Modern Wound Care

Knowledge of the process ofwound healing as well as the op-portunities for influencing thatprocess – especially with chronicwounds – have expanded signifi-cantly in recent years. Theindividual conditions of different

wound types and stages of woundhealing are increasingly taken intoaccount through, amongst otherthings, the specific selection ofsuitable dressings that areappropriate for the indications andthe healing phase.

The physiological process ofwound healing is a dynamicprocess that follows a complex pat-tern. There are three distinct, butoverlapping phases, each oneleading smoothly to the next and toa certain extent being dependenton one another.

Exudation phase

The exudation or cleansing phasecommences immediately after theinjury is sustained. In clinical terms, this presents as a local inflamma-tion and leads on to pronouncedexudation. During this phase thebody forms blood clots, cleansesthe wound and undertakes mea-sures to ward off infection; da-maged tissue and germs are re-moved (phagocytosis).

Granulation phase

In the granulation or proliferationphase, new cells are generated andthe granulation tissue is formed (akind of temporary tissue for fillingin the injury). Fibroblasts enter thewound in increased numbers, col-lagen synthesis begins, and capil-laries move into the centre of thewound. While all these changes aretaking place, exudation is reduced. The clinical presentation is now of

Epithelialisation phase

In the epithelialisation or regenera-tion phase, the differentiation of tis-sue and epithelialisation is deve-loped further. The wound edgesmove in toward one another andthe wound is gradually coveredwith epithelial tissue growing infrom the edge. As the wound con-tracts and scar tissue is formed, the process of healing is con-cluded.

In the exudation phase, the idealdressing will support the body’sown cleansing mechanisms andwill provide clean conditions inthe wound. Any coats and necro-tic tissue that may develop will besoftened up and can be carriedaway. Any surplus exudate, the de-tritus contained in it, germs andwaste products from metabolismwill be absorbed. Protection

against infections from outside isensured and the wound is keptmoist. The dressing will bechanged at intervals dictated bythe condition of the wound and thedegree of exudation. When dis-

charge is on the heavy side it maybe necessary to change the dress-ing several times a day. If neces-sary, necrotic tissue and purulentcoats will be removed.

In the epithelialisation phase, the ideal dressing will ensurethat the wound area is keptmoist and will promote furtherepithelialisation, offering protectionagainst adverse influences and en-

suring that the wound remainsundisturbed. Depending on thecondition of the wound, the dress-ing may be left on the wound for upto 7 days as physiological dis-charge continues to abate.

reddish tissue with a good bloodsupply.

In the granulation phase, the ideal dressing will provide abalanced, moist wound environ-ment. It will guard against dryingout and mechanical influenceswhile at the same time taking upany surplus discharge and offeringprotection against infection. Thenew granulation tissue reacts very

sensitively and must be protectedfrom adverse influences also whenthe dressing is being changed.By maintaining a moist woundenvironment, the dressing can bechanged without any trauma.

Dressings are changed accor-ding to the condition of the wound.Experience indicates that thedressing may be left on the woundfor several days.

Suprasorb® A Creates clean woundconditions naturally!

Suprasorb A provides cleanwound conditions in heavily ex-uding wounds. With the strongabsorptive capacity of calciumalginate from natural brownalgae.

The exchange of calcium ionsfrom the wound dressing withthe sodium ions from the woundexudate leads to a formation ofgel which binds exudate and

encloses bacteria as well as detritus, which are removedwith the alginate fibres whenthe dressing is changed.

Indications: heavily exuding, superficial and deep wounds inthe exudation and granulationphase, e.g.: decubitus, ulcers ofvarious origins (e.g. venous, arterial), diabetic foot syndrome(DFS), skin donor and skin grafting sites, postoperativewounds, second degree burns.

Cleanses the wound rapidlyand reliably.

Suprasorb A shows its special ben-efits in use:a high capacity to absorb exudate, rapid formation of gel, soft and with excellent tamponing charac-teristics, conforms to every woundbase, binds bacteria into the geland cleanses the wound.

Suprasorb A is available as dress-ing and as rope.

Suprasorb® H Provides reliable support forwound healing!Suprasorb H creates an idealmoist wound environment inlightly to moderately exudingwounds. With the convincing effectiveness of a proven hydrocolloid. The dual-layer wound dressingconsists of a polyurethane filmand an absorbent hydrocolloidlayer. The hydrocolloid layer, which ismade up of absorbent and ex-pandable particles, creates anideal moist environment andstimulates granulation and epithelialisation. On contact

with the wound, the exudate isabsorbed. This causes the hydrocolloid to swell and toform a gel which binds exudateand detritus. This gel formationenables the dressing to moulditself excellently to the woundstructure.

Indications: lightly (Suprasorb Hthin) and moderately exuding(Suprasorb H standard, border, sacrum), superficial, non-infected wounds in all phases ofwound healing, e.g.: decubitus, ulcers of various origins (e.g. venous, arterial), diabetic footsyndrome (DFS), exuding abra-sions, postoperative wounds, second degree burns.
