The Drug S y n t h e s i s and Chemistry Branch (DS&CB), o f t h e Developmental T h e r a p e u t i c s Program (DTP), D i v i s i o n o f Cancer Treatment (DCT), N a t i o n a l Cancer I n s t i t u t e ( N C I ) , is seeking c o n t r a c t o r s w i t h e x p e r t i s e i n chemical s y n t h e s i s and drug d e s i g n , t o s y n t h e s i z e a v a r i e t y o f compounds f o r e v a l u a t i o n as p o t e n t i a l anti-AIDS a g e n t s . The a s s i g n e d o b j e c t i v e s o f t h i s p r o j e c t are t o d e s i g n and s y n t h e s i z e t h e f o l l o w i n g : ( a ) Congeners o f l e a d compounds having confirmed a c t i v i t y , t o enhance a c t i v i t y o r potency; ( b ) Prodrugs w i t h s t r u c t u r a l m o d i f i c a t i o n s t h a t may provide a l t e r e d pharmacokine t ics , a l t e r e d drug t r a n s p o r t , improved b i o - a v a i l a b i l i t y th rough i n c r e a s e d water s o l u b i l i t y , o r i n c r e a s e d chemical s t a b i l i t y ; ( c ) Other a l t e r e d s t r u c t u r e s t h a t p o s s e s s elements of b o t h congener and prodrug; and ( d ) Compounds r e l a t e d t o n a t u r a l p r o d u c t s , e . g . , a l k a l o i d s , h e t e r o c y c l e s , n u c l e o s i d e s , p e p t i d e s , e t c . Each c o n t r a c t o r should have a v a i l a b l e a f u l l y o p e r a t i o n a l f a c i l i t y , i n c l u d i n g a l l n e c e s s a r y equipment and i n s t r u m e n t a t i o n for a l l a s p e c t s of t h e con t r ac t . The s u c c e s s f u l o f f e r o r w i l l b e expec ted t o s y n t h e t i c a l l y modify such commercially c o n f i d e n t i a l ( d i s c r e e t ) materials. Thus, pharmaceut ica l o r chemical companies could o b t a i n v a l u a b l e d a t a on new l e a d compounds. Therefore , i n o r d e r t o honor t h e c o n f i d e n t i a l i t y agreement w i t h t h e o r i g i n a l s u p p l i e r , t h e N C I b e l i e v e s t h a t t h e compounds cannot be s e n t t o p o t e n t i a l c o m p e t i t o r s o f t h e s u p p l i e r , and t h u s pharmaceut ica l and chemical compounds must b e excluded from t h e c o m p e t i t i o n . For purposes o f t h i s r e s t r i c t i o n , a pharmaceut ica l o r chemical company is d e f i n e d as an o r g a n i z a t i o n which sells d r u g s and chemica ls t o t h e g e n e r a l p u b l i c f o r p r o f i t .

T h i s is a r e c o m p e t i t i o n o f c o n t r a c t s c u r r e n t l y h e l d by t h e U n i v e r s i t y o f Alabama, Georgia Tech Research Corp. The Human Learning and Behavior Branch (HLB) of t h e Center f o r Research �or Mothers and C h i l d r e n (CRMC), N a t i o n a l I n s t i t u t e o f Chi ld Heal th and Human Development ( N I C H D ) , i n v i t e s g r a n t a p p l i c a t i o n s f o r r e s e a r c h t o develop measures which are proxy o r s u b s t i t u t e s f o r s e r i o u s , p o t e n t i a l l y f a t a l , o r handicapping i n j u r i e s i n c h i l d r e n and youth . These measures are needed t o develop and e v a l u a t e i n t e r v e n t i o n s f o r s e r i o u s i n j u r i e s wi thout depending on t h e a c t u a l occur rence o f t h e s e i n j u r i e s . Such measures used i n l i e u o f f a t a l o r handicapping i n j u r i e s are c o n s i d e r e d proxy measures f o r s e r i o u s i n j u r i e s .

