The data for Physical Abuse Treatment Outcome Project:
Application of Parent-Child Interaction Therapy (PCIT) to
Physically Abusive Parents, have been given to the National Data
Archive on Child Abuse and Neglect for public distribution by Mark

ABSTRACT
This project was a randomized clinical trial designed to test
the efficacy and sufficiency of Parent-Child Interaction Therapy
(PCIT) in preventing re-reports of physical abuse among physically
abusive parents entering the child welfare system. One hundred ten
physically abusive parents and their abused children were randomly
assigned to one of three intervention conditions: 1) A modified
PCIT-based parenting program, 2) The same modified PCIT-based
program plus individualized wrap-around services, and 3) a
community parenting group program that is representative of current
standard care in the field. Pre- and post-test measures of the
family were taken. Areas of assessment included depression,
severity of abuse and neglect, internalizing and externalizing
problem behaviors, child abuse potential, parent drug and alcohol
disorders, parent-child interaction, parent and child intelligence,
child’s perception of parent behaviors, and parent’s willingness to
change behavior. A demographics questionnaire was also administered
and a review of child maltreatment reports was conducted.

Purpose of the Study
The present study was designed to answer three research
questions. First, can an adapted form of PCIT reduce physically
abusive behavior relative to a standard community-based
abusive-parent group parenting program? Second, will the addition
of individualized wrap-around services enhance the outcomes of
PCIT? Third, if PCIT is more efficacious than a standard
community-based abusive-parent group parenting program, are the
benefits consistent with the PCIT model which would predict that
increased benefits would be mediated by changes in parent-child
interaction patterns? It was hypothesized that PCIT would prove
more efficacious, that the addition of wrap-around enhancements
would improve outcomes, and that relative reductions in future
physical abuse due to PCIT would be mediated by changes in
parent-child interaction patterns, rather than by non-specific
treatment factors such as reduced parent distress.

Study Design
This study consisted of a sample of families referred to the
study by the county welfare office. All cases had a recent
confirmed report for child physical abuse. A randomized design was
used whereby families were randomized into one of three groups: 1)
A modified PCIT-based parenting program, 2) The same modified
PCIT-based program plus individualized wrap-around services, and 3)
a community parenting group program that is representative of
current standard in the field. Data collectors were unaware of
treatment condition assignments. Pre and Posttest measures were
taken. Please read the final report for more study design
information.

Unit of Observation
The unit of observation for the DS100 file is the family. The
unit of observation for the DPICS Pretest and Posttest files is the
rater’s behavior coding for each of the three video-taped
situations.
Sample
A total of 300 cases were referred to the project by the county
child welfare office. All cases had a recent confirmed report for
child physical abuse. Referrals were eligible for the study if: 1)
both the abusive parent and abused child were available to
participate together in treatment, and no legal termination of
parental rights or abdication of parenting role had been initiated,
2) the abusive parent had a minimum measured K-BIT score of 70, 3)
the child was between four and twelve years old, 4) the identified
parent did not have a child welfare report as a sexual abuse
perpetrator, and 5) the parent and child appeared able to
participate in the intervention. Basic demographic information was
collected from the referring child welfare worker on all referrals.
Of those referred, 112 were enrolled as participants and randomized
into one of three treatment groups. Two families were removed from
the data analysis based upon the opinion of the data collector that
the participant did not comprehend the assessment questions and
could not provide valid data (N= 110). Non-abusive parents or
parent’s partners, and non-abused or non-index siblings were
included as collateral participants in the interventions, but did
not provide data.
Data Collection Procedures
A baseline assessment was conducted that included review of the
index child welfare investigation and all prior child welfare
reports, administration of self-report measures or structured
interviews, and observational coding of a structured parent-child
interaction. Following baseline assessment, parent-child dyads were
randomly assigned to one of three parenting intervention
conditions. All three intervention conditions were structured and
required approximately 6 months to complete. Ongoing service
utilization was tracked, and after program completion (or at 6
months if participants dropped

out of treatment prematurely) the assessment package was
repeated. Data collectors were uninformed about treatment condition
assignment. Follow-up for detected child maltreatment outcomes was
obtained from the statewide child welfare administrative database.

