    FOREWORDThisCodeofPracticeonsurveyanddraftingforminesurveyplansisanapprovedcodeofpracticeundersection274oftheWorkHealthandSafetyAct(theWHSAct).

    Anapprovedcodeofpracticeisapracticalguidetoachievingthestandardsofhealth,safetyandwelfarerequiredundertheWHSActandtheWorkHealthandSafetyRegulations(theWHSRegulations).

    Acodeofpracticeappliestoanyonewhohasadutyofcareinthecircumstancesdescribedinthecode.Inmostcases,followinganapprovedcodeofpracticewouldachievecompliancewiththehealthandsafetydutiesintheWHSAct,inrelationtothesubjectmatterofthecode.Likeregulations,codesofpracticedealwithparticularissuesanddonotcoverallhazardsorriskswhichmayarise.Thehealthandsafetydutiesrequiredutyholderstoconsiderallrisksassociatedwithwork,notonlythoseforwhichregulationsandcodesofpracticeexist.

    CodesofpracticeareadmissibleincourtproceedingsundertheWHSActandRegulations.Courtsmayregardacodeofpracticeasevidenceofwhatisknownaboutahazard,riskorcontrolandmayrelyonthecodeindeterminingwhatisreasonablypracticableinthecircumstancestowhichthecoderelates.

    Theremayberelatedjurisdictionalspecificlegislatedrequirementsforsurveyinganddraftingplans.Personsshouldinvestigateandidentifythoserequirementsintheirjurisdictionandcomplywiththem,alongsidetheWHSlegislativerequirementsandguidanceprovidedinthisCode.

    CompliancewiththeWHSActandRegulationsmaybeachievedbyfollowinganothermethod,suchasatechnicaloranindustrystandard,ifitprovidesanequivalentorhigherstandardofworkhealthandsafetythanthecode.

    Aninspectormayrefertoanapprovedcodeofpracticewhenissuinganimprovementorprohibitionnotice.

    ThisCodehasbeendevelopedbySafeWorkAustraliainconjunctionwiththeNationalMineSafetyFrameworkSteeringGroupasamodelcodeofpracticeundertheCouncilofAustralianGovernmentsInterGovernmentalAgreementforRegulatoryandOperationalReforminOccupationalHealthandSafetyforadoptionbytheCommonwealth,stateandterritorygovernments.

    AdraftofthisCodewasreleasedforpublicconsultationon[tobecompleted]andwasendorsedbytheSelectCouncilonWorkplaceRelationson[tobecompleted].

    SCOPEANDAPPLICATIONThisCodeappliestosurveyinganddraftingforthepreparationandreviewofaminesurveyplan(s)andkeepingacopyforinspectionundertheWHSRegulations.ItprovidesguidanceformineoperatorsonhowtoapplytheWHSRegulationsbycoveringthehardcopy,digitalthemes,minespatialinformationandstorageoftheminesurveyplan.Italsoprovidesguidanceonthepreparationofotherplans,whichmaybeusedtopreparetheMineSurveyPlan,byaccessingtheminesstoredsurveydata.

    TherequirementsoftheWHSRegulationsforminesurveyplansareintendedtohelpensurethesafetyoftheminebyaccuratelyidentifyingthelocationandfeaturesofminingoperationsandinfrastructure.OperationalsurveyinganddraftingunrelatedtotherequirementsoftheWHSActandRegulationisnotcoveredbythisCode,forexampleengineeringplanspreparedforproductionandrecoverytocalculatestockpileandexcavationvolumes.

    AsstatedintheForeword,theCodedoesnotextendtojurisdictionalspecificlegislativerequirementsforthepreparationofminesurveyplans.Mineoperatorsandcompetentpersonsshouldinvestigatewhetherthereanyfurtherlegislativerequirementsandguidanceintheirjurisdiction.

    WhoshouldusethisCode?

