“The Providence Center’s
integrated primary and behavioral
healthcare model is not only changing
and saving lives, but also shaping the future of
the delivery of mental and physical health services nationwide.”
Treating the mind as something separate from the body
has never made sense.
At The Providence Center, we are committed to treating the
whole person and have created a robust, innovative primary and
behavioral healthcare system that treats mind and body as one and
meets our clients’ needs with care and compassion.
Person-centered care is at the core of our commitment to
integrated healthcare. We believe that with proper services our
clients, regardless of daily life challenges, can and do recover.
We know we can’t do the important work we do alone and strive
to establish partnerships that strengthen our organization and
make it possible for our clients to receive the unique primary, mental health and substance use care they
need. Providence Community Health Centers (PCHC) at north Main is just one example of our many
partnerships.
located at our Adult services site at 530 north Main street,
PCHC at north Main is a full-service primary care center that
represents our commitment to integrated healthcare. The first
of its kind in Rhode island, this center gives Providence Center
clients easy access to medical care and makes it convenient for
our behavioral health doctors, nurses and clinicians to collaborate
and coordinate treatment quickly and efficiently with primary
care providers. our partnership with PCHC and the partnerships
we have established with other community organizations, like
the Greater Providence YMCA and shri Yoga studio in Pawtucket,
have helped to ensure that our clients remain active participants
in their recovery as we work to address their behavioral health,
physical health and overall well-being.
The Providence Center’s integrated primary and behavioral healthcare model is not only changing and saving
lives, but also shaping the future of the delivery of mental and physical health services nationwide.
We hope you enjoy reading “Mind & Body: The synergy of Care,” our Fiscal Year 2011 annual report, and
learning about the amazing outcomes our clients have achieved through the integration of primary and
behavioral healthcare services at The Providence Center.

InShape/InShape Seniors InShape and InShape Seniors are research-based wellness
programs that bring together community partners and
specialists in the areas of health, exercise and
nutrition to provide one-on-one physical fitness
and mental health support for adults of all ages
experiencing mental health and substance use
problems. Health mentors work with clients to
create individualized wellness plans aimed
at reaching each person’s personal fitness and
nutritional goals.
InShape Seniors is specifically designed to address the physical health needs of people 65
and over. The program places an emphasis on decreasing isolation and addressing physical
health issues like high blood pressure, obesity, arthritis and diabetes.
“As a health mentor and personal trainer,
I work one-on-one with my clients
teaching them how to live healthier lives
through exercise and good nutrition. To
see my clients’ self-confidence, strength
and overall health improve and know that
I’ve played a part in changing their lives is
a very humbling and rewarding experience.” Daniel Hann, health mentor specialist and A.C.e.
certified personal trainer
“Since I started working out with Dan just three months ago, my depression
and anxiety have gotten so much better. I’ve learned how to eat healthier
and lost 15 pounds! I’m getting out of the house more and taking classes
at The Providence Center on healthy cooking and nutrition to keep me on
track with my weight loss. This program has truly changed my life.”David, a Providence Center client since 2000
“Recently I saw a woman who works with me
one-on-one and participates in my aerobics class
taking the initiative to exercise all by herself. I couldn’t
help but notice how much stronger and happier she’d
become since I started working with her just a few months ago.
My clients are a joy to work with. Teaching here is so rewarding.”
Heather Beckwith, health mentor and A.F.A.A. certified group exercise instructor
“I have arthritis in my hands. Before I started exercising with
Heather, my hands would tremble so much that I could not hold a
cup or pen. Now, I can hold a cup; my writing is not as shaky; and
best of all, my hands don’t hurt as much! Joining this class was the
best thing I could ever have done to help myself.”
Ana, a Providence Center client since 2010
Community Treatment Team NursesPrimary care medical nurses are part of a team of professionals dedicated to serving
Providence Center clients experiencing severe and persistent mental illnesses.
These nurses help their patients to understand physical health issues like diabetes,
obesity, heart disease and high blood pressure and guide them in selecting primary
care doctors and wellness programs designed to promote a healthy lifestyle.
“I help my patients to understand their illnesses, teach them how they can help themselves and check on them regularly to be sure they’re following through with treatment. Having a medical nurse and medical doctor on-site at The Providence Center is convenient for them and an excellent way to coordinate their care. It’s a dream come true for everyone.” Vilma Guevara, R.n.
“Whether it’s medical help or support from my case manager, The Providence Center is always there for me. The InShape yoga and water aerobics classes have given me more energy and helped me to feel better. I feel cared for and supported at The Providence Center. The people here are my family.” Gil, a Providence Center client since 2003
Imagine is a BrightStars 4-star program led by credentialed, knowledgeable educators recognized locally and nationally for exceptional teaching.
“Andrew is an active, growing boy who would rather be running around playing with friends than taking time to eat a nutritious meal or snack. At preschool, Andrew’s teachers talk a lot about the importance of healthy eating and taking time to recharge your body. My husband and I are very pleased with Imagine’s approach to learning. It reinforces everything Andrew is learning at home.” Jessica, Andrew’s mom
“Nutrition in preschool is all about learning how to eat healthy and making healthy choices. We do a lot with ‘always’ versus ‘sometimes foods.’ Always foods are foods that are good for you like veggies and whole grains. Sometimes foods, like candy and doughnuts, may taste good but are not always a good choice. The earlier we help children develop healthy habits, the more likely they are to carry them throughout their lives.”simmy Carter, liCsW, R.n., associate director of the early Childhood institute and director of imagine Preschool
Imagine PreschoolImagine Preschool is a positive place for
children ages three to five years old to learn and grow.
The curriculum is designed to prepare children to succeed in
school by instilling in them a love of learning, an excitement
about school and the skills and knowledge that will form the
foundation for a lifetime of learning. Imagine is committed to
helping children develop healthy habits through fun and play.