Response Rates
Thirty-seven percent of the cases referred by the child welfare
office were enrolled into the study.
Sources of Information
Data were collected through a review of the index child welfare
investigation and all prior child welfare reports, administration
of self-report measures or structured interviews, and observational
coding of a structured parent-child interaction. Data about child
maltreatment outcomes were obtained from the statewide child
welfare administrative database.

Measures
ABUSE DIMENSIONS INVENTORY (ADI): The ADI (Chaffin, Wherry,
Newlin, Crutchfield, & Dykman, 1997) is an ordinal measure of
the severity of sexual and/or physical abuse across several
dimensions (e.g., type and extent of abuse, duration, relationship
between the abuser and victim, use of force or coercion, and
reaction of the abuser). Severity rankings were developed based on
a national survey of professionals working in the field of child
abuse and the instrument has been shown to have high inter-rater
reliability. The research assistants completed the ADI based upon a
review of written CPS investigative information and/or an interview
with the CPS worker. Inter-rater reliability for the ADI was
monitored by randomly selecting 10% of the sample for re-coding by
an independent rater. The mean inter-rater reliability for ADI
scores for the current study was 0.76. SPECIAL NOTE(S): Contributor
provided the scores for this measure, but did not archive the
individual items. Two ADI files are included. The referral event
file represents information provided by the referring CPS worker.
The "all CPS Records" file represents codings based on a review of
the present as well as all prior CPS investigation records.
BECK DEPRESSION INVENTORY (BDI): The BDI (Beck, Ward, Mendelson,
Mock, & Erhaugh, 1961) is a well known and widely used 21-item
self-report measure of depressive symptoms. Higher scores indicate
more severe symptoms of depression, but not a diagnosis of
depressive disorders. This measure has been found to be a highly
reliable and valid indicator of depressive symptoms and sensitive
to changes in those symptoms over time and treatment. The
identified abusive parent completed the BDI. SPECIAL NOTE(S):

BEHAVIOR ASSESSMENT SYSTEM FOR CHILDREN (BASC): The BASC
(Reynolds and Kamphaus, 1992) is a set of instruments for rating
behavior, thoughts, and emotions of children ages 4 to 18 relative
to standardized age and gender referenced norms. It provides
measures of adaptive as well as problematic behaviors, includes
specific age-appropriate items, and compares information from
multiple sources (self, parent, and teacher report) of a variety of
internalizing and externalizing behaviors: aggression, anxiety,
depression, disruptive behavior, etc. Internal consistency and
temporal stability of the scales ranges from the mid .70’s to the
low .90’s. The identified abusive parent completed the
parent-report version of the BASC. The identified abused child
completed the child-report version of the BASC. If possible, a
teacher-report version of the BASC was obtained from the child’s
most recent classroom teacher. SPECIAL NOTE(S): Contributor
provided the scores for this measure, but did not archive the
individual items. All scores are t-score outputs from the BASC
scoring program.
CHILD ABUSE POTENTIAL INVENTORY (CAP): The Child Abuse Potential
Inventory (Milner, 1986) is a widely used 160 item agree/disagree
format questionnaire developed to estimate risk for committing
child physical abuse. The CAP Abuse Scale, which is the main
interpretative scale of the CAP, has been reported to have high
internal consistency (KR-20 = .92 - .95), a one-month test-rest
stability of 0.83, and good discriminant and future predictive
validity (Milner, 1986). Normative values for the Abuse Scale are
slightly less than 100 (Milner, 1986). Both English and Spanish
language versions were made available to participants. The
identified abusive parent completed the instrument. The instrument
has logically-derived validity scales to detect positive response
bias, although it is not clear that invalid profiles actually void
the validity of the test.
CHILD MALTREATMENT REPORTS: As authorized by participants’
written consent, the study was able to obtain past and future
records of maltreatment reports made to the state CPS system.
Computer database matches were executed using the unique identifier
assigned to the identified abuser and the family within the CPS
database. Referrals that were classified as screened-out or
ruled-out without subsequent assessment or investigation were
excluded, and reports were aggregated across incidents, dates, and
children in order to remove duplications. Separate aggregations
were executed for each major maltreatment category (i.e., physical
abuse reports, neglect reports, sexual abuse reports). Both past
reports (i.e., prior to study enrollment) and future reports (i.e.,
after study enrollment) were gathered. Each report was examined
manually to insure that the match was exact for the family and
reported abuser. Any reports made by the study service providers
(i.e., surveillance effect) were removed (N = 5). All surveillance
effect reports occurred in cases that also had other re-reports
from different sources. Thus, removing surveillance effect reports
altered the time to re-report, but did not effect cumulative
re-report rates.
CHILD NEGLECT INDEX (CNI). The CNI (Trocme, 1996) is an easy to
use instrument yielding an overall severity score for child
neglect. Adequate temporal stability and correlation with lengthier
instruments have been reported. A research assistant completed the
CNI based upon a review of written CPS investigative information
and/or an interview with the CPS worker. SPECIAL NOTE(S):

