Otot-otot tersebut membesar hingga dapat mencapai 8 kali normal, lalu menekan nervus optikus. Degenerasi dari serat otot menyebabkan fibrosis, sehingga terjadi myopati restriktif dan diplopia.Infiltrasi sel inflamasi, yaitu limfosit, sel plasma, makrofag, dan sel mast dari jaringan intersisial, lemak orbital, dan kelenjar lakrimal dengan penumpukan glikosaminoglikan dan retensi cairan.