    TheprincipalusersofthisCodearemineoperatorsandcompetentpersonsinvolvedinthepreparationofminesurveyplans.Themineoperatormayusethiscodetoassistinensuringthattheplansdevelopedbyemployedorcontractsurveyorsmaymeetthemineoperatorslegislativeduty,assetoutinclause675O.

    HowtousethisCodeofPractice

    ThisCodeincludesreferencestobothmandatoryandnonmandatoryactions.ThereferencestolegalrequirementscontainedintheWHSActandRegulations(highlightedintextboxesinthisCode)arenotexhaustiveandareincludedforcontextonly.

    Thewordsmust,requiresormandatoryindicatethatlegalrequirementsexistandmustbecompliedwith.Thewordshouldindicatesarecommendedcourseofaction,whilemayindicatesanoptionalcourseofaction.

    1 INTRODUCTIONThiscodeshouldbefollowedbymineoperatorsandcompetentpersonsinallmineswhenpreparingminesurveyplans,whichmayinvolvesurveyors,asrequiredinregulation675O(3)below:

    (3)Theplanmustclearlyshowthefollowing:

    (a)thesurveyofthemine,includingdisusedsurveyandboreholes;

    (b)thelocationofelectricalinstallations;

    (c)thelocationoftelephonesandotherfixedplantassociatedwiththeradioand

    telecommunicationssystems;

    (d)waterdamsandtailingsdams;

    (e)naturalfeaturessurroundingthemine;

    (f)placesforthestorageofhydrocarbonsorexplosives;

    (g)pointsofentryandexit,includingemergencyexits;

    (h)refuges(inanundergroundmine).

    Note

    Regulation634requiresthemineoperatorofanundergroundmine

    toprepareaplanoftheventilationsystemforthemine.

    AMineSurveyPlanmayconsistofoneormoresheetsshowingtheentiresurveyofamineinplanorsection.Eachsheetrepresentsapartorwholeofeachmininglevelorseamundergroundoranareaofopencutorquarryingminingoperationsshowninplan.

    ThisCodeshouldbeusedwhencompilingaMineWorkingsPlanforeachminingoperation,sothattheplansdatamaybeincludedintheMineSurveyPlan.Mineoperatorsarenotrequiredbyclause675O(3)toprepareaMineWorkingsPlan,butshouldfollowtheadviceintheCodeconcerningmineworkingssoastoshowitclearlyontheminesurveyplan.Theymayalsofinditofassistancewhencomplyingwithanylegislativerequirementsformineworkingplansintheirjurisdiction.TheMineWorkingsPlaninthisCodemayincludeanycrosssectionorlongitudinalsectionsheetsused.

    1.1Preparingminesurveyplans

    Whenpreparingminesurveyplans,themineoperatormustensurethecompetentpersonreferencesitaccordingtotheWHSRegulationsrequirements:

    APPENDIXJURISDICTIONALNOTES(MINES)

    675O1Ajurisdictionmayomitsubregulation:

    3Ajurisdictionmayexcludeaclassorclassesofminesfromsubregulation(2).

    Theminingoperationmayoperateonalocalsurveygrid/datum.Howeverallminesurveyplans,includingitsminespatialinformation,mustbecompiledontheMapGridofAustralia1994(MGA94),basedontheGeocentricDatumofAustralia1994(GDA94)values,unlessotherwiseallowedforinajurisdictionbyclause675O(3).

    Classesofminesinjurisdictionsexcludedfromtherequirementsoftheaboveclause675O(2)shouldsurveyofftheirlocalgridandprovidetransformationdetails(orsimilar)ontheminesurveyplan.

    Arangeofminesurveyingmethodologiesmaybeusedandreferencedtotheabovelegislatedgridsanddatums.Thesemethodologiescaninvolveremotesensingtechnologiessuchas:

    photogeometrywithGPScoordinates airbornesensors,whichcaninvolve:

    aerialphotographyandphotogrammetricmapping

    aeriallaserscanningandterrestriallaserscanning

    orthoaerialphotography

    Whateverthemethodologyused,itshouldachieveafinalaccuracywithin2metresinordertoprepareanaccurateminesurveyplan.Mineoperatorsshoulddeterminebyriskassessmentthespecificlevelofaccuracytheyrequirefortheirsurveyinganddrafting,havingregardforpublicationssuchastheICSMSP1(seesection2.3ControlsurveysandSubsidiarySurveysbelowformoredetails).

