MINI FIXATOR SURGICAL TECHNIQUE AND PRODUCT DESCRIPTION

Descrip t onThe MINI FIX Hand Fixator consists of two or more t tanium blocks for the xat on of the threaded Pins. The glidingblocks are adjusted to the desired length on a threaded rod. MINI FIX I and II also permit secondary distrac t on orcompression. Joint mo t on tan be excluded permi ed. MINI FIX Ill is slim enough to interdigital applica t on, wherebyt ssue protec t on is guaranteed by its smooth design.

Preopera t ve PlaningFor op t mal xat on, careful preopera t ve planning is essen t al. Hereby injuries of the nerves and the vessels have tobe avoided. Where joints are mobilized, lateral posi t oning of the xator is necessary. If bone gra f ing is applicated,

all necessary measures should be implemented in advance. The Pa t ent must be informed of treatment demandsand poten t al complica t ons. An image intensi er is usually necessary to assure correct applica t on. Certain fracturePa erns require addi t onal xat on with screws, a tension band or a K-wire.

Indica t ons (examples)- Fractures of the sha f , simple or comminuted- Intraar t cular fractures- Compound fractures- Trauma or tumor - related bone defects of the carpus, metacarpus or phalanges- Arthrodesis- Distrac t on osteogenesis (i.e. lenthening of the thumb)- Metacarpal Segment transport- Callotaxis lengthening of the capitate a f er resec t on of the lunate bone (for treatment of Kienbock3 disease, StageIII- Fractures of the mandibula

Postopera t ve Care

The majority of injuries tan be stabilized an an ambulatory se ng. Postopera t vely wound are covered by a lightdressing. A f er three days dressings are changed or removed. The entrance Points of the pins should be cleanedwith soap water. Pin tract infec t ons may be ini t ated by frequent manipula t on of the entrance Points.

The frequency of X-ray controls should be based on the disloca t on risk of the stabilized fracture.

Pin tract infec t ons require oral an t bio t cs and the interrup t on of ac t ve or passive mo t on. Occasionally wideningof the entrance Points by incision is advisable. Apparent loosening requires pin removal. Reinser t on is seldomnecessary, as disloca t on usually occurs at the end of the xat on period. Primary consolida t on takes place within3 - 6 weeks, depending on the involved bone type and the extent of so f t ssue trauma. The MINI FIX can usually bere-moved without local anaesthe t cs. Further physiotherapy is needed, to op t mize func t onal out-come.
