Linking key groups to service reform?

• Addressing the needs of those who have complex

needs or experience the poorest health outcomes in

our communities is essential.

• Different expectations and experiences of health

services exist - often culturally or socially

determined, including stigma.

• Engagement requires identification of appropriate

conduits to engage with consumers and communities

• Actions require tailored activities and targeted

services.

What we know about our region ?

• People with mental illness do not always experience

coordinated care when needed.

• Some people are admitted to hospital or remain in

our hospital when they could be better supported in a

less restrictive environment in the community.

• We understand many people with alcohol and other

drug dependence also have a mental illness.

What we know about our region ?

• Effective coordination of services is required for

people with comorbid mental health and alcohol and

drug issues as well as for those who experience

comorbid physical health issues.

• We recognise people with alcohol and other drug

dependence are vulnerable, have complex needs

and require additional specific strategies

complementary to those identified for people with a

mental illness.

Key Strategies

• Be leaders in delivering evidence based quality

care to people with mental illness and/or alcohol and

other drug dependence.

• Increase access to recovery focused mental health

and alcohol and drug services available in the

region.

• Elevate the focus on physical health,

psychological and social wellbeing to support

consumers and carers in their recovery journey.

Key strategies

• Work with partners to increase and facilitate access

to a broader range of whole of life services, including

community based alternatives to hospital admission

and provision of meaningful vocational opportunities.

• Be leaders in quality health care and service

activities by delivering extensive and inclusive

education, training and research programs.

Commitment through Action

• Provide alternatives to hospital admission and

support recovery of consumers through additional

step up/step down facilities across MNHHS.

• Build on existing relationships with emergency

services in joint responses to persons who may be at

risk/in crisis, including Co-Responder models,

implemented in priority areas of need.

Commitment through Action

• Collaborate with partners to develop and implement

service models and associated care pathways for

inpatient services that meet the needs of older

people with a mental illness.

• Strengthen community resources particularly in

the northern part of MNHHS to improve service

responsiveness to people with mental illness and

people with alcohol and other drug dependence.

• Expand perinatal mental health services across

specialist community & inpatient services.

withdrawal management for adults & young people.

do together to improve quality, coordination and

integration (not just a plan about what the PHN will

do)

• Developed jointly by Brisbane North PHN and Metro

Blue Sky – March Symposium

Participants

• Over 70 stakeholders including people with a lived experience

attended

Process

• 1 additional feature recommended (9 10)

• A total of 82 actions suggested to achieve features and move

us from where we are now to where we want to be

Blue Sky – post workshop

With stakeholder feedback from the symposium and other

consultation activities, the PHN MHAOD team:

• Revised vision statement

• Developed the 9 features 10

• Developed consistent structure and language for all features

(i.e. A community where…took the focus off the system and

made it about people)

• All 82 actions were analysed and themed

• 10 common themes found across all 82 actions

• Based on themes, developed 10 ‘system enablers’ to achieve

the features of the vision

• Consolidated 82 actions into 47 actions across each ‘system

enabler’