I n t h e United S t a t e s , i n j u r i e s are t h e l e a d i n g cause o f d e a t h and d i s a b i l i t y among c h i l d r e n and young a d u l t s . They account f o r about h a l f o f a11 d e a t h s i n c h i l d r e n under 15 y e a r s and almost 8 0 p e r c e n t o f d e a t h s i n t h e 15 t o 24 age group. C l e a r l y , i n j u r y is one o f t h e most impor tan t c h a l l e n g e s f a c i n g t h e h e a l t h and s c i e n t i f i c community i n reducing m o r t a l i t y and s u f f e r i n g i n ch i ldhood. However d e a t h and d i s a b i l i t y due t o i n j u r y is s t i l l a r e l a t i v e l y uncommon e v e n t . S t u d i e s aimed a t development and e v a l u a t i o n of i n t e r v e n t i o n s t r a t e g i e s are compl ica ted by t h e l a r g e samples r e q u i r e d and dependence on t r a g i c e v e n t s as outcome measures. Therefore , measures are needed which p r e d i c t t h e p r o b a b i l i t y o f s e r i o u s i n j u r y more a c c u r a t e l y t h a n e x i s t i n g measures and can be used as dependent measures wi thout depending on a c t u a l occur rence of t h e s e i n j u r e s . Such measures must be o b s e r v a b l e and measurable . They may c o n s i s t o f sociodemographic, b e h a v i o r a l , p h y s i o l o g i c a l o r even g e n e t i c v a r i a b l e s o r combinat ions o f such v a r i a b l e s . They could comprise a set of v a r i a b l e s which d e s c r i b e s t h e p r o c e s s f o r s e r i o u s i n j u r i e s as a model

The major i n j u r i e s exper ienced by c h i l d r e n va ry by age , l o c a t i o n , and o t h e r f a c t o r s . The re fo re , no one measure o r s e t o f measures is l i k e l y t o p r e d i c t a l l i n j u r i e s o f c h i l d r e n . However, t h i s r e s e a r c h must focus on a t l eas t one major t y p e o r cause o f i n j u r y f o r a t l ea s t one age group and should apply o r g e n e r a l i z e t o more t h a n one i n j u r y o r age group.

It is a n t i c i p a t e d t h a t t h e development and v a l i d a t i o n o f such measures may llI(L.

r e q u i r e t h e u s e o f l a r g e p o p u l a t i o n s o r d a t a b a s e s w i t h s u f f i c i e n t i n fo rma t ion r e g a r d i n g ch i ldhood i n j u r i e s . However, development o f proxy measures f o r s e r i o u s i n j u r y would a l s o r e q u i r e l o n g i t u d i n a l follow-up and a d d i t i o n a l i n fo rma t ion t h a n is c u r r e n t l y a v a i l a b l e w i t h c r o s s - s e c t i o n a l samples . Linkages between e x i s t i n g d a t a b a s e s may prove u s e f u l .

The product o f t h i s r e s e a r c h should be a measure o r s e t o f measures which would p r e d i c t t h e p r o b a b i l i t y o f s e r i o u s i n j u r y wi th g r e a t e r accuracy t h a n anyo t h e r c u r r e n t l y e x i s t i n g measure. The proxy measure(s1 must a l s o be s u f f i c i e n t l y a c c u r a t e so t h a t w i th e v a l u a t i o n o f i n t e r v e n t i o n s , changes i n t h e measure w i l l r e f lec t changes i n t h e a c t u a l rates o f s e r i o u s i n j u r y wi th s u f f i c i e n t power t o be used f o r t h i s purpose . The f o c u s o f t h i s announcement i s t o develop measures which are proxy f o r s e r i o u s i n j u r i e s t o c h i l d r e n . Based upon unde r s t and ing ga ined from t h i s and o t h e r r e s e a r c h , i n t e r v e n t i o n s can be developed and e v a l u a t e d as p a r t o f f u t u r e planned funding i n i t i a t i v e s .

The PHS u r g e s a p p l i c a n t s f o r g r a n t s t o g i v e added a t t e n t i o n (where f e a s i b l e and a p p r o p r i a t e ) t o t h e i n c l u s i o n o f m i n o r i t i e s and females i n s t u d y p o p u l a t i o n s f o r research i n t h e behavioral and soc ia l s c i e n c e s . If m i n o r i t i e s and females are not inc luded i n a g iven s tudy , a clear r a t i o n a l e f o r t h e i r e x c l u s i o n should be p rov ided . I n v e s t i g a t o r s are reminded t h a t merely i n c l u d i n g a r b i t r a r y numbers o f m i n o r i t y group and females p a r t i c i p a n t s i n a g iven s tudy is i n s u f f i c i e n t t o gua ran tee g e n e r a l i z a t i o n o f r e s u l t s .
