On your journey to recovery from substance use disorders, CHR
provides Real Hope and support as you transition into a new
life.
• The Oak Street Recovery House also provides transitional
housing for individuals who are waiting for a bed at a residential
treatment program.
• Clients not seeking a continued treatment program are expected
to be seeking out employment, identifying natural resources for
continued housing and treatment needs.
• Clients already on Methadone or Suboxone MAT maintenance
program are welcome.
• All clients in Oak Street will be tested for substance
use.
Oak Street Recovery House is a supervised transitional house for
individuals in recovery from substance use. With fi ve male beds
and fi ve female beds, individuals may stay at Oak Street for up to
90 days. No direct services are provided at Oak Street; individuals
are expected to be connected to case management and ambulatory
treatment services during their stay.
PROGRAM DESCRIPTION:
Oak Street is open 24 hours a day, 7 days per week, and 365 days
a year.
HOURS OF OPERATION:
Clients’ referring agent may call Oak St Recovery House during
regular business hours to set up a phone screening. Clients are
admitted 7 days per week. Individuals must have Eastern Region
Service Center case manager and be eligible/receiving State

STAFF:
CHR accepts referrals from parents, guardians, family members
and friends. We also take referrals from schools, hospitals,
private providers, primary care physicians, pediatricians,
community agencies and emergency service personnel, to name a few.
We also welcome self-referrals.
CHR accepts Medicaid, HUSKY Part A & Part B, Medicare and
most major health insurances as partial or full payment for
services.