    Theminesurveyplanshouldbesectionedintosheetsthatbestfitthemine.Indeterminingthebestfitforthesheetlayoutthecompetentpersonshouldkeepinmindtheneedtoprovideforexpansionofthemineandadjoiningminessurveyplans.

    Whereoldsurveysexist,thecompetentperson(s)preparingandmarkingtheminesurveyplanshouldassumetheyconstituteahazardunlessprovenotherwise.Allplansshouldbereferencedwithcautionuntiltheiraccuracyhasbeenverifiedandvalidated.TheWHSRegulationsrequiresthemineoperatortoobtainhistoricalminesurveysoftheminetocarryoutthisverificationandvalidation:

    1.2Symbols

    Thetechnicalsymbols,signconventionsanddefinitionsforstrataandrocktypesshownontheminesurveyplansandassociatedminespatialinformationshouldbeinaccordancewiththe:

    AustralianStandardforMinePlansPreparationandSymbols(AS4368),and AustralianStandardforGeologicalSymbols(AS2916).

    IfasymbolisnotprovidedforinAS4368orAS2916thecompetentpersonshouldcreateasuitablesymboltobeshowninthelegendontheminesurveyplan.

    1.3Dutiesofamineoperator

    TheWHSRegulationsrequiremineoperatorstoensurethatadetailedsurveyplanofthemineispreparedbyacompetentpersonandkeptatthemine.ThisCodesetsoutanumberofwaysinwhichmineoperatorsmaydothis,includingengagingpersonsasrequiredunderWHSRegulationclause675O:

    1.4Functionsofacompetentperson

    Thefunctionsofthecompetentpersonshouldincludethefollowing.

    Toprepareandreviewminesurveyplansandbringtotheattentionofthemineoperator:o anyvariationintheminesurveyingfromtheminesurveyplanofwhichthe

    competentpersonbecomesaware,or

    o anyinterferencewithorobstructiontothecompetentpersonsperformanceorfunctions,or

    o anydoubtabouttheaccuracyofanyplans,oro conversionofanyplantotheMapGridofAustralia1994(MGA94)

    1.5Inspectionofminesurveyplans

    TheWHSRegulationsrequirethemineoperatortokeepminesurveyplansandmakethemavailableforinspectionundertheWHSAct:

    (1)ThemineoperatorofaminemustkeepthecurrentminesurveyplanandallpreviousversionsoftheplanavailableforinspectionundertheAct.

    toworkersatthemine.

    Plansshouldbekeptinaformatsuitablefortheminesotheycanbeviewedandreadilyaccessibletosatisfytheabovelegislativerequirements.Forsomeminesthemostappropriateformmaybeonpaper,whileothersitesmayfindanelectronicformatmoreappropriate.Previousversionsofretainedplansmayhavebeenpreparedunderdifferentgridsystems.Personsinspectingthemshouldbemadeawareofthisbydetailsshownontheplanand/orthroughtherecordssystem.Formoreguidanceonstorageseesection2.5belowSurveyrecordsandsupplyofsurveyinformation.

    2 SURVEYSTANDARDSANDPROCEDURES

    2.1OriginofCoordinates

    Allsurveysshouldoriginatefromtheminebaseline,oranyothermarkincludedinthestatesurveycontrolnetworkthathasastandardofaccuracyconsistentwiththeminebaseline.

    ThesurveyoftheminebaselineshouldbeplannedandsurveyedtoClassBstandardsofaccuracyasdefinedinICSMSP1.

    Note:foranexistingmine,thecurrentbaselinemarksmayberetained,providedtheyareofadurablenature.

