On April 4, 2005, Mr. Faulkner, age 26, operated a vehicle in an intoxicated condition. 2. On July 11, 2009, Mr. Faulkner, age 31, operated a vehicle while under the influence of alcohol. 3. On March 25, 2011, Mr. Faulkner, age 32, operated a vehicle while under the influence of alcohol. Disposition: 1. On June 14, 2006, Mr. Faulkner pled guilty and received a suspended execution of sentence to the misdemeanor, DWI-Alcohol, in the St. Louis County Circuit Court, case number 2105T-07049. Mr. Faulkner was sentenced to two years' supervised probation, which he completed successfully. 2. On March 31, 2012, Mr. Faulkner pled guilty to the felony, DWI-Alcohol-Persistent Offender, in the St. Louis County Circuit Court, case number 09SL-CR09351-01. On August 22, 2013, Mr. Faulkner withdrew his plea and instead pled guilty to the misdemeanor, DWI-Alcohol-Persistent Offender and received a suspended execution of sentence. He was sentenced to two years' supervised probation, which he is still currently serving. 3. On August 22, 2013, Mr. Faulkner pled guilty and received a suspended execution of sentence to the misdemeanor, DWI-Alcohol-Persistent Offender, in the St. Louis County Circuit Court, case number 11SL-CR06290-02. He was sentenced to two years' supervised probation, which he is still currently serving. Letters of Recommendation: Three (3) – (Attorney, Friend, Director). Carlos Faulkner was admitted into the residential program at Bridgeway Behavioral Health on 6/28/12 for chemical dependency and completed 21 days of treatment on 7/19/12. He participated fully in all program activities and was compliant with all behavioral expectations.
Upon completing the residential program, Carlos was transferred to our University City Outpatient facility for DWI Court, where he will complete Intensive Outpatient Treatment.
Bridgeway Behavioral Health recommends Carlos obtain a sponsor, attend 12-step meetings and abstain from using all mood altering chemicals.

Educator Certification: Explanation of my criminal record/behavior
I am writing this letter to explain the stupid mistakes that I made in my short brief life. I grew up
in a very good household. I didn't grow up poor or unfortunate. Being the oldest of 7 children I
did have to be held to a higher standard as I had my 6 siblings looking up to me and I had to be
an example, a positive one a role model if you will. Unfortunately, throughout my late teens
and twenties, I did make some stupid mistakes that obviously have followed me throughout my
life. I wasn't a good role model for my siblings nor was I doing anything to help my future. I
made foolish decisions. With my DWI's I did suffer from alcohol problem/dependence for a
long time in which since, I have received the proper treatment that I successfully completed
and I still keep an ongoing program through AA. The alcohol problem that I suffered from
stemmed a lot of my problems in my past. I don't want to give excuses but I know that I have
learned from my mistakes and I know that a clean present leads to a clean future and I want to
continue on my road of success and by not making any stupid mistakes that will cost me a job/career or my freedom.

Educator Certification: Why I want to teach in Missouri
I am the oldest of 7 children and ever since I can remember as a child being the oldest, I was
always trying to teach my brothers everything that I had learned from learning how to throw a
curve ball in baseball to throwing and catching a football to martial arts. I was always trying to
teach anyone I came across, friends, siblings, even strangers things I thought were pertinent to
help their lives and give them something to share with others. I believe I was born a teacher. I love helping people, especially kids.
Teaching has always been my passion as my resume states I have a combined 10 years of
teaching with 3 different schools full-time and several different schools in the sub-teaching
arena. I wish to teach in the great state of Missouri because I believe in Missouri Schools. I
believe in the mission statement that Missouri has in place for the education of our youth.
Missouri is one of the better states in the United States when it comes to education, not just in
the public schools which houses some of the best public school districts in the country i.e.:
Ladue School District, Rockwood School District, Blue Springs, Clayton, just to name a few. The
Private Schools are some of the best as well with schools like MICDS, John Burroughs, CBC and
Westminster. I myself graduated from Hazelwood Central in 1996 which at the time was one of and I still believe is one of the top schools in the Suburban North in St. Louis County.
How I got into teaching was very odd. I was substitute teaching in Hannibal, MO at Hannibal
High School. Where I had become one of the more consistent substitute teachers due to my
schedule in college where I attended Quincy University. As I began my class I noticed there was
a woman sitting back of my class room waiting and listening intently. I didn't let it bother me; I
continued to teach as I was given the curriculum at that time to actually teach the lessons.
When the class was over, all my students left and the lady in the back of the room came up to
me and said I was really good at doing teaching this class and she had never seen the students
with so well-mannered behavior and willing to listen and learn. I thanked her and she advised
me that I should think about doing this full-time as I would be really good at it and I was
talented and a great speaker. I proceeded again to thank her and let her know that was not my
intention as I was a business and communication major at Quincy University and I would be
pursuing a career in Corporate America as that is where "the money" is. She quickly dispelled
that notion and said Missouri has one the best retirement systems in the country and does pay
their teachers very well during their time in the classroom prior to retirement. I told her I would
think about it and thanked her again for the compliment. One week later I got a call from the
secretary of HHS saying I was recommended for an open teaching position for 7 & 8 grade
social studies and when would I like to come in for an interview? I informed the secretary that I
had no notion of that position as I did not apply and I do not know who recommended me. She

