Introduction
• Basic Science : last deciphered sense (Nobel Prize 2004)
• Complexity of a chemical sense ( continuous variable)
• Medical practice : anosmia impairs quality of life
• Economy: Smell adds value to food products and fragrances
• Hedonism: « I know I existed since I smelled » Casanova
• Metaphysical and religious: Anointing Oil in Old and New Testaments – Then the LORD said to Moses, "Take the following fine spices… liquid
myrrh, fragrant cinnamon, fragrant cane, cassia and a hin of olive oil… It will be the sacred anointing oil. Then use it to anoint the Tent of Meeting, the ark of the Testimony, … You shall consecrate them so they will be most holy, and whatever touches them will be holy ».
– Jesus Anointed at Bethany (John 12,1-11): « And the house was filled with the fragrance of the perfume. »

Physiologie
• La clé n ’est pas le
récepteur…dont il existe un
millier de types
• Le secret est dans la
connectique: un NOP
n ’exprime sans doute qu ’un
ou un petit nombre de R peu
spécifiques. Les NOP ayant
le même récepteur se
connectent au même
glomérule

Choice of odorants
• Olfactory : vanilla / H2S (rotten egg) / thymol (thym) / phenylethylalcool (PEA - flower)
• Trigeminal : CO2
• Mixed : menthol / acetic acid / lemon / anethol / NH3
Most of the odorants are detected by both olfactory and trigeminal systems. However trigeminal threshold is much higher than olfactory

Terminology
• Anosmia - Inability to detect odors
• Hyposmia - Decreased ability to detect odors
• Dysosmia - Distorted identification of smell
– Parosmia - Altered perception of smell in the presence
of an odor, usually unpleasant
– Phantosmia – Perception of smell without an odor
present
– Agnosia - Inability to classify or contrast odors,
although able to detect odors
Exploration: clinique
• Examen endoscopique à réaliser sans, puis avec
vaso-constricteurs
– vestibule et valve nasale
– cornets
– inflammation des méats
– état de la muqueuse / sensibilité
– fente olfactive
• Examens biologiques seulement en fonction du
contexte
Exploration: clinique
Interrogatoire :
– Type du trouble en sachant que phantosmie est
neurologique voire psychiatrique et cacosmie est
infectieuse
– Anamnèse: consécutif à un épisode viral?
– Antécédents de traumatisme / de chirurgie / médicaux
(diabète/thyroïde) / d ’irradiation / psychiatriques
– Exposition à des toxiques
– Cortico-sensibilité?
PATHOLOGY
Conductive defects
• Inflammatory
– allergy
– NARES / polyposis
– sarcoidosis...
• Tumors
• Iatrogenic
• Anatomical
Conductive defects
Chem Senses. 2007
Sensorineural defects
• Traumatic
– Stretching of the
olfactory nerves?
– Scarring in the
region of the
olfactory bulbs and
tracts
Sensorineural defects
• Degenerative disorders
– Early feature of Parkinson and Alzheimer
diseases • « In PD patients, but not in controls, cortical atrophy in
olfactory-related brain regions correlated specifically
with olfactory dysfunction » Wattendorf et al. J Neursci. 2009
– Tremor, motor neuron disease (MND),
Huntington's chorea (HC) and inherited
ataxia Hawkes C, Adv Otorhinolaryngol. 2006;63:133-51
Sensorineural defects
Congenital:
– May result from Kallman
syndrome
(hypogonadotrophic
hypogonadism)
– Hypotrophic or absent bulbs /
absent olfactory sulcus
– Genes: KAL1…
FGFR1,FGF8, PROK2,
PROKR2…
Sensorineural defects
Sensorineural defects
• Tumours
– Olfactory
neuroblastoma
– Olfactory groove
meningioma
Post-infectious olfactory loss
– 14 to 26% of anosmia cases
– 2d cause of sudden onset anosmia after
traumatisms
– Destruction of PON or lesion of olfactory
pathways
– Negative work-out
– Often hyposmia with failure of identification
Sensorineural defects
Should conductive smell disorder be
opposed to sensorineural
• Changes in olfactory bulb volume are
correlated with decrease in odor threshold
(Arch Otolaryngol Head Neck Surg. 2008)
• In the course of rhinosinusitis treatment, olfactory
bulb volume parallels changes in odour
thresholds! (The increase in olfactory bulb volume +8%
on the right and 19% on the left correlated significantly
with an increase in odour thresholds (P = 0.006, left side; P
= 0.03, right side), Brain 2009
Conclusions
• Remember that progessive smell disorder may be caused by systemic disease, neurologic disease or tumour
• Treatment remains challenging: conductive/sensorineural
• Hopes may come from: – theophylline
– ...
• At last I hope that you all find « an atmosphere that fosters intellectual rigor and creativity and at the same time is imbued with a spirit of warmth and collaboration » (Axel 2004) in your daily work and research!

Principe repris par le marketing
• Tests subjectifs

menthe, romarin, citronnelle, citron, eucalyptus,
myrte, cardamome, cyprès…
– Olfactifs purs: vanille, (thym), basilic
– Mixtes: menthe, citron voire ammoniac
Conclusions
• Aujourd ’hui: tests subjectifs. Demain
PEO ou IRM fonctionnelle?
• Exploration étiologique: en fonction du
contexte.
• Une dysosmie de perception
inexpliquée justifie de principe un
bilan d ’imagerie qui sera
heureusement le plus souvent normal
Conclusions
• Une dysosmie de perception évolutive peut
s ’intégrer dans une maladie générale
(diabète, hypothyroïdie) ou une maladie
neurologique qu ’elle peut révéler
• Espoirs thérapeutiques:
– théophylline
– caverine...