    2.2UndergroundBaseline

    Eachundergroundminemustestablishundergroundbaselines,insuitablepositionsintheundergroundsurveyandaslongaspracticable,soastoprepareaccurateanddetailedminesurveyplans.Theterminalmarksshouldbestableanddurable.Undergroundbaselinedetailsshouldberecordedonaminesurveyplan.

    2.3ControlSurveysandSubsidiarySurveys

    Wherenoprescribedaccuracyisdefinedinajurisdiction,eachControlSurveyshouldachieveastandardofaccuracyasprescribedinICSM,SP1toClassDorbetter.WhencalculatingcompliancetoICSMSP1ClassDusingtheformular=c(d+0.2)thisdirectionspecificallydefinesdasthefollowing:d=distancetoanystationinkm,withaminimumvalueof1(km).

    Eachcontrolsurveystationshouldbeadequatelyreferencedandsubstantiallymarked.Asfaraspracticablethemarksshouldbeplacedinapositionleastlikelytobedisturbed.

    Wheredifferentiallevellingisusedtoestablishtheheightofacontrolmark,ICSMSP1ClassLDshouldbeused.

    Subsidiarysurveysshouldbeemployedbythecompetentpersonwherenecessarytowithinafinalaccuracyof2metresofposition.ThisaccuracymaybeachievedusingremotesensingtechniquesandGlobalPositioningSystems(GPS).

    2.4CorrelationofSurfaceandUndergroundSurveys

    CorrelationbetweensurfaceandundergroundsurveysshouldbeconsistentwithaClassDsurveyasprescribedinICSMSP1,unlessotherwiseprovidedforinjurisdictionalspecificlegislation.

    Incorrelationofsurfaceandundergroundsurveys,wheremethodsotherthandirecttraverseareemployedforazimuthorcoordinatetransfer,thesurveysshouldbeshownonaplanseparatefromtheminesurveyplanandshoulddisclosethespecialsurveymethodsemployed.Theplanshouldberetainedattheoperationandthecompetentpersonshouldcertifythatthesurveyshownontheplanisaccurate.

    2.5SurveyRecordsandSupplyofSurveyInformation

    WHSRegulations

    675OSurveyplanofminemustbeprepared(4)Themineoperatorofaminemusttakeallreasonablestepstoobtainhistoricalminesurveysoftheminetoensuretheaccuracyoftheminesurveyplan.675QSurveyplantobeavailableforinspection(1)ThemineoperatorofaminemustkeepthecurrentminesurveyplanandallpreviousversionsoftheplanavailableforinspectionundertheAct.(2)Themineoperatorofaminemustmakethecurrentsurveyplanreadilyaccessibleonrequesttoworkersatthemine.

    Themineoperatormustensurethesafepreservationofallminesurveyrecordssoastosatisfytheregulationrequirementssetoutabovetoobtainhistoricalminesurveysandmakepreviousversionsofminesurveyplansavailable.Recordsshouldbekeptatthesurveyoffice,orothersuitablestoragefacility.

    Minesurveyrecordsshouldbemaintainedeithermanually(inafieldbookorotherstablematerial)orelectronically.Whereamachineorotherdeviceisrequiredtoaccessthestoreddatathemineoperatorshouldensurethedataisregularlyupdatedtoamediaandformatthatiscurrentlyavailable.

    Minesurveyrecordsshouldbepermanentlyrecordedandmaintainedsotheymaybeutilisedforfuturesurveys.

    Wheresurveysarerecordedinelectronicformtheinformationrecordedshouldbeconsistentwiththatrequiredforsurveybooks.

    Allsurveybooksshouldbemaintainedingoodorderandshouldhavethefollowingdescriptionclearlyandpermanentlymarkedonthecoverandinsidetitlepage:

    thenameofthemine, forundergroundmines,thelevelnameandcataloguenumbertowhichthebookrefers,

    and

    consecutiveindexnumber.