told me that Dr. Jill Janes (Superintendent of Hannibal Public Schools) had recommended me
for the job and encouraged me to interview. Dr. Janes was the lady that was in my classroom
that spoke to me after my class and was impressed with my teaching abilities. I was floored; I
hadn't realized I had been talking to the Superintendent of Hannibal Public Schools all along.
That gave me the courage and confidence to pursue my teaching career. I then started to look
for teaching positions in St. Louis, MO, where I'm from and I was able to land a teaching
position as a TA at a private school for kids with Autism called Giant Steps of St. Louis. My only
regret is that I didn't pursue my teaching certification upon graduating from college or make it
my major in college. I now know that teaching is my passion and is what I want to do for the
rest of my life and teaching in Missouri is where I want to be.

Educator Certification: What I have been doing since my criminal record
Through my recent bouts and encounters with the law due to my drinking problem I was
admitted to a program that changed my life along with completing successfully an Inpatient
program through Bridgeway Behavioral Health in St. Louis, Missouri. In August 2013 I graduated
DWI court in St. Louis, MO successfully. This program changed my life and had me address the
real issues behind my drinking. I am now faithfully employed; I have been sober for 27 months
(and counting). I have a relationship now with my family. I am now the son to my mother and
father that I knew I could be, the reliable and good role model big brother to the 6 siblings that
I have and devoted and healthy father to the 7 year old daughter and 5 year old son that I have.
I have my family back and my life back. I attend AA 3 times a week and I chair twice a month. I
also give back by speaking at the treatment facility center that I attended in 2012, once a
month. I do this to not only keep me sober, but to show that I appreciate the help that was
given to me that saved my life and to help others to realize that it is not over, they can beat this
disease and gain their life back through honesty, hard work, seeking a higher power, attending
meetings and just simply not picking up.
My weekly schedule includes working 8-5 Monday through Friday and every other Saturday. I
am an Insurance agent for a great company. Even though I earned an undergraduate degree
from Quincy University in Communications & Business Administration in 2001, I have obtained
my Missouri State License in Property & Casualty and Life & Health in 2012. I attend 3 weekly
meetings at AA once during the week and twice on the weekends. I serve at my church Grace
Church-St. Louis in the traffic ministry and student ministries twice a month (every other
Sunday morning) I also spend time my kids on the weekends attend their weekend activities on
the days that I don't work and during the week. With my schedule, it is full with positive and
diligent hard work that not only puts food on the table for my family but allows me to be a
productive member of society.
With this information, I hope the Educator Certification Department can find it in their hearts to
review this carefully and to find mercy with me and grant me a substitute teaching certification
thus being able to allow me to get back into my passion which is teaching. In my youth, I was
stupid and made dumb mistakes. As I am wanting and do accept responsibility for my actions.
Having this hanging over my head disabling me from not being able to properly teach in the
State of Missouri has been disheartening, but I am ready to move forward and by your grace be
allowed to teach going forward in the State of Missouri.
I am pleased to have an opportunity to write a letter of recommendation for Carlos Faulkner. I have known Carlos for almost 10 years and met him when he was working as a teaching assistant for the Ladue School District at my daughter's elementary school.
Carlos is an extremely personable professional with a passion for teaching and working with children. His pleasant demeanor was a hit with parents in the after school program, and he had a way of bringing out the best in the children. Many times, I would see him working with children on homework and engaging them in the process of learning. When he spoke, children listened. As a member of the Ladue Education Foundation, I am very involved with the administrators for our District and can attest to the support that Carlos has from several parents and leaders to further his career in the teaching field in Missouri.

Carlos has done an outstanding job of building and maintaining relationship with not only the students but also their families. He takes the times to get know the students and goes above and beyond expectations to find ways to help them at whatever level they are. He believes in being an advocate for the students to make sure that each one's needs are met. He promotes discipline but in an appropriate way that the students learn to admire and respect. This letter is in response to a request from Carlos Faulkner to provide a recommendation regarding his work as an educator at Spoede Elementary School in the after school BASK (before and after school care) program. During his years with BASK, he would arrive after school from his daytime work with autistic students at Giant Steps.
As the Director of the Spoede BASK program, I was grateful for Carlos' enthusiasm and interest in the children's welfare and progress. He had a professional demeanor and a variety of rich experiences to enhance his work with over 45 children in kindergarten through fifth grade. It was natural for Carlos to work with a group of boys and girls outdoors or in the gym, coaching and supervising active games.
I observed Carlos mediating disagreements between children, disciplining with fairness when it was required, and comforting anyone who might have been injured by another player. Having a positive male role model like Carlos helped to balance the school day, which was weighted towards women teachers and administrators.
It was Carlos' desire to work as an educator of children, and he expressed interest in qualifying as a certified teacher. I am happy to recommend him as an outstanding man of integrity and a sincere heart. His years working in BASK provided opportunities to demonstrate maturity, consistency, dependability, communication, compassion, empathy, and leadership.
