Weight loss surgery/ Bariatric surgery is a very exciting field which has revolutionized the process of weight loss. This surgery has not only given new lease of life to the patients who are severely obese but has also helped tremendously in controlling type-2 diabetes, hypertension, dyslipidemia, infertility and obstructive sleep apnea.

First description of weight loss surgery in 1960s by team of Dr. Mason and Dr. Ito. Last 25 years has seen renewed interest and popularity of weight loss surgery due to explosion in technology and wide spread use of laparoscopic weight loss surgery. Roux-en-y gastric bypass, one of the weight loss surgery was considered to be gold standard procedure for weight loss. Last 15 yrs has seen emergence of laparoscopic sleeve gastrectomy as a more popular choice as weight loss surgery all over the world.

Laparoscopic sleeve Gastrectomy is one of the most commonly performed bariatric procedure for Asian population around the globe. It is an irrervisble procedure in which approximately 60% of the stomach is removed, leaving a pouch of approximately 100cc in volume. Also, due to this procedure, constricrion of stomach size leads to elimination of hunger hormone grehlin after which you feel satisfied with smaller portion and lose weight.

Laparoscopic Sleeve GastrectomyLaparoscopic Gastric Bypass(LGB) orLaparoscopic Roux-en-Y Gastric Bypassis a complex and non-reversible procedure that helps severely obese patients to lose lot of excess weight. The American societies for Bariatric Surgery and the National Institute of Health have rated Roux-en-Y Gastric Bypass as the current gold standard procedure for weight loss surgery. In this procedure, a small stomach pouch (15 to 20 cc size) is created by stapling a section of the stomach so that the food directly enters into the lower portion of the jejunum, bypassing the beginning of the small intestine, hence reducing calories absorption. The remainder part of the stomach is stapled, not removed, and separated from the stomach pouch. This results in reduced appetite and satisfaction of hunger with smaller portion of diet. It is estimated that in post Laparoscopic Roux en y gastric bypass, one can lose 60-80% of excess weight in about 2 years time.

In this the Gastric pouch is like a long sleeve which is joined to the intestines, making a loop thereby finishing the procedure with only single anastomosis. It has a theoretical advantage over RYGB. The results of excess weight loss and resolution of co morbidities is similar to that of RYGB. This procedure is primarily a malabsorptive procedure as compared to the classical RYGB.

In this procedure the surgeons implant an inflatable silicone band around the upper portion of the stomach. The band creates a new, tiny pouch that limits and controls the amount of food consumed. The band also creates a small outlet that slows the emptying process into the stomach and the intestines allowing the patient to experience an earlier sensation of fullness and increased satisfied with smaller amounts of food. This ultimately results in weight loss.

Gastric Ballon is a non surgical procedure, in which a soft balloon is inserted into your stomach through your mouth, using an endoscope. The balloon is then filled with saline solution to partially fill your stomach, leaving less room for large amounts of food or drink.A gastric balloon is designed for a temporary weight loss solution, and will usually be removed after six months.

To qualify for surgery patients must have a Body Mass Index greater than 35 without associated co morbities (like diabetes, hypertension, sleep apnea, osteoarthritis) or more than 32.5 with associated co morbidities. However, patients who weigh less than this may be considered for surgery as per new IDF (International Diabetes Federation) guidelines for Asians.

Who are The Candidates for weight loss Surgery ?Most of the co morbidities can be greatly improved or, entirely resolved with successful weight loss. Most patients will lose about 75-80 % of their excess body weight with the gastric bypass procedure AND 70-80% with sleeve gastrostomy in 9-12 Months. You could start your office/desk work after 4 days of surgery. But if your job demands heavy lifting you may need to wait for at least a week, though different patients behave differently post operatively. After Surgery , when can resume work ?

Success Story

"I was suffering from blood pressure, sugar, gastritis, back pain and many other problems before surgery. My sugar level normally ranged between 260 to 300 for which I used to take insulin and oral medications, where then I decided to meet Dr. Deep Goel and his eam, Bariatric surgeon, he explained me about different types of bariatric procedures in detail. My surgery was done on October 2014 and I have already lost 24 Kg in three months and most importantly I am free from all the obesity related problem. The most happiest part of the surgery is that I am no longer taking medicines for diabetes."

"I had gotten to the point where I could no longer sit or walk for more than 10 minutes at a time. Diabetic, Hypertension, Depression, Anxiety, Obstructive Sleep Apnea and joint pain are a few of the conditions I was diagnosed with. I lost 42 Kg in 12 months time. The life threatening ailments are now gone!!! I not only lost the mental, physical and emotional, and spiritual weight on my life, I was transformed to a new way to live. Food is no longer my security blanket but through management and maintenance it's here to help me stay healthy and alive only."