    Thefollowingproceduresshouldbeadoptedforentriesintosurveybooks:

    allsurveyobservationsandmeasurementsshouldberecordedatthetimeofsurvey; whenamistakeneedstobealteredthereshouldbenoerasure,theerroneousentry

    shouldbestruckthroughandthecorrectionwrittenabove

    thedatumlineofthesurveyandtheazimuthadoptedshouldbeclearlyindicated lengthsshouldbeenteredatthetimetheyaremeasuredandwherenecessarycorrections

    shouldbeclearlyindicated

    2.6Beforepartsofaminebecomesinaccessible

    Beforeanypartofaminebecomesinaccessible,thepositionofthemineworkingsshouldbeestablishedfromacontrolorsubsidiarysurveybycompetentpersons.

    Wheremineworkingsareunabletobesurveyed,thecompetentpersonshouldtakeallreasonablypracticablestepstoassistthemineoperatorinthecompletionoftheminesurveyplan.

    2.7SurfaceMovementandSubsidence

    Surveysshouldbeundertakentorecordsurfacemovement,includingsubsidenceinducedbymining.

    Thesesurveysshouldbecarriedoutunderthesupervisionof,andcertifiedby,thecompetentperson.

    Acopyofallsurfacemovementand/orsubsidencesurveydata,includingfieldnotes,shouldberetainedbythemineoperator.

    2.8SurveyofBoreholes

    Boreholesshouldbeidentifiedbyuniquenameornumberandshouldbeshownontheminesurveyplan.Collarand/orcommencementlocationsofboreholes,shouldbeestablishedfromacontrolorsubsidiarysurvey.Thefollowinginformationshouldbeutilised,whenavailable,fromdirectsurveyorothersourcetodefinethepositionofaboreholeandsuitablyendorsed(forexampleadrillerslogorgeophysicalsurvey):

    totallength(ordepth); inclination(ordeclination);and planprojection(bearingorazimuth)whenotherthannearvertical.

    3. MINESURVEYPLAN

    3.1PreparingtheMineSurveyPlan

    Theminesurveyplanmustbepreparedby,orunderthesupervisionof,thecompetentpersonasrequiredbyWHSRegulation675O(1).Inaddition,themineoperatormayberequiredbyjurisdictionalspecificlegislationtosubmitcopiesofthePlantotheRegulatoratcertainspecifiedtimes.

    Inpreparingtheminesurveyplan,thecompetentpersonshouldcertifythattheyhavepreparedittosatisfythelegislativerequirements.See3.8Certificationbelowformoredetails.

    Inthecaseofanundergroundmine,theminesurveyplanmustcomprisesheetsforeachseamorLevelalongwithotherlayoutdataforthewholeofthemine,assetoutinthisCode.

    Inthecaseofanopencutmine,theminesurveyplanmustbecompiledontheOpenCutVoidSheetsalongwithotherlayoutdataforthewholeofthemine,assetoutinthisCode.

    TheLevelSheets,SeamSheetsandVoidSheetsmustbecompiledatascalethatbestreflectstheminesurveywithsufficientclarityandnumberofsheetstocovertheextentofthesurveyandnecessaryadjacentsurvey.

    Whennewsheetsoftheminesurveyplanarebeingpreparedandanypartofthepreexistingmineiscompleted,anoutlineofthepreexistingmineworkingsmaybedrawnandthenewplansheetendorsed,withreferencetoanypreviouslypreparedminesurveyplansfordetail.

    Shouldthepreexistingmineworkingsbecomeactive,orusedforaccessinanareapreviouslyshowninoutline,thepreexistingmineworkingsmustbetransferredinfulltotheminesurveyplan.

    3.2UpdatingtheMineSurveyPlan

    Exceptwherethemineisconsideredsuspendedorclosed,theWHSRegulationsrequiretheMineSurveyPlantobeupdated,assetoutintheWHSRegulationChapter10Miningclause675Pbelow:

    4 CLOSINGPLANS

    4.1Suspensionofoperations

    Whereaminehassuspendedoperations,themineoperatorshouldhavetheminesurveyplanchartedtothedateofsuspension.

