She is a Board-certified infectious disease physician who has a research and clinical background in managing infections and antibiotic resistant pathogens in post-acute and long-term care settings. In this presentation, Dr. Stone will provide an examination of Legionella, define elements of a nurse home water management program and helpful strategies for implementation. In light of the recent Flint Water Crisis, this presentation will provide a timely overview of associated risk factors of Legionella and how to ensure safe water for your residents.

ops & tactics track: 105 HEART FAILURE READMISSIONS: THE FREQUENT FLIERS THAT DON’T FLY FOR FREE: PART 1 OF 2Julie Savage | Capital Internal Medicine Associates HospitalistsNearly 6 million Americans are currently living with heart failure, and close to 700,00 new cases are diagnosed yearly costing insurance companies millions of dollars. Since the passage of the Affordable Care Act in 2012, hospitals are incentivized by CMS to improve quality care. One common method is through better use of post-acute care facilities, yet one in four patients discharged to a SNF is readmitted to the hospital within 30 days. This presentation will discuss the financial cost to both hospitals and SNF’s regarding readmitted HF patients. Participants are encouraged to participate in Part 2 of this program, “Heart Failure: Clinical Management Guidelines” as well.

This session will present the Management of Heart Failure in Skilled Nursing Facilities with focus on unique issues of SNF care related to guidelines and recommendations.

But is that really the case? In this fast-paced presentation, participants will be asked to examine the myths and the truths about Millennials, as well as what strategies provide greater insight into recruiting and retaining what is now the largest workforce age cohort in America. Beyond how you need to attract and keep them in your organization, the program will also explore what Millennials worry about, how they see their role in the world, what they expect in work culture and their commitment to work/life balance.

clinical track:112 SUSPECT SEPSIS. SAVE LIVES.Joyce Turner | Ciena Health CareAfter completion the participant will be able to enhance overall quality of patient care, improve nurses’ knowledge and reduce 30-day hospital readmissions for patients with infections leading to Sepsis using the

“SIRS” screening criteria for early identification and interventions. The program is relevant for all licensed nurses. It is appropriate for the novice nurse who is fine tuning their assessment skills, as well as an excellent review for the most experienced nurse as it is applied to their own nursing practice.

workforce track:113 HIRING & RETAINING QUALITY NURSES IN SENIOR CARE Lou Ann Brubaker | Brubaker ConsultingIt is heartbreaking when tremendous energy has been invested in recruiting senior care staff and then termination or resignation is the outcome. When this happens it puts the financial, regulatory, occupancy and customer delight goals of the facility at risk. In this 90-minute program the goals of hiring great staff and retaining them is distilled into clear strategies that can be immediately implemented by participants. Attendees will learn to interview potential hires more successfully as well as examine how their current orientation strategies may be contributing to a revolving door. The motivators that cause staff to staff in the long term will be addressed, as well as which of four categories all staff fall into and what to do with each type.

clinical track:114 NON-PHARMACOLOGICAL PAIN MANAGEMENT: EXPERIENCING THE BENEFITS OF TAI CHI/QIGONGFred Goff | The Art of Transition LLCQigong and Tai Chi are Chinese energy exercises that are a series of smooth, gentle, flowing physical movements, which combines rhythmic breathing and circular motion with focused intention. The unique feature of both Qigong and Tai Chi, is the internal cultivation of mental strength and tranquility. When done on a consistent basis, Qigong and Tai Chi have been shown to help strengthen muscles, increase flexibility, increase coordination, and help practitioners gain control of their lives by achieving mental and emotional peace.Almost anyone can learn Qigong or Tai Chi, regardless of his or her physical condition. The exercises can be done in a standing or seated position. The degree of exertion, similar to walking, can be easily adjusted to the participant’s ability. Participants are encouraged to wear loose-fitting clothes and a pair of soft-sole shoes or tennis shoes for interactive participation in this “live” class, appropriate for all levels of physical capabilities.

workforce track:115 COACHING YOUR NURSING TEAM TO INCREASED PERFORMANCELou Ann Brubaker | Brubaker ConsultingThe operational demands upon senior care leaders can often mean that daily coaching of staff for higher (and better) performance gets short shrift, with substantial impact on staff morale. The survey environment especially can affect managers’ focus on “catching people doing things right” versus wrong. In this session, participants will explore how informal coaching can be done more effortlessly, spontaneously and meaningfully. Within formal coaching discussions the writing of performance goals will be described in respect to staff evaluations and improvement in deficient performance.

EARLY REGISTRATION DEADLINEThe discounted early registration rate is available to current NADONA members only, whose registration is received by HCAM by Friday, Sept. 22, 2017.

REGISTRATION CONFIRMATIONAfter your registration form has been processed, an event confirmation will be emailed to your attention from [email protected]. Please be sure to include a current email address on this form.

PAYMENT & APPLICABLE RATEMembership will be verified for all registrations indicating NADONA membership status. All registration fees are due at the time of registration.

CANCELLATION & REFUND POLICYAll refund requests must be made in writing by e-mail to [email protected]. Telephone cancellations will NOT be accepted. Individual registration fees, less a 25% processing fee, will be refunded for cancellations received in writing prior to Sept. 22. Cancellations received after Sept. 22 and “no-show registrants,” will NOT receive a refund nor a credit to a future program.

The Michigan Chapter NADONA/LTC in exclusive partnership with the Health Care Association of Michigan (HCAM), is pleased to offer the only official national Infection Preventionist Board Certification Review Course for IP-BC Certification for skilled nursing/post-acute care clinical professionals to obtain their nationally recognized NADONA IP-BC certification. Completion of this course meets the new CMS requirements for Infection Preventionist training and satisfies CDC guideline requirements as well. This review course will provide health care professionals with evidence based resources and clinical recommendations for infection prevention and control for long-term care settings in preparation for obtaining this specialized certification.This certification review course is recommended for long-term care/post-acute care nurses (including RN & LPNs).
