To make things as easy as possible you are able to print this document and bring it to the conference, there are pages that you can print and complete for registration to the event and also to contribute to the Dream Team Awards to recognize the people within in your organization that routinely go above and beyond their job description. This year we have invited the Hospice Aides to join the rest of the IDT as just one step in embracing partnerships. You will notice that the concurrent sessions are named as TRACKS. It is possible to jump around the agenda, follow one TRACK or several. Whenever you see the INTENDED AUDIENCE is ALL that means it is a session that will benefit the entire IDT. MICHIGAN HOSPICE & PALLIATIVE CARE ORGANIZATION
2012 Annual Conference Key Note Speaker Judi Lund Person
Judi Lund Person MPH, Vice President, Regulatory and State Leadership from NHPCO. There is no
doubt you will recall Judi as she has been a keynote speaker at MHPCO in the past and holds a position
with NHPCO much like a trail blazer as you will learn below from her experiences documented at
National.
“A recognized national leader in the field of hospice and end of life care, Lund Person works with local
hospice providers and state hospice organizations on regulatory issues, compliance, data collection
and strategic planning. Lund Person spearheaded the creation and launch of NHPCO’s Quality
Partners, a national initiative designed to build organizational excellence and improve hospice and
palliative care delivery and outcomes.
Lund Person lends her regulatory expertise as a key contact with the Centers for Medicare and
Medicaid Services, interfacing with payment policy, survey and certification and program integrity
functions, among others. She also represents hospice and palliative care with other federal agencies
and many national organizations, including the Food and Drug Administration, the Pain Care Forum,
the National Consensus Project for Quality Palliative Care and the Hospice and Palliative Care Coalition.
She graduated with honors with a BA degree in Sociology from the University of North Carolina at
Greensboro and a Master’s degree in Public Health from the University of North Carolina at Chapel
Hill.”
MICHIGAN HAOSPICE & PALLIATIVE CARE ORGANIZTION
2012 Annual Conference Key Note Speaker Dr. Scott Shreve
Dr. Scott Shreve is the National Director of Hospice and Palliative Care Program for the Department of Veterans Affairs. He is responsible for all policy, program development, staff education and quality assurance for palliative and hospice care provided or purchased for enrolled Veterans. Dr. Shreve leads the implementation and oversight of the Comprehensive End-of-Life Care Initiative to change the culture of care for Veterans at end of life and to ensure reliable access to quality end of life care. Clinically, Dr. Shreve commits half of his time to front line care of Veterans as the Medical Director and teaching attending at a 17 bed inpatient Hospice and Palliative Care Unit at the Lebanon VA Medical Center in Central Pennsylvania. Dr. Shreve is an Associate Professor of Clinical Medical at The Pennsylvania State University and has been awarded the Internal Medicine Distinguished Teaching Award in 2007 and 2009. Dr. Shreve has board certifications in Internal Medicine, Geriatrics and in Hospice and Palliative Care. Prior to medical school, Scott was a corporate banker.
The Mission of VA Hospice & Palliative Care is personal for Dr. Scott; “To Honor Veterans’ preferences for Care at the End of Life”. In 2011VA was recognized for their efforts to improve care for the dying. At this event Dr. Scott Shreve was quoted as saying that VA “has transformed its health care system to include reliable access to quality palliative care.”
He added: “The We Honor Veterans campaign provides the venue for partnering and capacity building. This is important, since the model of hospice care for Veterans often includes VA care, as well as care provided in the Veteran’s home by a community hospice.”