    4.2ChartingforSuspension

    Theseproceduresshouldbefollowedinthecaseofthesuspensionofamine:

    5.2. EndorsingcombinedplansIfacombinedplanisprepared,itshouldbecertifiedandendorsedbyacompetentpersonauthorisedbythemineoperatortodosoontheirbehalf.Insomejurisdictions,theMineManager(orotherstatutoryofficial)mayberequiredbylegislationtoalsocertifytheplan.

    5.3. SubsidenceManagementPlansTheseplansmayberequiredbyminingleaseconditionsandarepreparedinaccordancewiththeregulatorybodyguidelinesorrequirementsinthejurisdiction.PlanspreparedunderthejurisdictionspecificguidelinesshouldalsobepreparedtobecompatiblewiththestandardsrecommendedbythisCode.Allsuchplansshouldbecertifiedbythecompetentpersonandsuitablyendorsed.Insomejurisdictions,theMineManager(orotherstatutoryofficial)mayberequiredbylegislationtoalsocertifytheplan.

    (1) Themineoperatorofanundergroundminemustensurethataplanoftheventilationsystemforthemineisprepared.

    (2) Theventilationplanmustshow:

    (a) thedirection,courseandvolumeofaircurrents;and

    (b) thepositionofallairdoors,stoppings,fans,regulatorsandotherventilatingplantandstructuresatthemine.

    (3) ThemineoperatormustensurethatacopyoftheventilationplanisavailableforinspectionundertheAct.

    Theventilationplanshouldbecompiledtomeettheselegislatedrequirements.

    Theplanshouldshowpositionsinwhichpipemains,hydrants,isolationvalves,firesubstationsandfiredepotsaresituated.Thisplanshouldalsoshowthepositionofventilationwalls/stoppings,trapdoors,regulators,seals,preparedsealsites,overcasts,aircrossings,ventilationdoors,beltconveyors,mainelectricsupplycables,fixedelectricalapparatus,highvoltagecablesandswitchgear,telephonesandthedirectionofventilationintheroadways.

    Theplanshouldbereviewedand,ifnecessary,updatedatnogreaterthanthreemonthlyintervalsandpossiblymorefrequentlydependingontheminingoperations.

    5.6. EmergencyEscapeandRescuePlan(undergroundoperations)Theescapeandrescuesurveyplanshouldbecompiledgenerallyatascalethatbestreflectstheminesurveyandescapeandrescueappliances.Theplanshouldincludeallsurveyoftheminetoenabletheselfescapeand/orrescueofpersonnelfromanywhereinthemine.

    Thisplanshouldshowmainandaccessroads,themeansofexitingfromeachpartoftheminetothesurface,allselfescapesystems,refugechambers,firstaidstations,telephonestations,beltconveyors,anygasdrainagemainsandbranches,knownfalls,accumulationsofwater,stowageidentifiedasnonpassable.

    Thisplanshouldalsoshowanyinformationfromthefirefightingplanandtheventilationplanoranyadditionalinformationidentifiedasnecessaryforthedealingwithanemergencyattheoperation.

    5.7. SurfacePlanThesurfaceplanshouldbekeptatthemine.Thesurfaceplanshouldbeofascaletobestrepresentthesurfacefeaturesofthemineandshouldcovertheareaswhereminingoperationshavebeenorarebeingcarriedoutandincludenonminingactivities.

    Theplanmaybemaintainedasspatialinformation.Topographymaybeshownifitaddstotheinterpretationoftheplan.

    Inthecaseofanundergroundmine,thesurfaceplanshouldshowallstreets,roads,reservoirs,swamps,waterbodies,unconsolidatedsurfacedeposits,railways,mainpipelinesandanyotherfeaturewhetherofthesameorofadifferentkindwhich,ifdisturbedbyminingoperations,islikelytocauseriskstothesafetyofthemine.