adults experiencing mental health and substance use problems. Patients
benefit from a team approach to treatment and easy access to all programs

“I’ve been a client at The Providence Center for a very long time. The
people here really care about me and want the best for me. That’s
why they connected me to this health center. Dr. Chan is helping me
to deal with my high blood pressure and diabetes. It’s so much easier
to be able to get my physical and mental health problems taken care
of in one place.” labe, a Providence Center client since 1977
“We use a team approach to primary care and mental health
treatment. Being able to confer regularly with each patient’s
case manager ensures that I have full knowledge of the mental
health history of my patients. The shared medical record gives me
immediate access to my patients’ records and medication history, so
I can safely prescribe medications and make an informed diagnosis.
Being on-site at The Providence Center has been an invaluable
benefit for our patients.”Maria Cristina Chan, M.D., a Providence Community Health Centers primary care

to change behavior in order
to positively impact our

“When the economy tumbled, my life tumbled along with it. I lost
my job, my home—everything. I sent out over 500 resumes but
didn’t get a single response. I had no choice but to move in with
my mother. I was about as low as you can get when I first started
seeing Anna, my therapist at the health center.
Anna let me cry and realize it was okay to be angry. Step by step
she helped me fight my way back to reality. Today, I have a job, a
beautiful apartment—a new life. Anna stuck by me. She showed
me the way back.” Jean, a former client of Anna Braz
Anchor Recovery Community CenterThe Anchor Recovery Community Center creates a
“one-stop shopping” experience that provides easy
access to a wide variety of supportive services
that help individuals, families and communities
build a strong foundation for recovery. Anchor is a
safe haven that offers hope, health and healing to
individuals and families affected by the disease
of addiction.
For people in recovery, adding a yoga practice to their recovery journey is beneficial
as a relaxation technique and as a way to restore health and balance to the body. Shri
Studio in Pawtucket has created an opportunity for the recovery community to explore
yoga through a partnership with Anchor Recovery Community Center.
“My goal is for my students to take something away from the mat they can use in their daily lives. Whether it is a calming breathing technique or a new awareness of how their bodies respond, I invite students to welcome the new spiritual or emotional insights they experience during the class.” Kate Hawley, instructor of Yoga
in Recovery at shri studio, has
been in recovery for almost
three decades.
Services at Anchor include:
• Peer-facilitated support
• Coaching and mentoring
• 12-Step groups
• Stress management, spirituality/
meditation, expressive arts and
wellness
• Career planning
• Access to computers and technology
• Alcohol and drug-free activities
The Providence Center sponsors the operations of Anchor Recovery Community Center.
“When I leave my yoga
class, I feel calm and
relaxed.
Yoga has taught me how
to meditate and get in
touch with my spiritual
core. It’s given me
something to focus
on other than my
addiction. Yoga has
played an important
role in my recovery
journey.”nick, a member and volunteer
at Anchor Recovery Community
Center
“I’m able to stay on track with my recovery because of all the
support I receive at The Providence Center. In the women’s
addiction program, we work on relapse prevention, talk about the
importance of eating healthy and exercising and even practice
yoga once a week. The Yoga in Recovery class at Shri gives me the
opportunity to further my yoga practice and strengthen my recovery
by helping me to cope with chronic pain and anxiety.”Michelle, a Providence Center client since 1998 and member of Anchor Recovery

Child and Family Services
Planning for Rhode Island’s first recovery high school began with legislation passed by the Rhode island General Assembly allowing for the establishment of a pilot recovery high school for students diagnosed with a substance use disorder. The Providence Center was instrumental in advocating for the passage of this legislation. The recovery high school will provide a safe haven for students to remain in school while starting on their road to recovery.
Imagine Preschool was named a four-star program by Brightstars, a quality rating and improvement system for Rhode island child care programs. The rating was based on an evaluation of six areas: child’s daily experience, teaching and learning, staff-to-child ratio and group size, family communication and involvement, staff qualifications and program management.
The Early Childhood Institute (ECI) provided consulting services to seven daycare centers with 45 classrooms and over 800 children through the Child Care support network. Mental health consultants worked with daycare providers to better understand children’s behavior and improve supports for their social and emotional development.
Through funding from the American Recovery and Reinvestment Act, the eCi began The Incredible Years, an evidence-based parent group program designed to improve parenting styles and foster parent-child relationships. The eCi was also chosen to participate in a pilot project for Brightstars and received additional funding from the state of Rhode island to provide consulting services to english-speaking home daycare providers.
The Providence Center School implemented Fit2Bhealthy, a program to help children achieve better physical and behavioral health, into the school curriculum. school-wide groups and activities focus on substance use awareness, nutrition and fitness.

InShape, a research-based wellness program, was implemented to decrease clients’ behavioral health symptoms, reduce chronic disease and improve overall well-being. The program later expanded to include InShape Seniors, funded by the Tufts Foundation, for people over 65 with behavioral health needs.