DEMOGRAPHIC QUESTIONNAIRE: Captured basic demographic
information, health related behaviors (e.g., use of tobacco, drugs,
alcohol), lifestyle characteristics (e.g., car and phone ownership,
geographic stability, time spent watching television, telephone
use, etc.), and social relationships (e.g., participation in social
activities, involvement with family). The questionnaire was
available in English and Spanish. An early version of the
questionnaire was pilot tested on 100 parents in similar programs,
and items answered inconsistently or indicated by parents to be
confusing were corrected. For ordinal or ratio level data items,
the mean two-week test-retest correlation was 0.74, and was 0.79
for nominal-level items , excluding items with no expected
variability (e.g. gender, date of birth) and items which might not
be expected to be stable (e.g., recency of last drink of alcohol).
The identified abusive parent completed the instrument. SPECIAL

DIAGNOSTIC INVENTORY SCHEDULE (DIS) ALCOHOL AND DRUG MODULES,
AND ANTISOCIAL PERSONALITY DISORDER MODULE: The DIS (Robbins,
Helzer, Croughan & Ratcliff, 1981) is a structured diagnostic
interview based upon DSM diagnostic criteria and designed for
epidemiological research. DIS self-reported alcohol and drug
disorders have been found to prospectively predict risk for
maltreatment (Chaffin et al., 1996). We modified the modules for
use as a self-report instrument, a modification which past studies
have suggested yields valid results (Kovess & Fournier, 1990).
The DIS alcohol disorders module has been used as a criterion
measure against which other alcohol measures are tested, and is
substantially correlated with other alcohol measures and with
clinician diagnoses. (Goethe & Ahmadi, 1991; Goethe &
Fischer, 1995; Watson, Detra, Fow, & Ewing, 1995). The DIS drug
disorders have high agreement with other diagnostic measures of
drug disorders (Hasin & Grant, 1987). SPECIAL NOTE(S): DIS
scoring uses the original DIS diagnostic algorithms. These may not
correspond exactly with DSM-IV criteria.
DYADIC PARENT-CHILD INTERACTION CODING SYSTEM (DPICS-II): The
DPICS-II is a structured observation of parent-child interactions
during a three-part task: child-directed activity, parent-directed
activity, and clean-up from the activity (Eyberg, Bessmer, Newcomb,
& Edwards, 1994). It contains codes for parent behavior
(commands, praises, etc.), child behavior (compliance, disruptive
responses), and parents’ responses to child’s behavior. Inter-rater
and test-retest reliability as well as discriminant validity
between referred and non-referred children are satisfactory.
Interactions were videotape recorded, then coded by a research
assistant who was blind to study condition. Coders were trained to
criteria using a standard videotape stimulus set before coding any
study data, and their coding checked periodically by the trainer to
prevent drift.
THE KAUFMAN BRIEF INTELLIGENCE TEST (KBIT): The KBIT (Kaufman
& Kaufman, 1990) is a brief (about 15 to 30 minute)
individually administered screening measure of verbal and nonverbal
intelligence for people ages 4 to 90. It provides a measure of
crystallized thinking and fluid thinking, with two sub-scales:
Vocabulary and Matrices which correlate highly with more
comprehensive tests of cognitive functioning. The KBIT was
administered as an eligibility screen for the identified abusive
parent and child. SPECIAL NOTE(S): Contributor provided the scores

