The driver no- ticed that the dogs had not been restrained. The driver made contact with the customer to re- quest that the dogs be restrained prior to him carry the loads in. Well done Singh Satvir ( Vic) CSA was travelling along Palmvale Rd when a ute towing a box trailer veered into the drivers lane causing a near miss. Driver slowed down to avoid a collision. Driver of ute appeared to be on the phone. Keeping a safe following distance of at least 4 seconds from the vehicle in front can avert a collision with other third party road users. Driver was able to identify the car driver was distracted , unlawfully using his phone whilst driving. Max Armstrong NSW took evasive ac- tion preventing an Incident. CSA spoke to customer and asked if it was safe to enter with the dog unrestrained. Custom- er advised CSA ,it was ok to enter ;as the dog did not bite. CSA entered premises and was immediately bit- ten by dog – minor bite mark on leg. Customer re- strained the dog and said, “the dog has been adopt- ed and she was unsure if the dog had come from un- fortunate circumstances”. Driver received Tetanus vaccine after his shift as a precaution. Near Miss Reporting– Injury avoided.

First Aid injury in QLD CSA arrived at customer address and noticed a dog

MOTOR VEHICLE REPORTS CSA was reversing out from a school when he noticed kids to the LHS of the truck, as he was keeping an eye on them, he has clipped a hand rail with the RHS rear of the Pantech. NSW 4/11 Driver was traveling up Parks Street Kirribilli NSW 2061 when a 3rd party pulled out from a parked position. Driver has moved over as the 3rd party nearly hit his truck. In doing so he has col-lided with a parked car with the front passenger side of the Pantech. It has pulled the front bar off and dented the quarter panel of the parked car NSW 6/11 Driver had reversed onto the dock and loaded all the totes into the vehicle. As the driver pulled the truck forward off the dock. The rear right pan door had caught a pole which pulled the door off the hinges. VIC 6/11

CSA delivering to a customer inside the house and has heard a loud noise. CSA has come out of the house and could see his vehicles mirror had been hit. There were two girls who saw what happen and said it was a male a silver ute. The CSA drove around for 20 minutes and found a ute that looked like the one the 2 girls had observed. The ute driver denied hitting the Online vehicle. Trucks mirror was replaced. The inci-dent is now in the hands of the insurance company. CSA now folds his mirrors in when parking. QLD 5/11 Driver was making a delivery to Pepper Court Wattle Glen. Whilst the

driver was making the delivery inside the customer's house, a truck drove past the station-ary Woolworth's vehicle, making contact with the right hand side mirror. VIC 19/11 As the CSA was driving when he approached a sharp curve on Mountain Creek Road. The road turned into an unsealed gravel dirt road, CSA slammed his brakes on, resulting in the vehicle losing traction. The vehicle veered off the road, hitting a fence before coming to a

complete stop. The airbags deployed VIC 26/11

You have certainly provided at lot of great information which we can use to make the workplace safer. We have commenced collating the data. We will provide more detail as to it’s results and improvement strategies in future newsletter issues. Here’s some of the more frequent response's

CSA delivering to units parked in the visitor car park.. He completed the delivery and was reversing out of the car park space. CSA was watching a car in the reversing camera. He did not notice the roof. He heard a bang and stopped to look at the damage. There was

damage to the customers roof, no damage to the vehicle. QLD 3/11 CSA had entered a private road, when inside the private road the CSA tried to reverse closer to the delivery point, in doing so he hasn't checked his surroundings and collected an old over-hanging phone line,.

CSA was sitting at traffic lights on North Terrace street Bankstown, Light turned green, driver then took off and hit the 2.4 low bridge at around 10kph CSA said he didn’t know where the height indicator of the truck was. Truck height indicator is located on the wind-screen. NSW 30/11

After numerous Hazard being reported at the Rosebud store in Victoria. House keeping im-

provements have been implement to allow our CSA to better service customers and improve

safety. Some of the identified Hazards include. Pallets in front of the loading dock bell and emp-

ty boxes and totes stored making it hard to pass.
