Negros, Philippines February 2017 Dr John Pearman There is a world–wide health crisis occurring. It is insidious and “falls below the radar” most of the time, but it is causing massive health problems on a huge scale. Sugar, eight times more addictive than cocaine, is causing not only problems of rampant decay, but bringing infection , pain, and loss of function to children and adults alike, but also general health problems on a frightening scale. Over 14 days in February our team examined and treated some of the poorest sections of the large population on the island of Negros, Philippines. We assessed and treated patients in Bacolod City and in the region of Sipalay 150 kms. west of Bacolod. The combination of poverty, lack of education on oral hygiene and poor diet, coupled with an over abundance of sugar, is having a devastating effect on the oral health and consequently the general health of all the communities we treated, the most evident being shown in the health of the children. The diet of all communities we visited was heavily based on sugar…. Everything is sweet. The bread, even the spaghetti. We witnessed kids of 5,6 and 7 snacking on sweetened coffee creamer in sachets. Two Australian dentists, 2 Australian physicians, 6 nurses and some very willing volunteers travelled to Negros on a RAWCS (Rotary International) inspired medical/dental mission. The mission was well organized and we worked with local physicians and dentists to provide some very needed treatment for several disabled communities ….. the poorest of the poor was in Bacolod and

Sipalay.Foradentistwhohaspracticedfornearly40yearsinouraffluentsocietytoseean8yroldwithbothlowerfirstpermanentmolarsdecayedtogumlevelwithdrainingabscessesisconfrontingtosaytheleast. Patientswereassessedandtriagedbyregisterednursesandsenttothedentistsorphysiciansforfurtherassessmentandtreatment.Allpatientswereassessedforphysicalsuitabilitytotoleratedentalextractionspriortohavingtreatment.Duringthistime600patientswereassessedand200proceededtohavedentalextractions.Initiallywehadhopedtoinstigateapreventivebasedprogramwiththeuseoffissuresealantsinappropriatesituations.Theuseoftopicalfluorideswasalsoconsidered.However,thesepreventivemeasureswerequicklyabandonedwhenthescopeoftheproblemwasrealized.InthepoorercommunitiesofSipalay,childrenof8,9and10routinelypresentedwith

rampantcarieswithmanyteethinurgentneedofremoval.Everyonetreatedwasgivenatoothbrush/toothpastepackwithappropriateoralhealthinstructionanda“brochure”showingbasicoralhygiene.Inall,2,500oralhealthpacksweredistributed.

Intheareaswevisited,manyofthe“homes”didnothavecleanrunningwater.Onevillagewassoincrediblyheartwrenching.ItwasavillagelocatedinMansilinganthatwaspopulatedbypeople,placedinthisvillagebythegovernment,whoweresightaffected,sometotallyblind.Becausethereisnogovernmentsubsidiesforthesepeopletheyareperiodicallygivenabagofrice,thrownoverthefencetokeepthemfromstarving. Inthepoorerareasitbecamemanifestlyevidentthatitisthroughtheschoolsthatthemaximumeffortoforalandgeneralhealtheducationneedstobefocused.LocalPhilippineRotaryClubshaveaddressedtheproblemoframpantcariesinyoungchildrenandhaveprovidedrunningwaterinsomeofthepoorerschools,whichenablestheyoungchildrento,atleast,washtheirhandsandbrushtheirteethwhileatschool.The

provisionofpotablerunningwaterwillhavefarreachingpositiveeffectsontheoralhealthandindeedthegeneralhealthoftheschoolchildren,andthereforetheRotaryClubsinvolvedaretobecommendedfortheirforesightandeffortstoimprovethelivingstandardsoftheseimpoverishedcommunities.

ItisindeedfortunatethatourAustralianRotaryClubs,IllawarraSunriseandGlenhaven,whoprovidedprofessionalandnonprofessionalsupportforthemission,hasaverycloseliaisonwiththeRotaryClubofMetro-Bacolod. ThisverydedicatedlocalRotarygroupprovidedinvaluablelogisticsupport,andprovidedmuchneededlocalknowledge,whichaddedtotheeffectivenessofthe effortsofthemedical/dentalteam. Duringour14dayexpeditionwehadthegreatfortunetohelpmanymanypeople.Belowisanaccountofsomeofourobservations • Allpatientswereverystoicandgratefulforthesmallamountofrelief

beforeweretreatedmedicallyanddentally.• Thelackoforalhygienewasevenmoreevidenthereinavillageof

samesystemwasputintoaction….(TriagebyRN’swherevitalsignsandpresentingsymptomsassessedandthenreferraltophysiciansordentists).

• Lackofcleanpotablewater,lackofbasicsanitationandhygienecombinedwithanoverabundanceofhighsugarcontentdietmadeforsomecatastrophicpresentationstophysiciansanddentists:thiswasparticularlyevidentinthechildren.

• Thelackofanyradiographicaidtodiagnosiscombinedwithnofacilityforsurgicalretrievalofcariousorinfectedrootssometimesmeantanalgesic/antibiotictherapytheonlytreatmentprovided.

• Anythoughtofpreventionthroughplacementoffissuresealantswasabandonedwiththeroutinepresentationofgrosslydecayedandinfectedteeth.

• ThefissuresealantsandfluoridematerialsbroughtfromAustraliawasgiventolocalgovernmentdentistsalongwitha3Mcuringlight.Thisgiftwasgreatlyappreciated.
