The National Memorial Arboretum VisitThis is a special place that remembers those who have served, andcontinue to serve, our nation in many different ways.With some 50,000 trees already planted, and 200 dedicated memorialsestablished on the site, the Arboretum is a living tribute that will foreveracknowledge the personal sacrifices made by the armed and civil servicesof this country. Established in 1997, the site now hosts an abundance ofwild plants, woodland areas, grassland, a reed bed and wetland.It is populated by a variety of wildlife including brown hares, skylarks, lapwings, otters, tits and finches,green woodpeckers, buntings and an occasional black redstart.Our trip starts at the MILLENNIUM CHAPEL OF PEACE AND FORGIVENESS with a welcome talk in theChapel, an introduction to the National Memorial Arboretum, then the LAND TRAIN TOUR for an hourcovering also the outer lying points of the Arboretum, with a complete and informative commentary.We arrive back for a Traditional Afternoon Tea of a selection of freshly prepared sandwiches, a variety oflocally sourced pastries, cakes and biscuits and a choice of tea or coffee.20th June 2012 at 12.00 midday from Wellington Swimming Baths. Going on Holiday with Oxygen?It is possible to travel and go on holiday while dependant on oxygen. If you are going to a destination inEngland and Wales you do not necessarily need to go to the GP to arrange, you can order with AirProducts direct following the 7 steps below. It is beneficial to try and plan a holiday at least 2-4 weeks inadvance and always seek medical advice before you book and travel on holiday.Take details of your ‘home oxygen order’ with you and any accessories you may need.Step 1 Obtain permission from your GP or respiratory specialist to travel before you book your holiday.Step 2 Obtain permission for your holiday oxygen equipment to be delivered and stored at your holidayaccommodation.Step 3 (England & Wales) If you require the same oxygen equipment at your holiday destination asyou have at home, you can now contact our Healthcare Helpline directly without the need to request ahome oxygen order form (HOOF) from your GP or specialist team. Sr Jill Wilson’s Presentation onObstructive Sleep Apnoea Service at the PRH