PARENT PERCEPTION INVENTORY (PPI):
The PPI (Hazzard et al., 1983) is designed to assess a child’s
perception of positive and negative
parent behaviors. It contains 18 items, 9 items ask about
positive behaviors and 9 items ask about
negative behaviors. The instrument uses a five-point scale
ranging from "never" (0) to "a lot (5)."
Child participants completed the inventory for each parent. The
identified abusive parent completed a
similarly structured inventory about their child/household.
THE STAGES OF CHANGE READINESS AND TREATMENT EAGERNESS SCALE
(SOCRATES):
The SOCRATES (Miller and Tonigan, 1996) is brief structured
measure based on Prochaska &
DiClementi’s (1983) transtheoretical approach to measuring a
client’s willingness to change behavior.
The measure has been used in research on smoking cessation and
maintenance of other health
behaviors and has been shown to be predictive of treatment
compliance. Internal consistency and
temporal stability indices range from the .80’s to the low
.90’s. For this study, the measure was
adapted as a questionnaire reflecting motivation to change
parenting behavior in general and changing
the use of hitting as a parenting behavior in particular.
SPECIAL NOTE(S): No factor scores are included, instrument lacks
an established scoring metric.

Analytic Considerations
All variables in the archived data set are untransformed. Note
that distributional assumptions may not be met and transformations
may be required depending upon the analysis performed. Many
analyses reported in the final study report used transformed
values, typically square root transformations or binomial
transformation.
No multiple imputation data sets are included, although several
were used to compile the final study report. Imputed values in
these data sets will depend upon the imputation model selected,
which will vary with the analysis performed, and will usually
require that variables in the analytic model are also present in
the imputation model.

contributor, including dates, dates of birth, tribal affiliations,
occupation, zip codes, information about living arrangements, etc.
Ages are rounded to the nearest year.
Extent of Collection
This data collection consists of 28 SPSS system data files, a
User’s Guide and Codebook, the project’s final report and
appendices, a copy of the measures used in the study, and a syntax
file for deriving scale scores.
Extent of Processing
NDACAN produced the User’s Guide and Codebook, and created a
single data file that combines the most relevant study
variables.

Data File Notes
The "DS100" file consists of variables from 27 individual data
files. The file was created by including the score variables from
each of the individual files. In instances where score variables
were not archived by the contributor, all variables from those
files were included in the DS100 file. Please note that variables
from the DPICS Pretest and DPICS Post test files were not included
in the DS100 file because of a unit of observation mismatch. The
unit of observation for the DPICS files is the rater’s behavior
coding for each of the three video-taped situations, whereas, all
of the other files’ unit of observation is the family.
Other study raw data collected by the contributor were not
archived, such as data collected to establish inter-rater
reliability, protocol adherence data or data collected for separate
but related projects. Data on inter-rater reliability and protocol
adherence is contained in the final report.
No factor scores are provided for instruments lacking an
established scoring metric.
Two codebooks are provided, one for the DS100 file and one for
the DPICS files.

CODEBOOK: Dataset #100 Variable Information
Each Codebook consists of three sections. The first two sections
are lists of the variables in the file, which are first listed in
the order in which they appear in the data file followed by an
alphabetic listing. The third section contains a detailed
