Goals of Demonstration

Reduce reliance on institutional care

Increase effectiveness and efficiency of the long term care system

Two Programmatic Components

Design and implementation of a transition initiative moving people from institutions to communities and supporting them with community services for 365 days;

Design and implementation of infrastructure change initiatives supporting the State’s capacity to develop high quality care in the community.

Qualified Residences– Assisted Living– Apartment or own home– Group home with fewer than 4 unrelated

persons– Family home

Qualified Services

A qualified service is any service that the State has committed to continue beyond the MFP demonstration year (first 365 days in the community after transition).

Qualified Services will be sustained by one of the following “target waivers” in year 2

– DDS IFS or Comprehensive Waiver

– Elder Waiver

– PCA Waiver

– ABI Waiver

– MI Waiver

MFP Process

Selection Process – Must use MFP application/notice of interest– DDS WL/Olmstead – Contact Siobhan Morgan or Deb Duval

DDS case manager work with MFP transition coordinator to plan transition from institution to HCBS.
