primarily designed to provide residents with a safe and stable living environment in order to develop sufficient recovery skills Integrated Prenatal Care: Systematic linkage of both prenatal care and substance use treatment services
