A how-to guide for businesses and organizations interested in
utilizing Excelsior Pass and/or Excelsior Pass Plus.
For businesses and organizations based in New York… The information
provided in this guide may be adapted to comply with New York State
guidelines. Please note that use of Excelsior Pass and/or Excelsior
Pass Plus does not replace other New York State guidelines.

Excelsior Pass Plus is a secure, digital copy of COVID-19
vaccination record or negative test results. The inclusion of
health information enables individuals to have a verifiable record
of COVID-19 vaccination or negative test results at their
fingertips – and includes the same information you would find on a
CDC Vaccination Card or paper laboratory test results.
New Yorkers can display their Excelsior Pass Plus around the world
at hundreds of businesses and locations that require proof of
vaccination, as well as when traveling to entities where SMART
Health Cards are accepted.

*New York State residents who received a COVID-19
vaccine—authorized by the Food and Drug Administration (FDA) in the
United States—outside of the State of New York may retrieve a Pass
by contacting their New York State healthcare provider. **Excelsior
Pass Plus is built using SMART Health Cards Framework and
specifications, developed in partnership with VCI. VCI is a
voluntary coalition of 570 public and private organizations
committed to empowering individuals’ access to a trustworthy and
verifiable copy of their vaccination records or laboratory results
in digital or paper form using open, interoperable standards.

HOW IT WORKS

Communicate with the individuals you serve about the option for
quick entry using Excelsior Pass and/or Excelsior Pass Plus, so
they’re prepared to have their Pass scanned.
All individuals using Excelsior Pass and/or Excelsior Pass Plus
should come prepared with either a digital or a physical Excelsior
Pass and/or Excelsior Pass Plus printed upon entry.
STEP 2 Inform

Complement other safety measures by using the NYS Excelsior Pass
Scanner app to scan QR codes upon entry.
Verify identification for individuals aged 18+ with a photo ID that
includes the individual’s name and date of birth.
If individuals choose not to use Excelsior Pass or Excelsior Pass
Plus, they may provide alternate proof of COVID-19 vaccination or
test results.

Review health and safety protocols and ensure your employees and
any staff responsible for scanning have access to mobile devices or
tablets to scan an Excelsior Pass and/or Excelsior Pass Plus
through the NYS Excelsior Pass Scanner app.
Train your employees to use the NYS Excelsior Pass Scanner app to
scan and verify each Excelsior Pass and/or Excelsior Pass
Plus.
PrepareSTEP 1
Training • Train your employees to use the NYS Excelsior Pass
Scanner app, verify each scanned Excelsior Pass
and/or Excelsior Pass Plus for adults aged 18+ with photo IDs, and
accept alternative forms of verification. You can refer to the
online, dedicated Excelsior Pass FAQs and Excelsior Pass Plus FAQs
for more information.
• You should be prepared to enforce all NY State Department of
Health (DOH) and organization protocols for individuals with a
valid or invalid Excelsior Pass and/or Excelsior Pass Plus, and
those that need to verify their COVID-19 vaccination or test
results without the digital tool.
Operations • Include a way to scan an individual’s Excelsior Pass
and/or Excelsior Pass Plus at your check-in station,
front desk, and/or other points of entry using an iOS or Android
mobile device or tablet.
• Develop protocols for individuals who have an invalid Excelsior
Pass and/or Excelsior Pass Plus, expired Excelsior Pass, and others
who need to verify their COVID-19 vaccination or test results
without using the platform.
Technology • To utilize the NYS Excelsior Pass Scanner app, your
employees will need access to mobile devices or tablets with
a
camera to scan Excelsior Pass and/or Excelsior Pass Plus. You can
refer to the Excelsior Pass website for more details on how this
app works.
• Each NYS Excelsior Pass Scanner app must be downloaded separately
onto a mobile device or tablet and requires completion of a brief
form; this includes providing your business or organization’s name,
selecting the relevant industry type, and providing the zip code
where each Excelsior Pass and/or Excelsior Pass Plus will be
validated. Upon completing the form, you can begin using the NYS
Excelsior Pass Scanner app.

Messaging It is important to stress the accessibility, security,
and privacy components of Excelsior Pass and Excelsior Pass Plus in
your messaging:
• Participation in Excelsior Pass and/or Excelsior Pass Plus is
voluntary and designed to be inclusive, regardless of access to a
smartphone. Individuals can easily print an Excelsior Pass and/or
Excelsior Pass Plus via the Excelsior Pass Web Portal. Individuals
can also present an alternate proof of COVID-19 vaccination or
negative test results, like a paper form.
• Excelsior Pass and Excelsior Pass Plus have gone through thorough
accessibility testing to ensure they are easy to use for those who
may have hearing or vision impairments.
• When using the NYS Excelsior Pass Scanner app to scan an
Excelsior Pass and/or Excelsior Pass Plus, no personal health
information can be viewed by the organizer.*
• No personal information from the app is collected or stored by
the NYS Excelsior Pass Scanner app. The only personally
identifiable information visible is an individual’s name and birth
date, which is necessary for verification purposes.

*If a SMART Health Cards–compliant verifier application is being
used other than the NYS Excelsior Pass Scanner app, you will also
be able to see health information consistent with information on an
individual’s CDC Vaccination Card or test results.

When getting ready to scan an Excelsior Pass or Excelsior Pass Plus
with the NYS Excelsior Pass Scanner app, it is all about execution.
All preparations and training have taken place and now it is time
to execute accurately and consistently.
Communications • Ensure there are clear signs and markers (See
Excelsior Pass Educational Assets) to remind
individuals of the process and requirements for using Excelsior
Pass and/or Excelsior Pass Plus.
Operations • Cross-reference each valid Excelsior Pass and/or
Excelsior Pass Plus
and alternative forms of proof with a photo ID to verify name and
date of birth.
• Notify individuals if their Excelsior Pass and/or Excelsior Pass
Plus is invalid or expired, as indicated by the NYS Excelsior Pass
Scanner app, using your pre-planned protocols. In these cases, an
alternate form of proof, such as a paper form, will be required for
entry.
Technology • Ensure your staff has access to a tablet or mobile
device with the downloaded NYS Excelsior Pass Scanner app.
• Be prepared with backup devices and troubleshooting support for
common issues to avoid interruptions during the check-in
process.
• Establish a separate space away from the main queue to
troubleshoot any technology issues.
