You will need to provide the following: 1) Property Tax/Lease/Real Estate Closing documents. 2) 2 bills dated May 1, 2015 or later. These can be electric, water, sewer, cable, satel- lite, phone, cell phone, vehicle tax or voter registration. When a student and his/her parent/legal guardian must provide proof of acceptable address. The homeowner must provide the 3 proofs of residence above and an affidavit must be completed and notarized. Affidavits must be renewed yearly. Please call 476- 4600 if you have any questions.
