Southeast Michigan Health Information Exchange (SEMHIE)
• SEMHIE website is www.semhie.org
• MDMI is currently being used at SEMHIE in the Social Security Administration Contract
• Present Status of project� Infrastructure is in place
� MDMI Referent Index reversed engineered from scope of project
� MDMI map for SSA message (“Charlie Test”) built
� Next phase: Development of MDMI maps for Oakwood Hospital and Henry Ford Hospital
• After initial project, goal is to extend to other practitioners in SEMHIE. � “Town Center” - Develop an MDMI Map for their systems.

ONC Transition of Care Reference Implementation
