Defining Risk

• Ministry of Health had set a target:

– <10 cases (adverse event) per 1000 discharges

(data is pulled from the Discharge Abstract Database)

• Risk Stratification At PHC:

– >7 UTIs: 1000 patient discharge HIGH RISK

– 4-6 UTIs: 1000 patient discharge MEDIUM RISK

– <3 UTIs: 1000 patient discharge LOW RISK

Educational Interventions Based on Risk

• Low risk interventions include: monitoring data or

quick steps to serve as a reminder.

• Medium risk interventions include: simulation,

e-learning or tutorials to provide more detail.

• High risk interventions include: job aides, sneaker

support to educate and coach staff in the moment.