WHAT IS OBSTRUCTIVE SLEEP APNOEA? (OSA)This is defined by a decrease in tone of the muscles supporting the airway during sleep, it may be caused egoby obesity or anatomical reasons. This results in repeated episodes of narrowing (hypopnoea) or closure(apnoea) of the pharyngeal airway during sleep. Hypopnoea is partial obstruction decreasing airflow andapnoea is complete obstruction stopping airflow (cessation of breathing.) Apnoeas can last for 10 seconds ormore & can be very frequent, even several hundred times a night. When apnoeas occur & breathing stops, thebrain reacts by briefly waking the person, causing the airways to re-open & breathing to restart.Diagnosis involves physical examination, medical history, over night sleep study and Epworth score.Some of the main symptoms, excessive daytime sleepiness, snoring, morning headaches, impaired alertness,choking & gasping during sleep and Restless sleepRisk factors, incidence & associated featuresRisk factors for developing obstructive sleep apnoea/hypopnoea syndrome (OSAHS) are increasing age,obesity and being-male. Use of alcohol & sedatives can also increase risk or severity of the condition.It is associated with high blood pressure which increases risk of cardiovascular disease and stroke.The condition is associated with increased risk of road traffic accidents.The patient's quality of life can be greatly affected sometimes leading to relationship break downs, loss of joband income, also the partner’s quality of sleep can be affected.TREATMENTThis is aimed to reduce daytime sleepiness by reducing the no. of episodes of apnoeas/hypopnoeas experi-enced during sleep.Treatment modes:• CPAP treatment• Lifestyle management - losing weight, stop smoking and/or decrease alcohol consumption & improvingsleep hygiene• Dental devices - designed to keep the upper airway open during sleep. These are traditionally viewed as atreatment option only for mild and moderate OSAHS• Surgery - involves resection of uvula (fleshy extension at back of soft palate that hangs above the throat)and redundant retrolingual (tongue base) soft tissue. However, there is a lack of evidence of clinicaleffectiveness and surgery is not routinely used in clinical practice• Tonsillectomy has a role in the management of some, usually younger, individuals with significantlyenlarged tonsilsNICE Treatment guidance March 08:CPAP is recommended for adults with moderate or severe symptomatic obstructive sleep apnoea/hypopnoeasyndrome (OSAHS). CPAP is only recommended as a treatment option for adults with mild OSAHS if theyhave symptoms that effect their quality of life & ability to go about their daily activities and lifestyle advice& any other relevant treatment options have been unsuccessful or are considered inappropriatePrincess Royal Hospital Sleep ServiceInitial reviews & diagnostic tests are undertaken both at Shrewsbury & Telford Hospital sites by theRespiratory Consultants. However ongoing review & management of the patients is provided by SleepService staff at the Princess Royal.The ongoing review & management of the patients is undertaken by the Respiratory team including Sr.JillWilson, Hospital Respiratory Nurse Specialist, Nick John, Respiratory Physiologist assisted by Cardio-respiratory technicians & the Community Respiratory Nurse team.What is a CPAP machine?It is a machine that plugs into an electric socket & generates airflow through an air tubing and mask into thepatient's airway. The positive pressure acts as an air splint to open the upper airways & thus prevents upperairway collapse. In patients with obstructive sleep apnoea they must use the machine every time they go tosleep for the treatment to be effective.Long term follow up of patients is critical to ensure adherence to treatment.Information taken from NICE Guidelines on OSAHS (2008), IMPRESS Service Specification for Investiga-tion & treatment of Obstructive Sleep Apnoea Syndrome (2009). Could you have OSA?In the UK it is estimated that over one million people may be suffering from Obstructive Sleep Apnoea(OSA), with prevalence being higher in men and those that are overweight.To date only 10% of sufferers are aware of their condition and are being treated to alleviate theirsymptoms. (BLF statement March 2011).See below for BLF’s awareness campaign, do you know anybody who could be suffering?Feel free to pass this newsletter on or mention BLF’s website for full details.

If you think that you, or someone you know, might have OSA,try taking these steps:Ask family and friends if you snore loudly and have breathing pauses, ortell someone if they do thisTake the Epworth Sleepiness Scale test on the BLF website.This will not tell you if you have OSA but, depending on your results, it canindicate that you should seek further medical advice.If this sounds like you visit your GP, ask for a referral to a sleep clinic.When you visit your GP, tell him or her about what happens when you areasleep and awake.When asleep - tell your GP if you snore loudly, stop breathing or haveepisodes of difficulty breathingWhen awake - mention all your daytime symptoms, such as daytimesleepiness, headaches, poor concentration, depressionIf you have done the Epworth Sleepiness Scale, you could take a copy withyou

Are you fit for Spring?As we finally approach the 21st of March & theofficial onset of spring I can’t help but wonderhow many of us are as fit as we possibly can beto attack & enjoy the (hopeful) sunnier daysahead. With this in mind I have hatched a planto get ‘fit’ for summer & hope that you’ll all joinme. We will all be fine as long as we rememberit’s not how quick we get there that matters butit’s the getting there that counts!Exercise 1: Begin by standing on a comfortablesurface where you have plenty of room at eachside. With a 5lb potato bag in each hand, ex-tend your arms straight out from your sides &hold them there as long as you can. Try to reacha full minute & then relax. Each day you’ll findyou can hold this position for just a bit longer.After a couple of weeks move up to 10lb potatobags.Then try 50lb potato bags & eventually try toget to the point where you can lift a 100lb bagin each hand holding your arms out for a fullminute. After you feel completely confidentwith this – put a potato in each bag!

Australia 2011 - Blue MountainsAbout 100 Km out of Sydney you come across the Great Divid-ing Range. The views are spectacular with hidden canyons andfantastic waterfalls. Just outside Katoonba in the famous rockformation, The Tree Sisters, where I climbed down. The climbback up was not so good but the Sulphur Crested Cockatoo’senjoyed the view. Len Sankey
